Ganaton is a stimulant for the release of acetylcholine, a drug that increases motility and tone of the gastrointestinal tract (GIT).

Release form and composition

Dosage form of Ganaton - coated tablets: white, round, engraved on one side "HC 803", on the other - dividing risk (7, 10 or 14 pcs. In a blister, in a cardboard box 1, 2, 3, 4 or 5 blisters).

1 tablet contains:

  • active substance: itoprid hydrochloride - 50 mg;
  • auxiliary components: hypromellose, corn starch, carmellose, lactose, titanium dioxide, anhydrous silicic acid, macrogol 6000, magnesium stearate, carnauba wax.

Pharmacological properties

Pharmacodynamics

The active substance of Ganaton is itopride hydrochloride, which enhances the motility of the gastrointestinal tract due to the inhibition of acetylcholinesterase and antagonism of D 2 -dopamine receptors. Activates the release of acetylcholine and leads to its destruction.

Also, the substance has an antiemetic effect (due to interaction with D 2 -receptors, which are located in the trigger zone), activates the propulsive motility of the stomach, causes a dose-dependent suppression of vomiting caused by apomorphine.

Due to the specific effect on the upper part of the gastrointestinal tract, transit through the stomach is accelerated and its emptying improves.

Itopride hydrochloride has no effect on serum gastrin levels.

Pharmacokinetics

The substance is absorbed in the gastrointestinal tract quickly and almost completely. Its relative bioavailability is 60%. This is mainly due to the metabolism on the first pass through the liver. Food has no effect on bioavailability.

With max (maximum plasma concentration) after taking 50 mg of itopride hydrochloride is achieved in 0.5-0.75 hours and is 0.28 μg / ml.

After repeated administration of itopride hydrochloride orally at a dose of 50-200 mg 3 times a day for 7 days, the pharmacokinetic parameters of the substance and its metabolites were linear, and the cumulation was minimal.

Plasma protein binding - 96% (mainly with albumin), binding to α 1 -acid glycoprotein - less than 15% of the total binding.

After oral administration of itopride, active tissue distribution occurs. V d (volume of distribution) is 6.1 l / kg. High concentrations of the substance are found in the liver, kidneys, small intestine, stomach and adrenal glands. Itopride passes into breast milk, penetrates into the brain and spinal cord in a minimal amount.

Active biotransformation of itopride occurs in the liver. Three metabolites have been identified, one of them exhibits insignificant activity, which has no pharmacological significance (about 2-3% of that of itopride). The primary metabolite is an N-oxide formed by oxidation of the quaternary amino-N-dimethyl group.

Metabolism occurs under the action of flavin-dependent monooxygenase (FMO3). Depending on the genetic polymorphism, the effectiveness and number of FMO isoenzymes may differ. Rarely, an autosomal recessive condition known as trimethylaminuria (fish odor syndrome) has developed. The half-life of itopride increases in patients with trimethylaminuria.

Itopride has no inhibitory or inducing effect on CYP2C19 and CYP2E1. Therapy with itopride does not affect the activity of uridine diphosphate glucuronisyltransferase or CYP.

Excretion of itopride hydrochloride and its metabolites occurs mainly in the urine. After a single dose of Ganaton in therapeutic doses, the renal excretion of itopride and its N-oxide in healthy people is 3.7 and 75.4%, respectively. The terminal half-life of itopride hydrochloride is approximately 6 hours.

Indications for use

According to the instructions, Ganaton is indicated for the treatment of gastrointestinal symptoms caused by delayed gastric motility and chronic gastritis (functional non-ulcer dyspepsia): rapid satiety, nausea and vomiting, a feeling of fullness in the stomach after eating, bloating, discomfort or pain in the epigastric region, heartburn, anorexia.

Contraindications

  • gastrointestinal bleeding;
  • perforation or mechanical obstruction of the gastrointestinal tract;
  • age under 16;
  • the period of pregnancy and breastfeeding;
  • individual intolerance to the components of the drug.

With caution, Ganaton should be prescribed with concomitant therapy with other drugs, in elderly patients, in patients in whom the appearance of cholinergic side reactions may negatively affect the course of the underlying disease.

Instructions for the use of Ganaton: method and dosage

Ganaton tablets are taken orally before meals.

The dose of the drug is prescribed taking into account the clinical indications and the patient's age.

Recommended dosage: 1 pc. 3 times a day. The duration of treatment is no more than 2 months.

The daily dose of Ganaton should not exceed 150 mg (3 tablets).

Side effects

  • from the digestive system: abdominal pain, nausea, diarrhea, constipation, increased salivation, jaundice;
  • on the part of the blood and lymphatic system: thrombocytopenia, leukopenia;
  • endocrine disorders: gynecomastia, increased prolactin content;
  • from the nervous system: headache, dizziness, tremor;
  • allergic reactions: itching, rash, skin flushing, anaphylaxis;
  • laboratory parameters: an increase in the level of bilirubin concentration, the activity of gamma-glutamyl transpeptidase, aspartate aminotransferase (ACT), alanine aminotransferase (ALT), alkaline phosphatase.

Overdose

  • warfarin, diazepam, diclofenac sodium, ticlopidine hydrochloride, nifedipine, nicardipine hydrochloride do not cause changes in protein binding;
  • oral medicinal products with a low therapeutic index, or an enteric coating, or sustained release of the active substance may interfere with their absorption;
  • ranitidine, cimetidine, teprenone, cetraxate (antiulcer drugs) do not affect the effect of the drug;
  • anticholinergics can reduce the therapeutic effect of itopride.
