Desloratadine - the active substance of Desloratadine Canon, being the primary active metabolite of loratadine, is one of the long-acting antihistamines. Its effect is based on inhibition of the cascade of reactions of allergic inflammation: the release of pro-inflammatory cytokines (including interleukins IL-4, -6, -8 and -13); release of pro-inflammatory chemokines (RANTES); production of superoxide anions by activated polymorphonuclear neutrophils; adhesion and chemotaxis of eosinophils; release of adhesion molecules (such as P-selectin), IgE-mediated release of histamine, leukotriene C ₄ and prostaglandin D ₂… Thanks to the use of the drug, the development is prevented and the course of allergic reactions is facilitated. Desloratadine has an antipruritic and anti-exudative effect, helps to reduce capillary permeability, prevent smooth muscle spasm, tissue edema.

The substance does not affect the central nervous system, has practically no sedative effect (does not cause drowsiness), does not cause prolongation of the QT interval on the ECG, does not affect the speed of psychomotor reactions.

After oral administration, the effect of Desloratadine Canon develops within 30 minutes, its duration is 24 hours.

Pharmacokinetics

Desloratadine is well absorbed in the gastrointestinal tract. In blood plasma after oral administration is determined after 30 minutes. The average time to reach the maximum concentration of a substance is 3 hours. Desloratadine does not penetrate the blood-brain barrier. Binds to plasma proteins at the level of 83-87%. When used once a day at a dose of 5–20 mg in adolescents and adults for 14 days, there is no clinically significant cumulation of the drug. Food or grapefruit juice does not affect the distribution of desloratadine.

Desloratadine is not an inhibitor of CYP3A4 and C YP2D6, nor is it an inhibitor or substrate of P-glycoprotein. It undergoes intensive metabolism in the liver through hydroxylation, with the formation of a metabolite - 3-OH-desloratadine, combined with glucuronide. Only a small part of the dose is excreted by the kidneys and through the intestines (less than 2 and 7%, respectively). The half-life averages 27 hours (range 20 to 30 hours).

Indications for use

• allergic rhinitis: Desloratadine Canon is used to eliminate / relieve sneezing, nasal congestion, redness and itching of the eyes, watery eyes, nasal mucus, itching of the palate and nose;
• urticaria: used to reduce / eliminate itching, rash.

Contraindications

Absolute:

• pregnancy and the period of breastfeeding;
• age up to 12 years;
• hypersensitivity to the components of the drug.

Desloratadine Canon tablets are taken orally, regardless of food intake, preferably at the same time of day. The tablet must not be chewed, it must be swallowed whole and washed down with water.

The recommended dosage regimen for adults and children: 5 mg (1 tablet) per day, the duration of treatment depends on the severity of symptoms.

Recommended schemes for the use of Desloratadine Canon:

• intermittent seasonal allergic rhinitis (manifestation of symptoms no more than four days a week or their total duration no more than four weeks a year) and urticaria: an assessment of the course of the disease is required, if the symptoms disappear, Desloratadine Canon is discontinued, and if they reappear, they are resumed;
• persistent perennial allergic rhinitis (onset of symptoms more than four days a week or a total duration of more than four weeks a year): therapy can be continued throughout the exposure to allergens.

The duration of the course of therapy should be determined by the attending physician.

Side effects

Most often, in the course of clinical studies in adult patients, such adverse reactions were observed as headache - 0.6%, dry mouth - 0.8% and increased fatigue - 1.2%. In children and adolescents from 12 to 17 years old, complaints of headache were most often recorded - 5.9% (when taking placebo, this figure was higher - 6.9%).

Overdose

The use of Desloratadine Canon in a dose 5 times higher than the recommended dose did not lead to the appearance of any symptoms. During clinical trials, the daily use of the drug at a dose of up to 20 mg for 14 days was not accompanied by clinically or statistically significant changes in the cardiovascular system. Taking a daily dose of 45 mg (9 times higher than the recommended dose) for 10 days did not lead to a prolongation of the QT interval and the appearance of serious side effects.
