Oxydevit is a regulator of vitamin-calcium-phosphorus metabolism.

Release form and composition

Dosage form Oksidevita - solution for oral administration in oil 0.0009%: transparent liquid from light yellow to yellow (5 or 10 ml in vials or dropper bottles of dark glass, in a cardboard box 1 bottle or bottle-dropper).

Composition of 1 ml solution:

  • active substance: alfacalcidol - 0.009 mg;
  • auxiliary components: food ionol (butylhydroxytoluene), refined deodorized soybean or sunflower oil.

Pharmacological properties

Pharmacodynamics

Oxydevit regulates the exchange of phosphorus and calcium, compensates for the deficiency of vitamin D 3. The drug is a natural metabolite of vitamin D, formed in the kidneys from vitamin D 3. It enhances the absorption of calcium and phosphates in the intestines, increases bone mineralization, stimulates the synthesis of osteocalcin. Increases humoral immunity, normalizes the function of muscle tissue.

In patients with calcium malabsorption syndrome, Oxydevit restores calcium balance, reduces the intensity of the bone resorption process, thereby reducing the frequency of fractures. In addition, the drug reduces bone pain associated with impaired calcium-phosphorus metabolism.

The action of the drug lasts 48 hours.

Pharmacokinetics

Oxydevit has a high absorption. The maximum concentration of the drug in the blood is reached after 8-18 hours. The active substance binds to alpha globulins. The main part is metabolized in the liver to the active metabolite of calcitriol, a small part of the drug is metabolized in the bone tissue. Oxydevice accumulates in the body. The drug is excreted in equal parts by the kidneys and bile. Its half-life is about 19 days.

Indications for use

  • osteodystrophy (including with chronic renal failure);
  • hypoparathyroidism, pseudohypoparathyroidism;
  • rickets and osteomalacia of various origins;
  • osteoporosis (including postmenopausal, senile, steroid);
  • renal acidosis;
  • Fanconi syndrome (hereditary renal acidosis with nephrocalcinosis, late rickets and adiposogenital dystrophy).

Contraindications

Absolute:

  • hypercalcemia;
  • hyperphosphatemia (with the exception of hyperphosphatemia in hyperparathyroidism);
  • hypermagnesemia;
  • hypervitaminosis D;
  • period of breastfeeding;
  • age up to 3 years;
  • increased sensitivity to alfacalcidol.

Relative:

  • atherosclerosis;
  • nephrolithiasis;
  • pulmonary tuberculosis in active form;
  • granulomatosis;
  • sarcoidosis;
  • heart failure;
  • the presence of stones in the urinary tract and other risk factors for the development of hypercalcemia;
  • pregnancy;
  • chronic renal failure;
  • age from 3 to 18 years.

Instructions for the use of Oksidevit: method and dosage

Oxydevit is taken orally. One drop of the solution contains 0.25 μg of alfacalcidol. Drops are recommended to be taken on a piece of brown bread after meals. Oksidevit is given to children with milk or porridge.

In adult patients, the initial dose is 4 drops per day, then it is increased every 3 weeks by 2 drops to 8-12 drops per day. Maintenance dose - 1-4 drops per day.

When on hemodialysis, the initial dose is 4–8 drops per day, then - 2 drops per day.

The course of taking the drug lasts 60 days and can be repeated 2-3 times a year.

Recommended dosage for various conditions / diseases:

  • condition after kidney transplantation - 1-4 drops per day;
  • bone pathology - 2-12 drops per day, duration of admission - up to a year;
  • osteomalacia with impaired absorption of calcium, phosphates and vitamin D - 1-4 drops per day;
  • rickets-like diseases - 2-12 drops per day, duration of admission - 3 months or longer;
  • rickets - 4 drops per day for 10 days, then repeat the course 2 times at intervals of 14 days.

When using Oksidevit in pediatrics, the maintenance dose is 1 drop per day. Children on hemodialysis are prescribed 4 drops per day in combination with vitamin D 2 (2000 IU) for a long period of time.

Side effects

  • digestive system: nausea, vomiting, anorexia, dry mouth, discomfort, constipation, a slight increase in the activity of ALT, AST;
  • CNS: increased fatigue, weakness, dizziness, drowsiness;
  • immune system: allergic reactions such as skin rash, itching;
  • metabolism: hyperphosphatemia (occurs in patients with severe renal impairment), increased plasma high-density lipoproteins.

Overdose

With an overdose of Oksidevit, hypervitaminosis develops. Its early symptoms are constipation, dry mouth, thirst, polyuria, nocturia, pollakiuria, headache, nausea, vomiting, severe fatigue. In the later stages, bone pain, increased blood pressure, skin itching, cloudy urine, conjunctival hyperemia, arrhythmia, myalgia, pancreatitis, stomach pain, weight loss, drowsiness are possible. With chronic intoxication, the development of renal failure, arterial hypertension, calcification of the tissue of the lungs, kidneys and blood vessels is also possible.

Treatment: it is recommended to cancel the drug until the level of calcium in the blood is normalized, then a half dose of the drug is prescribed. In case of an acute overdose, it is advisable to wash the stomach, then take mineral oil, which reduces the absorption of the drug. If necessary, physiological saline, glucocorticosteroids and diuretics are prescribed.

While taking Oksidevit, regular monitoring of the level of calcium in blood plasma and urine is recommended to avoid the development of hypercalcemia and hypercalciuria.

Alfacalcidol is not metabolized in the kidneys. There is evidence of the effectiveness of the drug in secondary hyperparathyroidism caused by renal failure.

Influence on the ability to drive vehicles and complex mechanisms

There is no data on the effect of Oksidevit on the ability to drive vehicles and the speed of psychomotor reactions.

Application during pregnancy and lactation

The use of Oxydevit during pregnancy is possible only when the expected benefit to the mother outweighs the potential risk to the fetus.

If necessary, the use of the drug during lactation, breastfeeding should be discontinued.

With impaired renal function

Oksidevit should be used with caution in patients with chronic renal failure, as well as with urolithiasis.

Drug interactions

  • drugs that induce the induction of microsomal liver enzymes: reduce the therapeutic effect of alfacalcidol, which requires an increase in its dose;
  • digitalis drugs: the risk of arrhythmia increases;
  • calcium preparations, thiazide diuretics: the risk of hypercalcemia increases;
  • antacids and laxatives: the risk of developing hypermagnesemia increases;
  • antiepileptic drugs, phenytoin, barbiturates: reduce the effectiveness of alfacalcidol;
  • mineral oils, colestipol, cholestyramine, sucralfate, albumin and antacids: reduce the absorption of alfacalcidol.

With laxative dialysis, the risk of developing hypermagnesemia and hyperaluminemia increases.

According to the instructions, Oxydevit should not be taken together with cholecalciferol (vitamin D), as well as with its derivatives.
