Passage - a central blocker of dopamine receptors; antiemetic agent.

Release form and composition

  • chewable tablets: round, flat-cylindrical, white or white with a creamy shade, with a risk (4, 6 and 10 pcs. in blisters, in a cardboard box 1, 2, 3, 4 or 5 packages);
  • coated tablets: round, biconvex, white or white with a creamy / yellowish tinge, two layers are visible in the cross section (10 pcs. in blisters, in a cardboard box 1, 2 or 3 packages).

Active ingredient: domperidone, in 1 chewable and 1 coated tablet - 10 mg each.

Auxiliary components:

Pharmacological properties

Pharmacodynamics

Domperidone is a substance that increases the duration of peristaltic contractions of the antrum of the stomach and duodenum 12, eliminates the development of nausea and vomiting, increases the tone of the lower esophageal sphincter, accelerates gastric emptying if this process slows down.

The drug stimulates the release of prolactin from the pituitary gland; weakly penetrates the blood-brain barrier, therefore, with its use, extrapyramidal side effects rarely occur, especially in adults.

The antiemetic effect of domperidone is presumably due to a combination of gastrokinetic (peripheral) action and antagonism to dopamine receptors in the chemoreceptor trigger zone.

The drug does not affect gastric secretion.

Pharmacokinetics

Domperidone is rapidly absorbed after oral administration. With a reduced acidity of gastric juice, its absorption decreases.

The drug is characterized by low bioavailability - about 15%, high binding to plasma proteins - 91–93%.

The maximum concentration in blood plasma reaches 1 hour after administration. It is widely distributed in various tissues of the body, with the exception of brain tissues - here the concentration of the substance is low. It undergoes intensive metabolism in the intestinal wall and liver.

It is excreted: about 66% - through the intestines (unchanged 10% of the dose), about 33% - by the kidneys (unchanged 1% of the dose).

The half-life is 7-9 hours. In patients with renal insufficiency, it increases.

Indications for use

  • delayed gastric emptying, gastroesophageal reflux, esophagitis and other disorders manifested by a complex of dyspeptic symptoms: flatulence, belching, heartburn (including with the throwing of stomach contents into the oral cavity), feeling of bloating, feeling of fullness in the epigastrium, pain in upper abdomen;
  • nausea and vomiting of infectious, organic and functional origin, arising from a violation of the diet, radiotherapy and drug therapy, as well as caused by dopamine agonists (such as bromocriptine, L-dopa) in the case of their use in Parkinson's disease.

Contraindications

  • a prolactin-secreting pituitary tumor (prolactinoma);
  • gastrointestinal bleeding, perforation, mechanical obstruction and other conditions in which stimulation of the motor function of the stomach can be dangerous;
  • children under 5 years old;
  • individual intolerance to the components of the Passage.

Carefully:

  • lactation period;
  • renal / hepatic impairment.

Instructions for the use of Passage: method and dosage

Passage tablets should be taken orally, preferably 15-30 minutes before meals, since food slows down the absorption of domperidone somewhat.

Adults, depending on the indications, are prescribed:

  • chronic dyspepsia: 10 mg 3 times a day and, if necessary, at bedtime;
  • acute and subacute conditions, especially accompanied by nausea and vomiting: 20 mg 3-4 times a day and before bedtime.

Children from 5 years old, depending on the indications, are prescribed:

  • chronic dyspepsia: 2.5 mg / 10 kg 3 times a day and, if necessary, at bedtime;
  • acute and subacute conditions, especially accompanied by nausea and vomiting: 5 mg / 10 kg of body weight 3-4 times a day and before bedtime.

In chronic dyspepsia, in case of insufficient effect, the dose of the drug may be increased by 2 times.

Patients with renal insufficiency in case of repeated administration of Passage should reduce the frequency of its administration to 1-2 times a day and, if necessary, the dose.

Side effects

Passage is generally well tolerated, side effects are rare. There are isolated cases of transient bowel spasms and allergic reactions such as rashes and hives.

With increased permeability of the blood-brain barrier and in children, extrapyramidal disorders are possible. They are reversible and go away on their own after the end of therapy.

Passage may increase plasma prolactin levels. In exceptional cases, hyperprolactinemia leads to the development of gynecomastia, galactorrhea, dysmenorrhea.

Overdose

Domperidone overdose can cause drowsiness, disorientation and extrapyramidal phenomena, especially in children.

When taking significantly higher than the recommended dose, you should take activated charcoal and monitor the patient's condition. When extrapyramidal reactions occur, antihistamines and anticholinergic drugs, as well as drugs used for parkinsonism, can be effective.

In the case of the simultaneous appointment of antisecretory or antacid drugs, they should be taken after meals, while domperidone should be taken before meals.

Influence on the ability to drive vehicles and complex mechanisms

According to the instructions, Passage does not affect the ability to concentrate, does not disturb the speed of mental and motor reactions.

Application during pregnancy and lactation

In experimental studies on animals, domperidone at a daily dose of up to 160 mg / kg did not have a teratogenic effect. In addition, there is no evidence of an increased risk of malformations when using the drug in humans. Nevertheless, in the first trimester of pregnancy, Passage, like most drugs, is recommended to be used only if the expected therapeutic benefit outweighs the potential risks.

The concentration of domperidone in breast milk of a nursing woman is 4 times lower than in plasma. However, it is not known if this concentration can have a negative effect on infants. For this reason, it is recommended to refrain from breastfeeding while taking Passazhik, unless the intended benefits justify the potential risks.

Pediatric use

For the treatment of children under 5 years old, the use of Passage is contraindicated.

With impaired renal function

Only a small part of the drug is excreted unchanged by the kidneys, so dose adjustment is not needed. However, with repeated use of Passage tablets, it is recommended to reduce the frequency of administration to 1–2 times a day, depending on the severity of renal failure; in some cases, a dose reduction may also be required.

For violations of liver function

Domperidone is metabolized in the liver, therefore, patients with hepatic impairment should be careful during treatment. With prolonged therapy, regular monitoring is required.

Drug interactions

Anticholinergics, when used simultaneously, can neutralize the effect of domperidone. Antisecretory and antacid drugs reduce its bioavailability.

It is possible that domperidone may affect the absorption of drugs with a sustained release of the active substance or those coated with an enteric coating.

Passage does not affect the level of digoxin and paracetamol in the blood if they are used simultaneously.

Drugs that significantly inhibit the cytochrome CYP3A4 enzyme (HIV protease inhibitors, antibiotics from the macrolide group, antifungal agents from the azole group, antidepressant nefazodone) are believed to increase plasma domperidone levels.
