Pangrol 25000 is an enzyme preparation that compensates for the deficiency of pancreatic enzymes in diseases of the gastrointestinal tract (GIT).

Release form and composition

The drug is produced in the form of enteric capsules: cylindrical with hemispherical ends, size No. 0, gelatinous, solid, opaque, light orange body, yellowish green cap; inside the capsules there are enteric-coated mini-tablets, which have a shiny surface of light beige color and a cylindrical shape (20, 50 or 100 pieces in polypropylene bottles, 1 bottle in a cardboard box and instructions for use of Pangrol 25000).

1 enteric-coated mini-tablet contains:

  • active substance: pancreatin - 356.1 mg, with a minimum lipase activity - 25,000 units (unit of action), amylase - 22,500 units, protease - 1250 units;
  • auxiliary components: croscarmellose sodium, microcrystalline cellulose, colloidal anhydrous silicon dioxide, hydrogenated castor oil, magnesium stearate;
  • enteric coating: copolymer of methacrylic acid and ethyl acrylate (1: 1) in the form of 30% dispersion, triethyl citrate, simethicone emulsion 30%, talc.

Pangrol 25000 - a drug that replenishes the deficiency of pancreatic enzymes; has proteolytic, amylolytic and lipolytic effects.

Its active component, pancreatin, is a powder extract obtained from the pancreas of pigs. Enzymes included in pancreatin (protease, lipase, amylase) are involved in the breakdown of carbohydrates, fats and proteins from food. Pancreatin, compensating for the enzymatic insufficiency of the pancreas, helps to normalize digestion processes and improve the functional state of the gastrointestinal tract.

Pharmacokinetics

After ingestion, the capsules dissolve rapidly in the stomach and release mini-tablets. The enteric coating of the mini-tablets is acid-stable, which allows the enzymes to remain stable in the acidic environment of the stomach. As a result of mixing the mini-tablets with the intestinal contents, the enzymes are evenly distributed. Enzymes are activated after dissolution of the mini-tablet shell in the small intestine at a slightly alkaline or neutral acidity (pH).

Not being absorbed in the gastrointestinal tract, pancreatin is excreted through the intestines.

Indications for use

The use of Pangrol 25000 is indicated for adults and children as a replacement therapy for insufficiency of the exocrine function of the pancreas in the following pathologies and conditions:

  • cystic fibrosis;
  • conditions after surgical interventions on the stomach or pancreas for complete or partial resection of the organ;

  • conditions accompanied by flatulence, impaired digestion of food, diarrhea, arising after irradiation of the digestive tract - as part of combination therapy;
  • Schwachman-Diamond syndrome;
  • other diseases that are accompanied by exocrine pancreatic insufficiency.

In addition, the drug is prescribed to patients with relative enzyme deficiency in conditions and situations such as:

  • functional disorder of the gastrointestinal tract due to acute intestinal infection, irritable bowel syndrome;
  • eating plant-based or fatty foods that are difficult to digest.

Pangrol 25000 capsules are also used in preparation for X-ray or ultrasound examination of the abdominal organs.

Contraindications

Absolute:

  • period of exacerbation of chronic pancreatitis;
  • acute pancreatitis;
  • hypersensitivity to the components of the drug.

During pregnancy, the use of Pangrol 25000 is allowed in cases where the expected effect for the mother justifies the possible risk to the fetus.

Pangrol 25000, instructions for use: method and dosage

Pangrol 25000 capsules are taken orally during meals, swallowing whole and drinking a sufficient amount (200 ml) of water or other liquid.

Taking the whole capsule can be difficult when treating young children or elderly patients. In this case, the capsule should be opened, separating the cap from the body, and pouring its contents (mini-tablets) into a glass with a small amount of liquid or food that does not require chewing (for example, applesauce, fruit juice). The resulting mixture is taken immediately after preparation, it cannot be stored.

The doctor selects the dose of the drug and the duration of the course of treatment individually, taking into account the severity and course of the disease, the volume and composition of the food consumed.

The recommended dosage of Pangrol 25000:

  • adults: 1–2 pcs. at every meal. The dose of enzymes should be increased only under the supervision of a physician, taking into account the dynamics of abdominal pain relief, reduction of steatorrhea and other symptoms. The maximum daily dose is 15,000–20,000 lipase U per 1 kg of patient weight (U / kg);

With cystic fibrosis, children under 4 years of age are prescribed 1000 lipase U / kg for each meal, children over 4 years old - 500 lipase U / kg at each meal. Side effects

  • from the gastrointestinal tract: very often (≥ 1/10) - pain in the abdomen; often (from ≥ 1/100 to <1/10) - bloating, nausea, vomiting, diarrhea;
  • on the part of the skin and subcutaneous tissues: rarely (from ≥ 1/10 000 to <1/1000) - rash; the frequency has not been established (it is impossible to establish the frequency based on the available data) - itching, urticaria.

Overdose

In patients with cystic fibrosis, taking a high dose of pancreatic enzymes is associated with the risk of developing hyperuricosuria and hyperuricemia.

If you suspect an overdose, the drug should be stopped immediately. To stop undesirable phenomena, symptomatic therapy is performed.
