Finasteride-OBL is a 5-alpha reductase inhibitor, a drug for the treatment of benign prostatic hyperplasia.
Release form and composition
The drug is produced in the form of film-coated tablets: from light pink to pink, round, biconvex; the core is white or almost white (7, 10, 14, 15, 20 or 30 pcs. in blisters, in a cardboard box 1, 2, 3 or 4 packages and instructions for use of Finasteride-OBL).
1 tablet contains:
- active substance: finasteride - 5 mg;
- auxiliary components: lactose monohydrate, microcrystalline cellulose, calcium hydrogen phosphate dihydrate, sodium dodecyl sulfate, sodium croscarmellose, hyprolose (hydroxypropyl cellulose), magnesium stearate;
- the composition of the film shell: hyprolose (hydroxypropyl cellulose), copovidone, titanium dioxide, dye azorubin, talc.
Pharmacological properties
Pharmacodynamics
Finasteride-OBL is a drug for the treatment of benign prostatic hyperplasia. Its active ingredient is finasteride, a synthetic 4-azasteroid compound that is a competitive and specific inhibitor of such an intracellular enzyme as type II steroidal 5-alpha reductase. Inhibition of this enzyme slows down the conversion of testosterone to the active androgen 5-dihydrotestosterone, reducing the level of its concentration in the blood and prostate tissue.
By suppressing the formation of dihydrotestosterone, Finasteride-OBL helps to increase the maximum flow rate of urine, decrease the volume of the prostate gland and the severity of symptoms associated with prostatic hyperplasia.
Finasteride does not bind to androgen receptors. Does not affect the degree of plasma lipid concentration or the content of estradiol, cortisol, thyroid-stimulating hormone, prolactin, thyroxine in the blood plasma.
Pharmacokinetics
After taking the tablet inside, finasteride is absorbed from the gastrointestinal tract within 6–8 hours. The bioavailability of the drug is approximately 80%, with simultaneous ingestion of food it does not decrease. The maximum concentration of finasteride in blood plasma is reached after 2 hours.
Plasma protein binding - 93%.
It overcomes the blood-brain barrier, after 7-10 days of treatment, finasteride is found in an insignificant concentration in the cerebrospinal fluid. In semen, its concentration in adult men is 50–100 times lower than in blood plasma.
With prolonged use, a slight accumulation of the active substance occurs.
Metabolized by the liver.
Systemic clearance is about 165 ml / min. The half-life (T 1/2) is 6 hours.
In men over the age of 70, T 1/2 is about 8 hours, but this is not a reason to reduce the dose of Finasteride-OBL.
Finasteride is excreted in the form of metabolites: through the kidneys - 39% and through the intestines - 57%.
In elderly patients, the rate of elimination is somewhat reduced.
In renal failure, the concentration of finasteride metabolites in the urine decreases, and in the feces, their amount increases. In this regard, dose adjustment of Finasteride-OBL is not required if hemodialysis is not indicated in patients with renal insufficiency.
Indications for use
The use of Finasteride-OBL is indicated for the treatment of benign prostatic hyperplasia in order to improve urine flow and reduce the risk of acute urinary retention, reduce the size of an enlarged prostate and the severity of symptoms associated with benign prostatic hyperplasia.
In addition, the drug is prescribed to reduce the need for surgical procedures, including transurethral resection of the prostate and prostatectomy.
Contraindications
Absolute:
- prostate cancer;
- obstructive uropathy;
- lactase deficiency, lactose intolerance, glucose-galactose malabsorption (Finasteride-OBL contains lactose);
- age up to 18 years;
- hypersensitivity to the components of the drug.
Finasteride-OBL is not prescribed for the treatment of women.
The tablets should be taken with caution in case of impaired liver function, a large volume of residual urine and / or a significantly reduced urine flow.
Finasteride-OBL, instructions for use: method and dosage
Finasteride-OBL tablets are taken orally before or after meals.
Recommended dosage: 1 pc. 1 per day. The duration of the course of treatment should be at least 180 days.
In case of impaired renal function or treatment of elderly patients, dose adjustment is not required.
Side effects
- from the nervous system: drowsiness, headache, dizziness;
- from the skin and subcutaneous tissues: itching, rash, urticaria, allergic reactions, angioedema of the lips and face;
- from the reproductive system: decreased libido, soreness of the testicles, decreased volume of ejaculate, ejaculation disorders, impotence;
- from the digestive system: diarrhea, abdominal pain, increased liver transaminases;
- from the endocrine system: gynecomastia, breast tenderness;
- on the part of the cardiovascular system: increased heart rate;
- laboratory parameters: a decrease in the level of prostate-specific antigen.
Overdose
Overdose symptoms of Finasteride-OBL have not been established.
special instructions
The appointment of Finasteride-OBL should be carried out after receiving the results of relevant studies confirming the absence of infectious prostatitis, urethral stricture, prostate cancer, bladder hypotension and other diseases that stimulate benign prostatic hyperplasia in the patient. In the course of treatment, it is recommended to periodically conduct rectal and other methods of research for the presence of prostate cancer.
The use of the drug in patients with a sharply reduced urine flow and / or a large residual urine volume must be accompanied by careful medical supervision due to the increased risk of obstructive uropathy.
The patient should be informed about the need to immediately consult a doctor if such undesirable phenomena as swelling and pain in the chest area, discharge from the nipple, gynecomastia appear.
It should be borne in mind that with prolonged therapy with finasteride, the frequency and severity of side effects does not increase, and the number of cases of sexual dysfunction caused by taking the drug decreases.
Influence on the ability to drive vehicles and complex mechanisms
Due to the fact that taking Finasteride-OBL can cause side effects from the nervous system, patients are advised to be careful during treatment, and if dizziness or drowsiness occurs, refrain from driving or performing other potentially hazardous activities that require increased attention and high speed of psychomotor reactions.
Application during pregnancy and lactation
Finasteride-OBL is not intended for use in women.
Women of childbearing age and pregnant women should not be allowed direct contact with tablets that have lost the integrity of the shell. This is due to the teratogenic properties of finasteride, which, when it enters the body of a pregnant woman, is capable of causing the pathology of the intrauterine formation of the external genital organs in a male fetus. It should be borne in mind that the active substance penetrates the seminal fluid and, therefore, use reliable barrier contraceptives during the period of treatment of a sexual partner.
Pediatric use
Finasteride-OBL is contraindicated in patients under 18 years of age.
With impaired renal function
In case of impaired renal function, dose adjustment is not required.
For violations of liver function
Finasteride-OBL tablets should be used with caution when treating patients with impaired liver function.
Use in the elderly
When treating elderly patients, no dose adjustment of Finasteride-OBL is required.
Drug interactions
The simultaneous use of finasteride in combination with doxazosin is allowed.
No clinically significant interactions of finasteride with other drugs have been found.
