Amigraine is a remedy for migraine.

Release form and composition

The drug is produced in the form of tablets, film-coated, white in cell contour packages.

The active substance in the composition is sumatriptan, which acts on the smooth muscles of the blood vessels of the brain, contributes to the narrowing of the vessels of the carotid arteries and suppresses the activity of the trigeminal nerve.

The following components act as auxiliary substances in the preparation:

  • Microcrystalline cellulose;
  • Potato starch;
  • Lactose;
  • Magnesium stearate;
  • Sodium carboxymethyl starch.

Pharmacological properties

Pharmacodynamics

Sumatriptan, the active ingredient of Amigrenin, is an anti-migraine agent. It belongs to the specific and selective agonists of 5-HT 1 -serotonin receptors, which are found mainly in the blood vessels of the brain. Sumatriptan does not affect other types of 5-HT-serotonin receptors (5-HT 2-7). The compound contributes to the narrowing of the vessels of the carotid arterial bed, supplying blood to intracranial and extracranial tissues (the mechanism of development of migraine in humans is edema and / or dilation of the vessels of the meninges). In this case, the drug has practically no effect on cerebral blood flow.

Sumatriptan inhibits the activity of receptors localized at the endings of the afferent fibers of the trigeminal nerve in the dura mater (as a result, the excretion of sensory neuropeptides decreases). Amigrenin helps relieve the photophobia and nausea that accompany a migraine attack. When taken orally in a dose of 25-100 mg in 50-70% of cases, the drug quickly stops such an attack. Within 24 hours, a relapse is observed in one third of cases, requiring re-administration of sumatriptan. After ingestion of the drug with a dose of 100 mg, it begins to act in 30 minutes.

Pharmacokinetics

When taken orally, sumatriptan is absorbed at a high rate. After 45 minutes, 70% of the maximum plasma content of the substance is reached. When taken orally at a dose of 100 mg, the maximum plasma level of sumatriptan is approximately 54 ng / ml. Bioavailability does not exceed 14% due to incomplete absorption and first-pass metabolism.

Sumatriptan binds to plasma proteins by 14–21% and is metabolized by oxidation, in which monoamine oxidase takes part. As a result, several metabolites are formed, the main of which is the indoleacetic analogue of sumatriptan, which does not exhibit pharmacological activity against 5-HT 1 and 5-HT 2 -serotonin receptors.

The half-life of sumatriptan is 2 hours. The main metabolite of the drug - the indoleacetic analogue of its active component - is excreted in the urine in the form of free acid and its glucuronide conjugate. It is assumed that migraine attacks do not have a significant effect on the pharmacokinetics of sumatriptan when taken orally.

Indications for use

According to the instructions, Amigrenin is prescribed to relieve pain caused by migraines with and without aura.

Contraindications

The drug should not be prescribed for:

  • Angina pectoris;
  • Heart attack;
  • Uncontrolled hypertension;
  • Ischemia;
  • Stroke;
  • Occlusive peripheral arterial disease;
  • Transient circulatory disorders of the brain;
  • Renal and hepatic failure.

Contraindications to the use of Amigrenin are also pregnancy, lactation, age under 18 and over 65 years.

Instructions for the use of Amigrenin: method and dosage

The tablets are taken orally with water, regardless of the meal.

When an attack occurs, it is recommended to take 1 tablet (50 mg), in some cases it is allowed to take 100 mg.

The permissible daily dosage is 300 mg.

If, after using one dose (50 mg), the attack has not been eliminated, it is not recommended to take the second immediately. It is possible to take 50 mg of the drug again only if after the first dose of Amigrenin there is relief, but after a while the pain resumed. The break between pills should be at least 2 hours.

Amigrenin usually takes effect half an hour after ingestion.

Side effects

The use of Amigrenin in some cases can cause the following side effects:

In this case, it is recommended to flush the stomach and monitor the patient's condition for 10 hours. If necessary, symptomatic therapy is performed. There are no data on the effect of peritoneal dialysis or hemodialysis on the content of sumatriptan in blood plasma.

Amigrenin is prescribed with caution for epilepsy and controlled hypertension. You can not combine the drug with Ergotamine and MAO inhibitors (not only at the same time, but also for 2 weeks after stopping them).

Drug interactions

With the simultaneous administration of sumatriptan with ergotamine, prolonged vasospasm was observed. Amigrenin is recommended to be used no earlier than 24 hours after taking medications that include ergotamine.

Sumatriptan is able to interact with MAO inhibitors, therefore, simultaneous therapy with these drugs is contraindicated.

With the combination of sumatriptan and selective serotonin reuptake inhibitors, patients may develop impaired coordination of movements, hyperreflexia and severe weakness. For this reason, it is unacceptable to combine sumatriptan with drugs of this group.

You should not take lithium preparations at the same time.
