Hemofer is an antianemic drug.
Release form and composition
The drug is produced in the form of drops for oral administration: yellow-green, transparent, with a characteristic odor (10 or 30 ml each in dark glass dropper bottles, 1 bottle in a cardboard box and instructions for using Hemofer).
The composition of 1 ml drops includes:
- Active substance: ferric chloride - 157 mg (including iron (II) - 44 mg);
- Auxiliary components: citric acid, sodium benzoate, sodium hydroxide, dextrose, sorbitol, flavor, purified water.
Pharmacological properties
Pharmacodynamics
Hemofer is an antianemic drug containing ferrous iron in the form of a simple salt of iron (II) chloride. Designed for the treatment of iron deficiency states.
Iron in the body is required to ensure its normal functioning. It is a part of hemoglobin, cytochrome C, myoglobin. Stimulates erythropoiesis, participates in redox reactions and reversibly binds oxygen.
Iron chloride helps to replenish the existing iron deficiency in the body, and also in the case of insufficient iron intake, counteracts the appearance of its deficiency during the period of increased demand.
Pharmacokinetics
Absorption of iron ions occurs in the upper parts of the small intestine, mainly in the duodenum. The absorbed iron in the form associated with ferritin is stored mainly in the liver. It is incorporated into hemoglobin in the bone marrow.
Iron absorption is determined by the valence, acidity of the gastric contents and iron deficiency in the body. Ferrous iron preparations are easily absorbed and do not require an initial reduction.
In the presence of a significant deficiency, the degree of iron absorption is approximately 20%. With a decrease in the severity of the deficiency, iron absorption also decreases.
Indications for use
- Clinically expressed iron deficiency anemia (treatment);
- Latent iron deficiency (treatment);
- Iron deficiency in women of childbearing age, during pregnancy and during breastfeeding, as well as in children, adolescents and adults, including elderly patients and when following a vegetarian diet (prevention).
Contraindications
- Hemochromatosis;
- Aplastic anemia;
- Fructose intolerance, sucrase / isomaltase deficiency, glucose-galactose malabsorption;
- Inflammatory diseases of the digestive system;
- Excess iron in the body (including hemosiderosis);
- Anemias caused by disorders of iron utilization (lead, sideroachrestic anemia);
- Anemias not associated with iron deficiency (megaloblastic, hemolytic anemia);
- Hypersensitivity to drug components.
Hemofer should be used under medical supervision in the presence of the following conditions / diseases:
- Enteritis;
- Ulcerative colitis;
- Peptic ulcer of the stomach and duodenum.
Hemofer, instructions for use: method and dosage
Hemofer drops are taken orally, with juice or water, between meals.
The dosage regimen is determined by the doctor individually. As a rule, appoint:
- Premature babies: the daily dose is 1-2 drops (1.5-3 mg / kg);
- Children under 1 year old: daily dose of 10-20 drops (15-30 mg);
- Children 1-12 years old: single dose - 30 drops (45 mg), frequency of administration - 1-2 times a day;
- Children over 12 years of age: single dose - 30 drops (45 mg), frequency of administration - 2 times a day;
- Adults: single dose - 55 drops (88 mg), frequency of administration - 1-2 times a day.
The maximum daily dose for the treatment of anemia is: children - 3 mg / kg, adults - 180-200 mg.
For prophylactic purposes, the dose of Hemofer should be reduced by 2-3 times.
The duration of the course is at least 2 months. The hemoglobin level in clinically significant iron deficiency is normalized only 2-3 months after the start of therapy. After the normalization of indicators for the restoration of iron stores in the body, the drug should be taken for several months in preventive doses.
The duration of the course in premature babies varies from 3 to 5 months.
The daily dose can be taken in one or more doses.
1 ml (30 drops) of Hemofer contains 157 mg of ferric chloride (44 mg of elemental iron), 1 drop contains about 1.5 mg of elemental iron.
Side effects
During the use of Hemofer, disorders of the digestive system can be observed, manifested in the form of diarrhea, lack of appetite, abdominal pain, nausea, flatulence, constipation.
Overdose
The main symptoms are: decreased blood pressure, constipation / diarrhea, epigastric pain, vomiting, nausea; in more severe cases - necrosis of the mucous membrane of the gastrointestinal tract, pallor, drowsiness, hypovolemic shock up to coma.
Features of the development of signs of an overdose after taking Hemofer:
- peripheral circulatory collapse: 30 minutes;
- convulsions, metabolic acidosis, leukocytosis, fever, coma: 12-24 hours;
- acute hepatic / renal necrosis: 2–4 days.
Therapy: cancellation of Hemofer, gastric lavage, use of laxatives, symptomatic treatment.
The recommended method of using deferoxamine, depending on the severity of the poisoning:
- severe: intravenously slowly at 15 mg / h for children and 5 mg / kg / h (up to 80 mg / kg per day) for adults;
- lung: intramuscularly 1000 mg every 4-6 hours for children and 50 mg / kg (up to 4000 mg per day) for adults.
special instructions
It is not recommended to use Hemofer for patients with impaired digestion and absorption in the gastrointestinal tract.
Due to the increased iron content in the intestine during the period of taking Hemofer, the stool may be colored darker. When staining (darkening) teeth, it is recommended to take the drug through a straw or rinse your mouth immediately after using it.
The composition of Hemofer includes glucose, which must be taken into account when prescribing it to patients with diabetes mellitus.
During the period of therapy, it is recommended to monitor the concentration of iron and hemoglobin in the blood once a month. After the normalization of the hematological parameters, Hemofer should be used for another 1.5-2 months.
Application during pregnancy and lactation
Given the established iron deficiency, the use of Hemofer during pregnancy and breastfeeding is reasonable and safe.
Drug interactions
With the simultaneous use of Hemofer drops with some drugs, the following effects may occur:
- Antacids: decreased absorption of iron;
- Tetracyclines: decreased absorption of antibiotics and iron.
