Akineton is a synthetic drug used for Parkinson's syndrome, as well as for extrapyramidal symptoms caused by neuroleptics.

Release form and composition

• Tablets: flat-cylindrical, almost white, with chamfers, on one side there is a cross-shaped risk (10 or 20 pieces in blisters, to a cardboard box of 2, 5 or 10 blisters);
• Solution: transparent, colorless (1 ml in ampoules, in a cardboard box 5 ampoules).

Each pack also contains instructions for the use of Akineton.

The tablets contain 2 mg of biperidene hydrochloride. Auxiliary substances are microcrystalline cellulose, corn and potato starch, calcium hydrogen phosphate dihydrate, lactose monohydrate, copovidone, magnesium stearate, purified water, talc.

1 ml of the drug solution contains 5 mg of biperiden lactate. Excipients - sodium lactate and water for injection.

Pharmacological properties

Pharmacodynamics

Akineton is a centrally acting anticholinergic drug that reduces the activity of cholinergic neurons in the striatum (refer to the structural components of the extrapyramidal system). The peripheral anticholinergic effect of biperiden is less pronounced.

Biperiden reduces rigidity and tremors. Leads to psychomotor agitation, autonomic disorders.

Pharmacokinetics

After taking biperiden inside C _max (maximum concentration of the substance) is 1.01-6.53 ng / ml, the time to reach it is from 30 to 120 minutes. C _ss (equilibrium concentration) after oral administration of 2 mg of the drug 2 times a day is reached in 15.7–40.7 hours. Bioavailability after a single dose is approximately 33 ± 5%.

Indications for use

Akineton is used for:

• Parkinson's syndrome in adults;
• Poisoning with nicotine or phosphorus-containing organic substances in adults;
• Extrapyramidal symptoms caused by antipsychotics or drugs similar in action in children and adults.

Contraindications

The drug is contraindicated in:

• Stenosis and obstruction of the gastrointestinal tract;
• Closed-angle glaucoma;
• Megacolone;
• Increased individual sensitivity to any component of the drug.

Akineton tablets are not prescribed for children under the age of 3 years.

Akineton, instructions for use: method and dosage

In severe cases of Parkinson's syndrome in adult patients, the drug is administered slowly intravenously or intramuscularly in 2-4 ml doses, divided into several doses. When using Akineton tablets, they usually start with small doses - 1 mg up to 2 times a day. Depending on the side effects and the therapeutic effect, the dose can be gradually increased by 2 mg daily. The maximum dose of the drug is 16 mg. It can be divided into 4 steps.

For movement disorders that are caused by the drugs taken, a single use of Akineton is prescribed:

• Children under 1 year old - intravenously 0.2 ml;
• Children 1-6 years old - 0.4 ml;
• Children over 10 years old - 0.6 ml;
• For adults, the drug is prescribed intramuscularly or intravenously, 0.5-1 ml, the maximum dose is 2-4 ml.

If necessary, a repeated dose can be administered in half an hour. If during the administration, the development of side effects is observed, the use of the solution must be discontinued.

For movement disorders, Akineton can also be used in tablets taken during or after meals. The duration of treatment depends on the course of the disease. The dose for adults is 1-4 mg up to 4 times a day, for children 3-15 years old - 1-2 mg up to 3 times a day.

When discontinuing the drug, the dose taken should be reduced gradually.

In cases of poisoning with an organic phosphorus mixture, the dose is prescribed depending on the degree of poisoning, while Akineton is used until the symptoms disappear.

In case of nicotine poisoning, the drug is prescribed as an additional therapy intramuscularly, 1-2 ml.

Side effects

The use of Akineton in some cases can cause:

• Drowsiness;
• Dizziness;
• Ataxia;
• Weakness;
• Anxiety;
• Increased fatigue;
• Dyskinesia;
• Confusion of consciousness;
• Nervousness;
• Euphoria
• Memory impairment;
• Delirious disorders;
• Insomnia;
• Muscle cramps;
• Impaired speech.

• Drug dependence.

Side effects are most often observed in the early stages of treatment, as well as when the dose is increased too quickly.

In case of an overdose with Akineton, the following symptoms may occur:

• Redness of the skin;
• Hyperthermia;
• Atony of the bladder and intestines;
• Delirium;
• Dilated pupils slowly reacting to light - mydriasis;

• Cardiopalmus.

Overdose

The main symptoms: mydriasis (dilated, slowly reacting pupils to light), redness of the skin, dry mucous membranes, palpitations, intestinal and bladder atony, hyperthermia (especially in children), collapse, agitation, delirium, confusion.

Therapy: symptomatic. The antidote is acetylcholinesterase inhibitors and especially physostigmine, if necessary, bladder catheterization is prescribed.
