Aspikor is a non-steroidal anti-inflammatory drug (NSAID), an antiplatelet agent.

Release form and composition

Dosage form - enteric-coated tablets: round, biconvex, almost white or white; on a cross section - a homogeneous mass of almost white or white color (10 pcs. in blister contour packs, in a cardboard box 1, 2, 3 or 6 packs; 20 pcs. in a blister contour packs, in a cardboard box 1 pack; each pack also contains instructions for the use of Aspikor).

Active ingredient: acetylsalicylic acid - 100 mg in 1 tablet.

Auxiliary components: stearic acid, croscarmellose sodium, lactose monohydrate, microcrystalline cellulose, colloidal silicon dioxide (aerosil).

Shell composition: macrogol 4000, talc, titanium dioxide, propylene glycol, copolymer of methacrylic acid and ethyl acrylate.

Pharmacological properties

Pharmacodynamics

Aspicor belongs to the number of NSAIDs (non-steroidal anti-inflammatory drugs), is an antiplatelet agent.

The mechanism of action of the active substance of the drug, acetylsalicylic acid, is based on irreversible inhibition of COX-1 (cyclooxygenase-1), as a result of which thromboxane A 2 synthesis is blocked and platelet aggregation is suppressed. It is generally accepted that acetylsalicylic acid has other mechanisms for suppressing platelet aggregation. This allows you to expand the scope of Aspikor and use it for the treatment of various vascular diseases.

Also, acetylsalicylic acid has analgesic, anti-inflammatory and antipyretic effects.

Pharmacokinetics

After oral administration, acetylsalicylic acid is absorbed from the upper part of the small intestine. The time to reach C max (maximum concentration of the substance) in blood plasma is on average 3 hours.

Partial metabolism takes place in the liver. As a result, less active metabolites are formed.

Excretion is carried out by the kidneys (in the form of metabolites and unchanged). T 1/2 (half-life) of acetylsalicylic acid - about 15 minutes, metabolites - about 3 hours.

Indications for use

Treatment: unstable angina.

Prevention:

  • acute myocardial infarction in patients with concomitant risk factors (such as diabetes mellitus, obesity, arterial hypertension, hyperlipidemia, old age, smoking);
  • repeated myocardial infarction;
  • stroke (including in patients with transient cerebrovascular accident);
  • transient violation of cerebral circulation;
  • thromboembolism of the pulmonary artery and its branches, deep vein thrombosis (for example, in cases of prolonged immobilization due to major surgical intervention);
  • thromboembolism after surgery and invasive interventions on the vessels (such as endarterectomy of the carotid arteries, angioplasty of the carotid arteries, arteriovenous bypass grafting, coronary artery bypass grafting).

Contraindications

Absolute:

  • hemorrhagic diathesis;
  • gastrointestinal bleeding;
  • erosive and ulcerative lesions of the gastrointestinal tract;
  • bronchial asthma due to taking NSAIDs or salicylates;
  • Fernand-Vidal triad (combination of bronchial asthma, recurrent polyposis of the nose and paranasal sinuses, acetylsalicylic acid intolerance);
  • simultaneous use of methotrexate at a dose of 15 mg per week;
  • age up to 18 years;
  • I and III trimesters of pregnancy;
  • period of breastfeeding;
  • hypersensitivity to the components of Aspicor tablets or other NSAIDs.

Relative:

  • a history of ulcerative lesions of the gastrointestinal tract or gastrointestinal bleeding;
  • hyperuricemia;
  • gout;
  • history of bronchial asthma, hay fever, or nasal polyposis;
  • chronic respiratory diseases;
  • hepatic and renal failure;
  • vitamin K deficiency;
  • deficiency of glucose-6-phosphate dehydrogenase;
  • a history of allergic reactions to other drugs;
  • II trimester of pregnancy;
  • simultaneous use of methotrexate at a dose of less than 15 mg per week.

Aspikor, instructions for use: method and dosage

Aspikor tablets must be taken orally before meals with plenty of liquid.

Recommended daily doses:

  • prevention of new-onset acute myocardial infarction in the presence of risk factors - 100 mg;
  • prevention of deep vein thrombosis, thromboembolism of the pulmonary artery and its branches - 100-200 mg;
  • prevention for suspected acute myocardial infarction - 100-200 mg (the first tablet should be chewed for faster absorption);
  • treatment of unstable angina pectoris, prevention of transient disorders of cerebral circulation, stroke, repeated myocardial infarction, thromboembolic complications after invasive studies and surgical operations - 100-300 mg.

Aspicor is intended for long-term therapy, the duration of which is determined by the attending physician.

Side effects

  • from the central nervous system: tinnitus, dizziness;
  • from the respiratory system: bronchospasm;
  • from the digestive system: pain in the abdomen, nausea, gastrointestinal bleeding, vomiting, increased activity of liver enzymes, heartburn, ulcerative lesions of the gastric mucosa and duodenum (including with perforation);
  • from the hematopoietic system: increased bleeding; in rare cases - anemia;
  • allergic reactions: Quincke's edema, urticaria, anaphylactic reactions.

Overdose

The main symptoms of an overdose:

  • moderate severity: vomiting, nausea, hearing impairment, tinnitus, confusion, dizziness;
  • severe: ketoacidosis, fever, hyperventilation, respiratory and cardiovascular failure, respiratory alkalosis, severe hypoglycemia, coma.

Therapy:

  • moderate severity: dose reduction of Aspikor;
  • severe degree: immediate hospitalization is indicated in a specialized department for the purpose of emergency therapy, including the use of activated carbon, determination of acid-base balance, gastric lavage, alkaline and forced alkaline diuresis, hemodialysis, administration of solutions, symptomatic treatment. With alkaline diuresis, it is necessary to achieve pH values in the range of 7.5-8. Forced alkaline diuresis is indicated in cases where the plasma concentration of salicylates exceeds 500 mg / L (3.6 mmol / L) and 300 mg / L (2.2 mmol / L) in adults and children, respectively.

Acetylsalicylic acid (ASA) can cause bleeding during and after surgery.

Used in low doses, ASA can cause the development of gout in predisposed patients (people with reduced uric acid excretion).

ASA can cause an attack of bronchial asthma, bronchospasm and other hypersensitivity reactions. Risk factors include a history of bronchial asthma, nasal polyposis, hay fever, chronic respiratory diseases, allergic reactions to other drugs.

In high doses, ASA has a hypoglycemic effect, which should be taken into account when prescribing Aspikor to patients with diabetes mellitus who are taking hypoglycemic drugs.

During treatment, it is recommended to refrain from drinking alcohol, as it increases the risk of damage to the gastrointestinal mucosa and an increase in bleeding time.

Do not exceed the dose of Aspikor prescribed by the doctor, since this is fraught with the development of gastrointestinal bleeding. Overdose is especially dangerous for the elderly.

Influence on the ability to drive vehicles and complex mechanisms

Aspicor does not affect the speed of reactions and the ability to concentrate.

Application during pregnancy and lactation

  • I and III trimesters of pregnancy, lactation period: therapy is contraindicated. In case of an accidental single intake of Aspikor, the abolition of breastfeeding is not required, if long-term therapy is necessary, breastfeeding should be stopped immediately;
  • II trimester of pregnancy: Aspicor should be used under medical supervision only after assessing the risk-benefit ratio.

Pediatric use

Aspicor therapy is contraindicated in patients under 18 years of age.

Drug interactions

In the case of simultaneous use, ASA enhances the effect of the following agents: methotrexate, digoxin, indirect anticoagulants and heparin, valproic acid, hypoglycemic, thrombolytic and antiplatelet drugs.

With the combined use of ASA, it weakens the effect of uricosuric drugs.

With the simultaneous intake of ethanol, an additive effect is observed.

Aspicor is not recommended for use in combination with the following drugs:

  • anticoagulants, thrombolytics and antithrombotic agents - due to an increased risk of bleeding;
  • methotrexate - due to the high likelihood of side effects from the hematopoietic organs;
  • ibuprofen - because it impairs the beneficial effect of Aspicor on life expectancy.

With the simultaneous appointment of salicylates and glucocorticosteroids (GCS), it should be borne in mind that during therapy, the level of salicylates decreases, and after the abolition of GCS, it increases, as a result of which an overdose of salicylates is possible.
