Hepatrombin C is a combined drug for external use with an antithrombotic and tissue regeneration enhancing effect.
Release form and composition
The drug is produced in the form of a gel for external use 500 IU + 150 mg + 25 mg / g: transparent, colorless, homogeneous, with a characteristic odor (40 g in an aluminum tube, sealed with a screw-on plastic cap, in a cardboard box 1 tube and instructions for use of Hepatrombin C).
1 g of the preparation contains:
- active substances: sodium heparin - 500 IU (international units) *, dimethyl sulfoxide - 150 mg, dexpanthenol - 25 mg;
- additional components: carbomer 940, isopropanol, trolamine, macrogol glyceryl hydroxystearate, lemon oil, rosemary oil, mountain pine oil, purified water.
* 1 IU of sodium heparin - anticoagulant activity, which corresponds to the activity of 0.007 to 7 mg of the II International Standard of Heparin (WHO).
Pharmacological properties
Pharmacodynamics
The action of the gel is due to the properties of the active substances included in its composition:
- sodium heparin: is a direct-acting anticoagulant showing anti-inflammatory effect as a result of inactivation of biogenic amines and blocking of lysosomal cleavage enzymes in tissues; enhances local blood flow; in the area of subcutaneous capillaries accelerates the dissolution of microthrombi; by suppressing the activity of hyaluronidase, it promotes the regeneration of connective tissue; when used locally, it helps to eliminate puffiness;
- dexpanthenol (provitamin B 5): has anti-inflammatory and dermatoprotective effects; enhancing metabolic processes, stimulates regeneration; providing an improvement in epithelialization and granulation, helps in the restoration of damaged tissues;
- dimethyl sulfoxide: has anti-inflammatory, local anesthetic, analgesic and antimicrobial effects; exhibits moderate fibrinolytic activity; passes through mucous membranes, skin, biological membranes, contributing to a deeper distribution of other ingredients of the drug in tissues.
Pharmacokinetics
When used externally, sodium heparin quickly penetrates the skin. The level of this active substance 1 hour after the application of Hepatrombin C to the skin in the underlying tissues is comparable to the level that is observed after an intravenous injection of 5000 IU of heparin. It does not cross the placenta and is not detected in human milk.
Dexpanthenol, after being absorbed into the skin, is converted into vitamin B 5 - pantothenic acid, which is an integral part of coenzyme A, and also participates in numerous metabolic processes.
Dimethyl sulfoxide quickly passes through mucous membranes, skin, biological membranes (including the membranes of microbial cells). After application to the skin, the maximum plasma level is determined after 4–8 hours. Dimethyl sulfoxide is characterized by a uniform distribution in the body, except for the brain, where it is determined in minimal concentrations. It is excreted mainly in the urine, with 30% of the dose in the form of an unchanged drug and in the form of dimethylsulfone, in a small amount through the intestines and lungs. The metabolite of the active substance - dimethyl sulfide, in some patients leads to the appearance of a characteristic garlic odor from the mouth.
Indications for use
- hematomas, inflammation of muscles, soft tissues, ligaments, tendons, tendon sheaths after compression, bruises, injuries (including sports);
- epicondylitis of the shoulder (tennis elbow);
- humerus periarthritis;
- tendovaginitis, tendinitis;
- deforming osteoarthritis (with existing lesions of the periarticular tissues);
- bursitis;
- neuralgia.
Contraindications
- severe hepatic and / or renal failure;
- bronchial asthma;
- severe general atherosclerosis, myocardial infarction, severe angina pectoris, stroke and other severe cardiovascular diseases;
- lupus erythematosus;
- age up to 5 years;
- pregnancy and lactation;
- open wounds in the area of application of the gel;
- hypersensitivity to any active substance or auxiliary component of Hepatrombin C.
Hepatrombin C, instructions for use: method and dosage
Gel Hepatrombin C is for external use only.
The drug is recommended to be used 1-2 times a day for 7-10 days or until the symptoms of the disease, such as inflammation, pain, edema, are completely removed. The gel should be applied in a thin layer to the affected areas of the skin or areas located above the painful area, avoiding the use of the product directly on abrasions and wounds. Before the procedure, the treated skin area must be cleaned and dried. It is not recommended to apply a bandage after applying the gel.
If after 10 days of using the gel it is not possible to achieve a clear improvement, you need to consult a doctor.
Side effects
Against the background of drug therapy, the following side effects may occur: a change in taste, the smell of garlic from the mouth, allergic reactions (angioedema, urticaria), itching, burning, hyperemia at the site of application.
Overdose
Until now, no cases of drug overdose have been recorded. If the gel is used in accordance with the above recommendations, an overdose is unlikely to occur.

The gel should not be applied to open wounds.

Application during pregnancy and lactation
It is contraindicated to use Hepatrombin C gel for pregnant and breastfeeding women.
Pediatric use
Hepatrombin C is contraindicated for use in children under 5 years of age.
With impaired renal function
Severe renal failure is a contraindication for the use of Hepatrombin C.
For violations of liver function
Severe liver failure is a contraindication for the use of the drug.
Drug interactions
The dimethyl sulfoxide in the gel contributes to a significant increase in the penetration of other medicinal substances through the skin. Before and after the application of Hepatrombin C, it is not recommended to apply other drugs to the skin (to prevent the appearance of undesirable reactions of the latter).
