Release form and composition

Dosage form - granules for the preparation of a solution for oral administration: almost white or white, with a strawberry smell [3 g each in bags made of a combined material (paper / foil / polyethylene), in a cardboard box 20 bags at a dosage of 100 or 200 mg, 6 or 10 packets at a dosage of 600 mg and instructions for use of Acetylcysteine Canon].

Pharmacological properties

Pharmacodynamics

The active substance Acetylcysteine Canon is a mucolytic and expectorant agent that is used in diseases of the respiratory system to dilute sputum. Acetylcysteine is a derivative of the natural amino acid cysteine (N-acetyl-L-cysteine) and belongs to the class of direct mucolytics.

The effect of the drug is due to the ability of the free sulfhydryl group to break down the inter- and intramolecular bonds of aggregates of glycoproteins in sputum, as a result of which mucoproteins are depolarized, which has a pronounced thinning effect and reduces the viscosity of mucus. Acetylcysteine has mucolytic activity against any type of sputum: purulent, mucous and mucopurulent. Increases the secretion of less viscous sialomucins by goblet cells, reduces the adhesion of bacteria to epithelial cells of the bronchial mucosa. Stimulates mucosal cells of the bronchi, the secretion of which lyses fibrin.

Acetylcysteine Canon dilutes and increases the volume of sputum, facilitates its discharge, and softens cough well.

In addition to direct mucolytic action, acetylcysteine has a powerful antioxidant and pneumoprotective effect, which helps to protect the respiratory system from the toxic effects of such negative factors as environmental influences, tobacco smoke, and inflammation metabolites.

The direct antioxidant effect of acetylcysteine is explained by the presence of a free thiol group (-SH) in it, which can directly interact with electrophilic oxidative toxins and neutralize them.

The drug protects alpha-1-antitrypsin (an enzyme that inhibits elastase) from possible loss of activity under the influence of the powerful oxidizing agent HOCL, which is produced by the myeloperoxidase enzyme of active phagocytes.

Acetylcysteine easily penetrates into the cell, where it deacetylates and releases the amino acid L-cysteine, which is involved in the synthesis of glutathione - the most important factor of intracellular defense against various cytotoxic substances, endogenous and exogenous oxidative toxins. Due to this feature, acetylcysteine is successfully used in acute intoxication with paracetamol and other toxic substances (for example, phenols and aldehydes).

The mucolytic effect of Acetylcysteine Canon appears within 1–2 days after starting treatment.

With prophylactic use, the drug reduces the frequency and severity of exacerbations of chronic bronchitis and cystic fibrosis.

Pharmacokinetics

Once in the digestive tract, acetylcysteine is absorbed quickly and almost completely. It is metabolized in the liver to cysteine, which is a pharmacologically active metabolite, diacetylcystine and cystine, then to mixed disulfides.

Possessing a pronounced effect of the first passage through the liver, the drug is characterized by low bioavailability - about 10%.

The maximum concentration reaches 1-3 hours after administration. In the human body, acetylcysteine and its metabolites are found in different forms: as a free substance, in connection with blood plasma proteins, and in the form of incorporated amino acids. Acetylcysteine crosses the placental barrier and can accumulate in the amniotic fluid.

It is excreted by the kidneys almost completely in the form of inactive metabolites (diacetylcystine and inorganic sulfates). A small amount of acetylcysteine is excreted unchanged through the intestines.

The plasma half-life depends on the rate of biotransformation in the liver and is about 1 hour, it can increase up to 8 hours in patients with hepatic insufficiency.

Indications for use

Acetylcysteine Canon is intended for the treatment of the following respiratory diseases, accompanied by the formation of viscous and difficult to separate sputum:

  • laryngotracheitis;
  • tracheitis;
  • obstructive bronchitis;
  • acute and chronic bronchitis;
  • chronic obstructive pulmonary disease (COPD);
  • bronchial asthma;
  • bronchiectasis;

Contraindications

Absolute:

  • phenylketonuria;

  • children under 2 years old - for a dosage of 100 mg, up to 6 years - for a dosage of 200 mg, up to 14 years - for a dosage of 600 mg;
  • the presence of increased sensitivity to any component of the drug.

Acetylcysteine Canon should be used with caution in bronchial asthma, renal and / or liver failure, arterial hypertension, varicose veins of the esophagus, adrenal diseases, gastric ulcer and duodenal ulcer in history, a tendency to hemoptysis or pulmonary hemorrhage, as well as acetylcysteine should not be used for a long time, since it affects the metabolism of histamine, which can cause the development of signs of intolerance, such as vasomotor rhinitis, headache, itching).

Acetylcysteine Canon should be taken orally, after meals, after dissolving the granules in water at room temperature.

The mucolytic effect of the drug is enhanced by additional fluid intake.

The duration of treatment for short-term colds is 5-7 days. The course of treatment for long-term illnesses is determined by the attending physician. In chronic bronchitis and cystic fibrosis, in order to achieve a preventive effect in infections, Acetylcysteine Canon should be taken for a longer time.

Granules at a dosage of 100 and 200 mg

Recommended doses for cystic fibrosis:

  • Children over 6 years old: 200 mg (2 sachets of 100 mg or 1 sachet of 200 mg) 3 times a day. For patients weighing more than 30 kg, if necessary, the daily dose is increased, but not more than up to 800 mg per day;
  • children 2-6 years old: 100 mg 4 times a day.

Recommended doses for mucolytic therapy:

  • adults and adolescents from 14 years old: 200 mg 2-3 times a day;
  • children 6-14 years old: 100 mg (1 sachet of 100 mg or ½ sachet of 200 mg) 3 times a day or 200 mg 2 times a day;
  • children 2-6 years old: 100 mg 2-3 times a day.

Unless otherwise prescribed by a doctor, adults and adolescents from 14 years old are advised to take 1 sachet 1 time per day.

Side effects

  • from the gastrointestinal tract: infrequently - diarrhea, nausea, dyspepsia, abdominal pain, heartburn, vomiting, stomatitis;
  • on the part of the cardiovascular system: very rarely - tachycardia, decreased blood pressure;
  • from the nervous system: very rarely - headache;
  • from the senses: infrequently - tinnitus;
  • others: very rarely - fever; unknown frequency - bleeding due to hypersensitivity reactions, decreased platelet aggregation.

Overdose

If you take an excessive dose of Acetylcysteine Canon, you may experience heartburn, stomach pain, diarrhea, nausea, and vomiting. No serious and dangerous side effects have been reported.

Overdose treatment is symptomatic.

When preparing the solution, use glassware. Avoid contact of the drug with metals, rubber, easily oxidizing substances and oxygen.

It is not recommended to take Canon Acetylcysteine immediately before bedtime (the optimal time is before 18.00).

Patients with diabetes mellitus must take into account the presence of sucrose in the composition of Acetylcysteine Canon: in 1 sachet of 100 mg - 0.24 XE (Bread units), in 1 sachet of 200 mg - 0.23 XE, in 1 sachet of 600 mg - 0.19 XE.

Acetylcysteine Canon is pharmaceutically incompatible with proteolytic enzymes and antibiotics such as penicillins, tetracycline, erythromycin, amphotericin B.

Acetylcysteine reduces the absorption of cephalosporins, penicillins and tetracycline. If it is necessary to carry out combined therapy, they should be taken at least 2 hours after acetylcysteine.

On contact with metals and rubber, sulfide compounds with a characteristic odor are formed.
