Beloderm Express is a glucocorticosteroid (GCS) for external use.

Release form and composition

The drug is produced in the form of a spray for external use: a viscous solution, colorless, from transparent to slightly opalescent, with the smell of isopropanol (20 or 50 ml in a white polyethylene bottle with a plastic spray and lid, in a cardboard box 1 bottle and instructions for use Beloderma Express).

1000 mg of solution contains:

  • active substance: betamethasone dipropionate - 0.64 mg (0.064%), which is equivalent to betamethasone in an amount of 0.5 mg;
  • additional components: carbomer 934P, isopropanol, sodium hydroxide, water.

Pharmacological properties

Pharmacodynamics

Betamethasone dipropionate is a synthetic GCS that exhibits anti-inflammatory, anti-allergic, anti-exudative, antipruritic and vasoconstrictor effects. After applying the drug to the skin surface, it helps to reduce the release of inflammatory mediators from mast cells and eosinophils, as well as γ-interferon and interleukins (IL) 1 and 2 from macrophages and lymphocytes. The active substance slows down the activity and weakens the permeability of the vascular wall, interacts with specific cellular receptors located in the cytoplasm, initiates the synthesis of matrix ribonucleic acid (mRNA), which is involved in the formation of proteins that mediate cellular effects, including lipocortin. The latter inhibits phospholipase A 2, counteracts the release of arachidonic acid and the biosynthesis of prostaglandins, leukotrienes, endoperoxides (inducing the development of allergies, inflammation and other pathological reactions).

Pharmacokinetics

With the external use of betamethasone in therapeutic doses, its transdermal absorption into the blood is extremely insignificant. Skin diseases, inflammation and the use of occlusive dressings increase the percutaneous absorption of the active substance, which can increase the likelihood of systemic adverse effects. The metabolic transformation of the agent proceeds predominantly in the liver. The drug is excreted in urine and bile.

Indications for use

Beloderm Express is recommended for the treatment of skin diseases amenable to GCS therapy:

  • bullous dermatoses;
  • eczema (all forms);
  • atopic dermatitis / neurodermatitis;
  • allergic contact dermatitis;
  • non-allergic dermatitis (including radiation and solar);
  • contact dermatitis (including professional);
  • erythroderma;
  • exudative erythema multiforme;
  • psoriasis;
  • lichen planus;
  • discoid lupus erythematosus;
  • itching of various origins;
  • photodermatosis (including sunburn);
  • reactions to insect bites.

Contraindications

Absolute:

  • post-vaccination skin reactions;
  • viral (herpes simplex, chickenpox), bacterial (skin manifestations of syphilis, skin tuberculosis) and fungal skin diseases;
  • acne vulgaris, rosacea;
  • trophic ulcers of the leg;
  • open wounds;
  • nevus, hemangioma;
  • atheroma;
  • xanthoma;
  • skin cancer, melanoma, sarcoma;
  • age up to 2 years;
  • lactation period;
  • hypersensitivity to any component of Beloderma Express.

With caution and only if the intended benefit to the mother exceeds the risk to the fetus, the spray can be used during pregnancy.

Beloderm Express, instructions for use: method and dosage

Spray Beloderm Express is intended exclusively for external use.

The drug is recommended to be sprayed 2 times / day on the problem area of the skin in the amount required to completely cover the affected surface. One spray dose released with one press of the dispenser is 0.15 ml, containing 0.075 mg of betamethasone. In order to evenly apply the product to a 100 cm² area of skin, spray 2 ml of spray (equivalent to 1 mg of betamethasone), namely, make 13 full and smooth clicks on the dispenser.

The duration of the course of therapy depends on the effectiveness and tolerability of Beloderma Express and, as a rule, does not exceed 28 days. Repeated courses are possible throughout the year. When using the drug in children and in patients with facial skin disease, the duration of treatment should not exceed 5 days.

If there is no clinical improvement at the end of therapy, you should consult your doctor to clarify the diagnosis.

Side effects

When using Beloderma Express in therapeutic doses, it is usually well tolerated, undesirable effects are mild or absent.

Against the background of drug therapy, as well as with the appointment of other GCS, hypersensitivity reactions may appear in the form of hyperemia, dry skin, burning, itching, acne-like changes (hypopigmentation, steroid acne, perioral or allergic contact dermatitis).

When treating large surfaces of the body with a spray, the development of systemic side reactions of GCS - signs of hypercortisolism, such as glucosuria, hyperglycemia, manifestation of Itsenko-Cushing's syndrome, reversible suppression of the function of the adrenal cortex - can be recorded.

In the case of prolonged use of Beloderma Express, as well as its spraying under occlusive dressings, the following disorders can be observed: secondary infections, prickly heat, maceration / atrophy of the skin, telangiectasia, hypertrichosis, hirsutism, folliculitis, purpura.

If during the treatment period the occurrence of side effects is noted, the drug should be canceled and a specialist should be consulted.

Overdose

Acute overdose with betamethasone is unlikely. Nevertheless, with prolonged or excessive use of the spray, especially in children, on large surfaces of the skin, when treating skin with impaired integrity or using it under an occlusive dressing, a chronic overdose may occur, occurring with clinical symptoms of hypercortisolism.

In this condition, a gradual cancellation of treatment with Beloderm Express and symptomatic therapy is required.

special instructions

Betamethasone in the form of a spray is preferably used for the treatment of acute dermatosis, including those accompanied by the appearance of weeping surfaces (exudation).

In patients with facial skin lesions, it is recommended to use Beloderm Express for no more than 5 days, due to the aggravation of the risk of rosacea, acne and perioral dermatitis.

You should not apply the spray to the eye area due to its possible contact with the mucous membrane, which can provoke the development of fungal eye infections, cataracts, glaucoma, as well as exacerbation of herpes infection.

It should be borne in mind that in such areas of the body as the groin folds and armpits, there is some kind of natural occlusion, which increases the likelihood of striae. As a result, the duration of continuous use of Beloderma Express in these areas should not exceed 28 days.

If, during the period of therapy, the development of a bacterial or fungal infection is detected on the skin, appropriate antibacterial or antifungal treatment is required.

In the case of using the spray on large skin surfaces and / or applying under an occlusive dressing, the risk of suppression of the function of the hypothalamic-pituitary-adrenal system (HPA) and the appearance of signs of hypercortisolism increases, an increase in intracranial pressure and a decrease in the excretion of growth hormone may be recorded.

Influence on the ability to drive vehicles and complex mechanisms

Beloderm Express does not affect the ability to drive vehicles and control other complex mechanisms.

Application during pregnancy and lactation

The use of the drug during pregnancy is allowed only when the expected benefit to the woman outweighs the possible threat to the fetus. In these cases, GCS therapy should be short-term and carried out under medical supervision. Use the spray in pregnant women, if possible, only on small areas of the skin.

If the use of Beloderma Express is necessary during lactation, you must stop breastfeeding.

Pediatric use

In children under 2 years of age, the use of Beloderm Express spray is contraindicated, in children over 2 years of age, it is possible only on strict indications and under the close supervision of the attending physician due to the increased risk of systemic adverse events caused by betamethasone.

When carrying out drug therapy in children, its duration should be minimized and the spray should not be applied under occlusive, warming and fixing dressings, as well as diapers, as this causes increased resorption and absorption and exacerbates the threat of adverse effects.
