Pharmacological properties

Pharmacodynamics

Pancreatin, the active ingredient of Pangrol 10000, is a powder from the pancreas of pigs. The enzymes it contains help break down proteins, fats and carbohydrates from food.

By means of lipolytic, amylolytic and proteolytic action, pancreatin normalizes digestion processes, improves the functional state of the gastrointestinal tract, compensates for the enzymatic insufficiency of the pancreas.

Pharmacokinetics

Gelatin capsules Pangrol 10000 quickly dissolve in the stomach, releasing acid-resistant (enteric) coated mini-tablets. Due to this, the enzymes remain protected from inactivation in the acidic environment of the stomach.

The release form of the drug provides mixing of mini-tablets with intestinal contents and uniform distribution of enzymes. Enzyme activation and dissolution of the mini-tablet coating occurs at slightly alkaline or neutral pH in the small intestine. Pancreatin is not absorbed into the gastrointestinal tract, it is excreted through feces.

Indications for use

According to the instructions, Pangrol 10000 is used for the replacement treatment of insufficiency of the exocrine function of the pancreas in conditions such as:

  • subacute pancreatitis;
  • Schwachman-Diamond syndrome;
  • narrowing of the pancreatic duct, for example, due to a tumor or gallstones;
  • after irradiation of the organs of the gastrointestinal tract, accompanied by diarrhea, flatulence, impaired digestion of food (as part of a combination treatment);
  • conditions after complete or partial resection of the stomach, pancreas;
  • pancreas cancer;
  • cystic fibrosis;
  • chronic pancreatitis;
  • other pathologies accompanied by exocrine pancreatic insufficiency.

The drug is prescribed to patients with relative enzyme deficiency against the background of the use of hard-to-digest fatty or plant foods, irritable bowel syndrome, acute intestinal infections, and functional gastrointestinal tract disorders.

Pangrol 10,000 is also used in preparation for ultrasound and X-ray examination of the abdominal organs.

Contraindications

Absolute:

  • exacerbation of chronic pancreatitis;
  • acute pancreatitis;
  • individual intolerance to the components that make up the drug.

A relative contraindication (a condition in which the appointment of Pangrol 10000 requires caution) is pregnancy.

Instructions for the use of Pangrol 10000: method and dosage

Pangrol 10000 capsules are taken orally, without chewing (whole), with a sufficient amount of liquid. In cases where it is impossible to take a whole capsule (for example, elderly patients or small children), it is allowed to take its contents (mini-tablets). It is washed down with a liquid or added to liquid food that does not require chewing (for example, fruit juice, applesauce). To obtain the contents, the capsule is opened by separating the body from the cap. You cannot store a mixture of mini-tablets with liquid or food, it is taken immediately after preparation.

The dose of the drug is set individually, taking into account the severity of the pathology, the composition and volume of food consumed.

The recommended dose of Pangrol 10000 for adults is 2–4 pcs. at every meal.

Only the attending physician can increase the dose, taking into account the dynamics of symptoms (for example, relief of abdominal pain, reduction of steatorrhea).

It is not recommended to exceed the daily dose of enzymes - 15,000–20,000 lipase units per 1 kg of body weight.

The duration of the course is set by the doctor individually for each patient, taking into account the course of the disease.

For children, the doctor sets the dosage regimen and the duration of the course, taking into account the severity of the pathology and the composition of the food, based on the calculation - 500-1000 lipase units per 1 kg of body weight for each meal.

In cases of cystic fibrosis, the dose of Pangrol 10000 depends on the patient's weight: at the beginning of therapy for children under 4 years of age, it is 1000 lipase units per 1 kg of weight with each meal, over 4 years of age - 500 lipase units per 1 kg of weight with each meal.

The dose of the drug is selected individually, depending on the severity of the pathology, under the control of steatorrhea and subject to the optimal diet. In most cases, it should not exceed 10,000 lipase units per kg of body weight per day or 4000 lipase units per gram of fat consumed.

Side effects

Possible adverse reactions (> 10% - very common;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rare):

  • gastrointestinal tract: often - bloating, vomiting, nausea, diarrhea; very often - pain in the abdomen;
  • skin and subcutaneous tissue: rarely - rash, urticaria, itching.

Overdose

The main symptoms are hyperuricemia and hyperuricosuria with cystic fibrosis.

Therapy: cancellation of Pangrol 10000, symptomatic treatment.

special instructions

Taking high doses of the drug in patients with cystic fibrosis can cause fibrosing colonopathy (strictures of the colon, cecum, ileum).

In cases of unusual symptoms or changes in the nature of the symptoms of the underlying disease, a medical examination is recommended as a precautionary measure to exclude damage to the colon (especially when taking a dose of the drug in excess of 10,000 lipase units per kg of body weight per day).

Application during pregnancy and lactation

There are no clinical data on the therapy of pregnant women with drugs that include pancreatic enzymes. During pregnancy, taking Pangrol 10,000 is possible in cases where the predicted benefits of therapy to the mother are higher than the potential risk to the fetus.

During lactation, it is allowed to take the drug, since it does not have a harmful effect on the baby's body through breast milk (data obtained in the course of animal studies).

Drug interactions

There are no data on the interaction of Pangrol 10000 with other drugs.

Reviews about Pangrol 10000

According to reviews, Pangrol 10000 is an affordable and effective drug that promotes the absorption of food and improves the functioning of the pancreas.

Among the disadvantages are the difficulty in calculating the dosage. Since only the attending physician can correctly determine the dose of the drug, taking into account the severity of the disease, it is rather difficult to independently use the drug without its recommendation.
