Benacort is a synthetic corticosteroids (glucocorticosteroid) for topical application of antiallergic, anti-inflammatory and immunosuppressive action.
Release form and composition
Benacort is available in the following forms:
- powder for inhalation: white or almost white, fine-crystalline (100 or 200 doses in Cyclohaler inhalers, one inhaler in a plastic bag, in a cardboard box one packet with an inhaler);
- solution for inhalation: slightly yellowish or colorless, sometimes slightly opalescent (2.2 ml each in colorless or dark glass bottles, 10 bottles in a cardboard box).
Composition for 1 dose of powder for inhalation:
- active substance: budesonide - 200 mcg;
- auxiliary component: sodium benzoate.
Composition for 1 ml of solution for inhalation:
- active substance: budesonide - 250 mcg or 500 mcg;
- auxiliary components: succinic acid, propylene glycol, nipagin, polyethylene oxide 400, Trilon B, purified water.
Indications for use
- COPD (chronic obstructive pulmonary disease) in case of proven effectiveness of GCS use;
- bronchial asthma (to reduce the dose of oral corticosteroids; in case of insufficient effectiveness of cromoglycic acid and / or bronchodilators; as a means of basic anti-inflammatory therapy).
Contraindications
Absolute:
- status asthmaticus, acute bronchospasm (as a means of emergency therapy);
- open pulmonary tuberculosis;
- non-asthmatic bronchitis;
- viral, bacterial and fungal infections of the respiratory system;
- children and adolescents up to 16 years old;
- hypersensitivity to individual components of the drug.
Relative (Benacort is used with caution):
- osteoporosis;
- hypothyroidism;
- cirrhosis of the liver;
- parasitic infections (including amebiasis);
- glaucoma.
Method of administration and dosage
Powder for inhalation
Benacort in powder form is intended for inhalation use with a portable inhaler "Cyclohaler". The dose of the drug depends on the severity of the disease and the maintenance dose of oral corticosteroids.
The initial daily dose is from 400 to 1600 mcg, the maintenance dose is from 200 to 800 mcg. With exacerbations of bronchial asthma, you can increase the dose to 800-1600 mcg per day. The maximum dose of the drug is 2000 mcg per day. The prescribed daily dose is divided into 2-4 inhalations.
The duration of therapy depends on the severity of bronchial asthma and ranges from 3 to 12 months.
Instructions for use of the Cyclohaler inhaler:
- Remove the protective cover from the inhaler, shake the inhaler.
- Pull the dispenser out to the stop, tap the dispenser base on the palm of your hand, then push the dispenser back into the inhaler body.
- Take a deep breath, then insert the mouthpiece into your mouth, wrapping your lips tightly around it, and take a strong and deep breath.
- Remove the mouthpiece from your mouth, hold your breath for at least 10 seconds. Do not breathe out through the inhaler. If necessary, repeat the procedure with a second breath.
- Close the inhaler with a protective cover.
Inhalation solution
Benacort solution is intended for inhalation use with a nebulizer. One bottle with a solution contains a single dose of the drug.
The initial dose is 1000-2000 mcg per day, the maintenance dose is 500-4000 mcg per day. When a therapeutic effect is achieved, the dose is gradually reduced to the minimum effective one, which contributes to maintaining a stable state. Sometimes, to quickly achieve the desired effect, the drug is used in higher doses.
For inhalation using a nebulizer, you must open the bottle with Benacort and fill the nebulizer with the required amount of solution. Then perform the nebulization procedure (you should follow the instructions for use of a specific type of nebulizer). The solution for inhalation is allowed to be diluted with isotonic sodium chloride solution.
When using a mask, make sure that it fits snugly against the face during inhalation; after the procedure, the face should be washed with water. Also, after each inhalation, it is necessary to rinse your mouth with water, rinse the nebulizer and mask or mouthpiece.
Transfer from GCS for oral administration (including with hormone-dependent asthma) to the inhalation form should be carried out in the stable phase of the disease. The first 10-14 days (with hormone-dependent asthma - 14-20 days) Benacorta inhalation is combined with oral corticosteroids, after which the dose of oral corticosteroids is gradually reduced, if possible, until completely canceled.
Side effects
- digestive system: rarely - change in taste, nausea, esophageal candidiasis;
- respiratory system: dry mouth, candidal stomatitis, irritation of the mucous membranes of the mouth and pharynx, sore throat, hoarseness, cough; rarely - paradoxical bronchospasm (most often with incorrect inhalation technique);
- central nervous system: rarely - depression, nervousness, behavior change, headache, motor restlessness;
- other reactions: rarely - punctate hemorrhages.
When using Benacort in doses of more than 2000 mcg per day, systemic effects of GCS may appear.
special instructions
Avoid getting the powder or inhalation solution in the eyes.
Rinsing your mouth with water after inhalation can help reduce the risk of developing candidal stomatitis.
When transferring a patient with GCS for oral administration to Benacort, with a decrease in the dose of oral GCS, symptoms not associated with asthma (eczema, rhinitis, muscle and joint pain) may appear. In these cases, it is required to temporarily increase the dose of GCS for oral administration. Headaches, vomiting, nausea, and fatigue were rarely observed. After the complete cancellation of systemic corticosteroids, long-term observation of the patient and a regular assessment of respiratory function are required.
Benacort can be prescribed to elderly and elderly patients who cannot use other forms of inhaled GCS.
Drug interactions
Before using budesonide, it is recommended to inhale beta-adrenostimulants, since they expand the bronchi, improve the flow of Benacort into the respiratory tract and enhance its effect.
The effectiveness of the drug decreases when used simultaneously with rifampicin, phenytoin and phenobarbital, and increases when combined with estrogens or methandrostenolone.
Benacort is used with caution simultaneously with ketoconazole, since a noticeable increase in the concentration of budesonide in plasma is possible.
Terms and conditions of storage
Store in a dark, dry place, out of the reach of children, at a temperature not exceeding 25 ° C.
The shelf life is 3 years.
