Release form and composition
The drug is produced in the form of a powder for oral administration, which is a homogeneous mass of white or yellowish-white color, sometimes compressed into easily disintegrating agglomerates, with a blackberry smell and, in some cases, with a sulfur aroma (1.6 g each in paper sachets, laminated aluminum foil; in a cardboard box 10 or 20 sachets and instructions for use ACTS Active).
Composition for 1 dose (sachet):
- active substance: acetylcysteine - 600 mg;
- auxiliary components: sorbitol, citric acid, magnesium citrate, aspartame, polysorbate 65, xylitol, sodium dihydrogen citrate, sodium carmellose, glyceryl tripalmitate, Blackberry "B" flavor, magnesium stearate.
Pharmacological properties
Pharmacodynamics
Acetylcysteine is a cysteine derivative. It has secretory and mucolytic effects. Sputum discharge is facilitated due to the direct effect on its rheological properties. Free sulfhydryl groups of a substance are capable of breaking intermolecular and intramolecular disulfide bonds of acidic mucopolysaccharides of sputum, and this causes depolymerization of mucoproteins and leads to a decrease in the viscosity of sputum. Acetylcysteine reduces the induced hyperplasia of mucoid cells, stimulates ciliary activity and enhances surfactant production, which leads to improved mucociliary clearance.
The activity of the drug is preserved in the presence of mucous, purulent and mucopurulent sputum.
ACTS Active increases the secretion of less viscous sialomucins by goblet cells, reduces the adhesion of bacterial cells to the epithelium of the bronchial mucosa. It stimulates the cells that are located in the bronchial mucosa and produce secretions that lyse fibrin. A similar effect is observed in relation to the secretion formed in inflammatory diseases of the ENT organs.
Acetylcysteine quite easily penetrates cells and there it turns into L-cysteine, from which glutathione is then synthesized. Glutathione is a powerful antioxidant and cytoprotectant that traps toxins as well as exogenous and endogenous free radicals. Thus, acetylcysteine helps to increase the synthesis of glutathione inside cells, and, as a result, detoxify harmful substances (which is why acetylcysteine is used as an antidote for paracetamol overdose).
When using ACC Active for prophylactic purposes, there is a decrease in the frequency and severity of exacerbations of bacterial infections in patients with cystic fibrosis and chronic bronchitis.
Pharmacokinetics
After oral administration, acetylcysteine is rapidly and almost completely absorbed. The bioavailability of the drug is only about 10%, since it undergoes the effect of primary passage through the liver. The maximum plasma concentration of acetylcysteine is observed approximately 1-3 hours after ingestion and is 15 mmol / l.
About 50% of the active substance binds to plasma proteins after 4 hours, but after 12 hours the connection with plasma proteins decreases and is no more than 20%. Acetylcysteine is distributed in the body both unchanged (approximately 20%) and in the form of active metabolites (80%). It is found mainly in the lungs, bronchial secretions, kidneys and liver. Passes through the placental barrier, is determined in the umbilical cord blood. There are no data on penetration through the blood-brain barrier and into breast milk.
Metabolism takes place in the liver. As a result, one pharmacologically active metabolite is formed - cysteine, as well as other metabolites - cystine, diacetylcysteine, mixed disulfides. Acetylcysteine and its metabolites are detected in the body in different forms: associated with plasma proteins, in free form, in the form of linked amino acids.
It is excreted by the kidneys almost exclusively in the form of inactive metabolites (diacetylcysteine and inorganic sulfates). In small amounts, it is excreted through the intestines (unchanged).
The half-life is about 1 hour (it depends on the rate of hepatic biotransformation). In patients with impaired liver function, the half-life can be extended to 8 hours.
Indications for use
ACC Active is used for chronic and acute respiratory diseases, accompanied by the formation of difficult-to-separate bronchial secretions (as an expectorant):
- chronic and acute sinusitis;
- rhinosinusitis;
- laryngotracheitis;
- bronchitis;
- bronchiolitis;
- bronchiectasis;
- tracheitis;
- cystic fibrosis;
- pneumonia;
- emphysema of the lungs;
- atelectasis of the lung (due to blockage of the bronchi with a mucous plug);
- lung abscess;
- interstitial lung disease.
In addition, ACC Active is used for otitis media, as well as for postoperative and post-traumatic conditions (in order to remove viscous secretions from the respiratory tract).
Contraindications
Absolute:
- exacerbation of peptic ulcer of the duodenum and stomach;
- pulmonary bleeding;
- hemoptysis;
- phenylketonuria (since the drug contains aspartame);
- hereditary fructose intolerance (due to sorbitol content);
- period of pregnancy and lactation;
- children and adolescents up to 18 years old.
Relative (powder ACTS Active 600 mg is used with caution):
- indications of a history of peptic ulcer of the duodenum or stomach;
- adrenal gland diseases;
- renal and / or hepatic impairment;
- arterial hypertension;
- varicose veins of the esophagus;
- obstructive bronchitis;
- bronchial asthma;
- histamine intolerance (long-term simultaneous use is not recommended, since acetylcysteine affects the metabolism of histamine and can cause symptoms indicative of histamine intolerance, such as itching, vasomotor rhinitis and headache).
ACTS Active, instructions for use: method and dosage
ACTS Active powder is taken orally. The contents of one sachet are poured directly onto the tongue. The powder stimulates salivation, mixes with saliva and is quite easy to swallow, that is, you do not need to drink it with water. Do not chew the powder before swallowing.
The recommended daily intake for adults is 600 mg (1 sachet). Due to the high content of the active substance in 1 sachet, the drug should not be given to children and adolescents under 18 years of age (other dosage forms of acetylcysteine can be used).
The duration of therapy for short-term colds is 5-7 days, and in the case of treatment of chronic processes - up to several months (established by the doctor).
Side effects
- digestive system: infrequently - nausea, vomiting, stomatitis, abdominal pain, diarrhea; rarely - dyspeptic disorders; with an unknown frequency - heartburn;
- cardiovascular system: infrequently - arterial hypotension, tachycardia; very rarely - bleeding; with an unknown frequency - collapse;
- respiratory system: rarely - bronchospasm (usually in patients with bronchial hyperreactivity due to bronchial asthma);
- nervous system: infrequently - headache;
- sense organs: infrequently - ringing in the ears;
- immune system: infrequently - hypersensitivity reactions; very rarely - anaphylactoid / anaphylactic reactions, anaphylactic shock;
- skin and subcutaneous fat: infrequently - itching, rash, exanthema, urticaria, Quincke's edema; very rarely - Lyell's syndrome, Stevens-Johnson syndrome;
- other reactions: infrequently - fever; with an unknown frequency - facial edema.
Overdose
Even after taking high daily doses of acetylcysteine (up to 11.6 g per day for 3 months), no severe adverse reactions develop. Symptoms from the gastrointestinal tract (vomiting, nausea, diarrhea) may occur.
If necessary, symptomatic treatment is carried out.
special instructions
For patients with obstructive bronchitis and bronchial asthma, ACC Active is prescribed with caution, systematically monitoring bronchial patency. If the patient cannot effectively cough up his throat on his own, secretion aspiration or drainage is performed.
If any changes in the mucous membranes and skin appear during the treatment period, the drug should be canceled and immediately seek medical advice.
It is not recommended to take ACC Active before bedtime (it is advisable to take the drug no later than 18:00).
Acetylcysteine can distort the data of some laboratory studies, for example, affect the results of the analysis when determining ketone bodies in urine and the level of salicylates in blood plasma (with a colorimetric method of research).

Application during pregnancy and lactation
There is not enough data on the use of the drug during pregnancy, so its use in pregnant women is contraindicated.
If during lactation the intake of ACC Active is necessary, breastfeeding should be discontinued.
Pediatric use
ACC Active is contraindicated in children and adolescents under the age of 18.
With impaired renal function
For kidney diseases, ACTS Active is used with caution.
For violations of liver function
Acetylcysteine is used with caution in patients with liver disease.
Drug interactions
The simultaneous use of antitussives is not recommended, as they suppress the cough reflex and can cause sputum stagnation.
ACC Active and antibacterial drugs (with the exception of cefixime and loracarbef) should be taken at intervals of at least 2 hours, due to possible pharmaceutical incompatibility.
Acetylcysteine may enhance the antiplatelet and vasodilating effects of nitroglycerin. If necessary, joint use with nitroglycerin, the patient's condition should be monitored for the possible development of arterial hypotension.
Acetylcysteine can reduce the concentration of carbamazepine to subtherapeutic values and eliminate the toxic effect of paracetamol.
Activated carbon can reduce the effectiveness of ACC Active.
