Pharmacological properties
Pharmacodynamics
Encetron-SOLOpharm is a nootropic drug. Its active substance, citicoline, is a natural endogenous compound, an intermediate metabolite in the synthesis of one of the main structural components of the cell membrane, phosphatidylcholine.
The wide spectrum of action of the drug is due to such properties of citicoline as restoration of damaged cell membranes, inhibition of the action of phospholipases, prevention of the formation of excess free radicals, prevention of cell death due to the effect on the mechanisms of apoptosis.
In the acute period of stroke, it improves cholinergic transmission, reduces the amount of damage to the brain tissue.
The use of Encetron-SOLOpharm in traumatic brain injury (TBI) reduces the duration of post-traumatic coma, helps to reduce the severity of neurological symptoms and reduce the duration of the recovery period.
In chronic hypoxia of the brain, taking citicoline contributes to the effective treatment of such cognitive disorders as lack of initiative, memory impairment, difficulties arising from self-care and performing daily activities. In patients, the manifestation of amnesia decreases, the level of attention and consciousness increases markedly.
Encetron-SOLOpharm is effective for sensitive and motor neurological disorders of degenerative and vascular etiology.
Pharmacokinetics
Citicoline is well absorbed when taken orally, intravenously and intramuscularly. Its bioavailability after oral administration is comparable to that after intravenous administration.
It is largely distributed in the structures of the brain, embedded in cellular, cytoplasmic and mitochondrial membranes. Takes part in the construction of the phospholipid fraction.
Citicoline is metabolized with intravenous and intramuscular administration in the liver, when taken orally - in the intestine and liver, with the formation of choline and cytidine. There is a significant increase in the level of choline concentration in the blood plasma. Upon reaching the brain, the choline fractions are rapidly incorporated into structural phospholipids and cytidine fractions (cytidine nucleotides and nucleic acids).
Only 15% of the administered dose of citicoline is excreted from the body: with exhaled air - about 12%, through the kidneys - less than 3%. In the elimination of the drug, two phases are distinguished. The first phase of excretion with urine lasts about 36 hours, in this phase the rate of excretion decreases rapidly, the second phase - with a slower rate of excretion. The rate of excretion of citicoline with exhaled air in the first phase, on average, during the first 15 hours after administration, decreases rapidly, in the second phase - much more slowly.
Indications for use
- acute (as part of complex therapy) and recovery period of ischemic stroke;
- acute (as part of complex therapy) and recovery period of TBI;
- the recovery period of hemorrhagic stroke;
- cognitive and behavioral disorders due to degenerative and vascular diseases of the brain.
Contraindications
Absolute:
- severe vagotonia (high tone of the parasympathetic autonomic nervous system);
- breast-feeding;
- age up to 18 years;
- hypersensitivity to the components of the drug.
With caution and only in cases of extreme necessity, it is allowed to use Encetron-SOLOfarm during pregnancy.
Encetron-SOLOfarm, instructions for use: method and dosage
Oral solution
The solution is taken orally during or after meals, including by mixing it with 100-120 ml of water.
Recommended dosage:
- acute period of ischemic stroke or TBI: 1000 mg (10 ml) 2 times a day at regular intervals. The duration of the course of treatment is at least 42 days;
- recovery period for TBI, ischemic and hemorrhagic strokes, cognitive and behavioral disorders in patients with degenerative and vascular diseases of the brain: 500–2000 mg (5–20 ml) per day. The dosage and duration of therapy are determined individually, taking into account the severity of the symptoms of the disease.
Elderly patients do not need Encetron-SOLOpharm dose adjustment.
Solution for i / v and i / m administration
The solution is used by intramuscular or slow (within 3-5 minutes) intravenous injection, as well as intravenous infusion (injection rate 40-60 drops per minute).
Encetron-SOLOpharm is compatible with all types of intravenous dextrose solutions or isotonic solutions.
After opening the ampoule, the solution should be used immediately!
Polymer ampoules are equipped with a twisting cap, which allows the intake of the solution into the syringe without a needle. Before the procedure, after shaking the contents of the ampoule, it is lightly squeezed by hand and rotated with a rotating movement and the protective valve is separated. Having inserted a syringe into the resulting hole, the ampoule is turned over and a slow intake of the solution is made. Then put the needle on the syringe.
It is preferred to use the intravenous route of administration. With intramuscular injection, injections should not be allowed in the same place.
Recommended dosage of Encetron-SOLOpharm:
- acute period of ischemic stroke and TBI: 1000 mg 2 times a day at regular intervals. Treatment should be started immediately after diagnosis, the duration of treatment is at least 42 days. Patients with normal swallowing function after 3-5 days can be transferred to the oral form of the drug;
- cognitive and behavioral disorders in degenerative and vascular diseases of the brain, the recovery period of ischemic and hemorrhagic strokes, the recovery period of TBI: 500-2000 mg per day. The doctor sets the dose and treatment period individually, taking into account the patient's condition.
If intracranial bleeding is suspected, the dose of the drug should not exceed 1000 mg per day. The drug should be injected intravenously (infusion rate - 30 drops per minute).
When Encetron-SOLOfarm is prescribed to elderly patients, there is no need to adjust the dose.

Application during pregnancy and lactation
The use of Encetron-SOLOpharm during breastfeeding is contraindicated.
If necessary, the appointment of the drug during lactation should stop breastfeeding.
With caution and only on condition that the expected therapeutic effect for the mother outweighs the potential threat to the fetus, Encetron-SOLOpharm may be prescribed during pregnancy.
