Mebendazole is an anthelmintic agent derived from benzimidazole. Leading to irreversible disruption of glucose utilization, Helmindazole depletes glycogen stores in helminth tissues, prevents the synthesis of cellular tubulin, and also slows down the production of adenosine triphosphate (ATP).
Pharmacokinetics
Mebendazole is practically not absorbed in the intestine. With continuous oral administration for three days 2 times / day at 100 mg, the plasma content in the blood of the active substance and its metabolite, a 2-amino derivative, does not exceed 0.03 and 0.09 μg / ml, respectively.
Mebendazole binds to plasma proteins by 90%. It is distributed unevenly throughout the organs, accumulates in the liver, adipose tissue, helminth larvae.
Metabolic transformation of mebendazole occurs in the liver with the formation of a 2-amino derivative, which does not exhibit anthelmintic activity.
The half-life is 2.5–5.5 hours, over 90% of the substance is excreted unchanged through the intestine. The absorbed part (about 5-10%) is eliminated by the kidneys.
Indications for use
- ankylostomiasis, ascariasis, strongyloidiasis, trichinosis, enterobiasis, teniodosis, trichocephalosis - in the case of monoinvasion or mixed helminthiasis;
- echinococcosis - if surgical treatment is impossible.
Contraindications
- Crohn's disease, ulcerative colitis;
- liver failure;
- glucose-galactose malabsorption, lactase deficiency, lactose intolerance (since the product contains lactose);
- pregnancy, lactation period;
- age up to 3 years;
- combined use with phenytoin, ritonavir, metronidazole, carbamazepine;
- hypersensitivity to mebendazole and / or any auxiliary component of Helmindazole.

Helmindazole tablets are taken orally with a little water.
Recommended dosage regimen:
- trichocephalosis, hookworm disease, ascariasis and mixed helminthiasis: for children after 3 years and for adults - in the morning and in the evening, 100 mg (1 tablet of Helmindazole), the course of therapy is 3 days;
- enterobiasis: children after 3 years and adults - once 100 mg / day, course - 1 day. Due to the fact that with this helminthiasis reinfection occurs quite often, the treatment must be repeated after 2 and 4 weeks; all family members should be treated simultaneously;
- strongyloidosis, teniodosis: children after 3 years - in the morning and in the evening, 100 mg (200 mg / day), adults - in the morning and evening, 200 mg (400 mg / day), course - 3 days;
- echinococcosis: adolescents over 14 years of age and adults - the first 3 days 2 times / day, 500 mg, in the next 3 days in the same dose 3 times / day, then the dose is increased to 1000-1500 mg 3 times / day; on average, the course of therapy for echinococcosis excited by Echinococcus granulosus is 4-6 weeks, excited by Echinococcus multilocularis - up to 2 years;
- trichinosis: 200-300 mg 3 times / day on the 1st day and 4 times / day at the same dose on the 2nd day; from 3 to 14 days - 500 mg 3 times / day.
Side effects
- hematopoietic organs: neutropenia;
- nervous system: drowsiness, headache, dizziness, convulsions;
- digestive organs: abdominal pain, nausea, vomiting, diarrhea, increased activity of alkaline phosphatase, hepatic transaminases, hepatitis (against the background of using high doses for a long period);
- urinary system: hypercreatininemia, glomerulonephritis (in the case of prolonged use of large doses of Helmindazole);
- allergic reactions: urticaria, skin rash, exanthema, angioedema, toxic epidermal necrolysis, Stephen-Johnson syndrome, anaphylactoid and anaphylactic reactions;
- others: hair loss (with prolonged use at high doses).
Overdose
Symptoms of an overdose of Helmindazole are nausea, diarrhea, vomiting, abdominal pain. Against the background of a long course in high doses - reversible violations of the liver, neutropenia, hepatitis.
Treatment: evacuation of mebendazole from the stomach by vomiting or gastric lavage, taking activated charcoal and symptomatic treatment.

With prolonged use of Helmindazole, monitoring of the peripheral blood picture, renal and liver function should be carried out.
Patients with diabetes mellitus need to monitor the plasma glucose concentration in the blood.
After completion of therapy, it is necessary to periodically conduct a study of feces and smears of the anal area, since the treatment can be considered effective if, within 7 days after its completion, helminths or their eggs are not found.
After taking Helmindazole, it is forbidden to consume ethanol-containing drinks, fatty foods and use laxatives during the day.
According to the results of studies on the development of Steven-Johnson syndrome and toxic epidermal necrolysis, it is assumed that there is a connection between the occurrence of the observed pathologies and the combined use of mebendazole and metronidazole. As a result, the combined use of these drugs should be avoided.
Influence on the ability to drive vehicles and complex mechanisms
During therapy with Helmindazole, dizziness and drowsiness may develop, therefore, patients who drive vehicles or work with complex equipment should be careful.
Application during pregnancy and lactation
During pregnancy, the use of Helmindazole is contraindicated.
Whether mebendazole is excreted in breast milk has not been established. If it is necessary to take an anthelmintic drug during lactation, breastfeeding must be stopped.
Pediatric use
Children under 3 years of age are not prescribed Helmindazole therapy.
For violations of liver function

Drug interactions
- lipophilic substances: it is not recommended to use simultaneously with mebendazole;
- metabolic inducers (including carbamazepine): the concentration of mebendazole in the blood decreases, its serum level should be monitored;
- insulin: the need for this substance decreases in patients with diabetes mellitus;
- cimetidine: the content of mebendazole in the blood may increase and its effect may increase; the combination requires caution.
