Release form and composition
Dosage form - film-coated tablets: white or almost white, biconvex, oblong with rounded ends, a dividing line is applied on one side (5, 7, 10, 14, 15, 20 or 30 tablets in blisters from PVC film / printed aluminum lacquered foil; in a cardboard box 1, 2, 3 or 4 packages and instructions for use of Tolperisone-OBL).
Composition for 1 tablet:
- active substance: tolperisone hydrochloride - 50 or 150 mg;
- auxiliary components: citric acid, milk sugar (lactose monohydrate), hydroxypropyl cellulose (hyprolose), crospovidone, stearic acid;
- film casing: Opadray II (Series 85) [polyvinyl alcohol, macrogol (polyethylene glycol), titanium dioxide, talc].
Pharmacological properties
Pharmacodynamics
The active component of Tolperisone-OBL is tolperisone, a muscle relaxant of the central mechanism of action. It has a membrane stabilizing efficiency, slows down the conduction of nerve impulses in motor neurons and primary afferent fibers, which causes blocking of mono- and polysynaptic reflexes in the spinal cord. In all likelihood, it indirectly blocks the release of mediators by inhibiting the entry of Ca +2 ions into synapses. Inhibits the conduction of excitation along the reticulospinal tract in the brain stem.
Increases peripheral blood flow, regardless of the effect on the central nervous system (central nervous system). The development of this effect may also be influenced by the weak antispasmodic and antiadrenergic effects of tolperisone.
Pharmacokinetics
After oral administration, tolperisone is well absorbed from the small intestine. C max (maximum concentration) of a substance in blood plasma is reached after 0.5-1 hours.
Due to the pronounced presystemic metabolism, bioavailability is noted at the level of 20%. Tolperisone undergoes extensive metabolism in the liver and kidneys.
The substance is excreted in the urine, mainly (more than 99%) in the form of metabolites, the pharmacological activity of which is unknown.
Indications for use
Tolperisone-OBL is recommended for use in the following cases:
- therapy of pathologically increased tone and spasms of skeletal (striated) muscle tissue caused by organic neurological diseases, including lesions of the pyramidal tract, stroke, myelopathy, multiple sclerosis, encephalomyelitis;
- therapy of increased muscle tone and spasticity (painful muscle spasm), muscle contractures accompanying such diseases of the musculoskeletal system as spondylosis, arthrosis of large joints, spondyloarthrosis, lumbar and cervical pain syndromes;
- rehabilitation therapy after surgical interventions in traumatology and orthopedics;
- combined therapy of obliterating vascular diseases - obliterating atherosclerosis, diabetic angiopathy, thromboangiitis obliterans, Raynaud's disease, as well as pathologies caused by impaired vascular innervation - acrocyanosis, intermittent angioneurotic dysbasia.
Contraindications
- myasthenia gravis (myasthenia gravis);
- pregnancy and lactation period;
- children and adolescents under 18 years of age;
- lactose intolerance, lactase deficiency, glucose-galactose malabsorption (the tablets contain lactose);
- hypersensitivity to tolperisone and / or auxiliary components of the drug.
Tolperisone-OBL, instructions for use: method and dosage
Tolperisone-OBL tablets are intended for oral administration.
Adult patients are recommended to take the drug 2-3 times a day, starting with a dose of 50 mg, with a gradual increase to 150 mg.
The duration of the therapeutic course depends on the nature of the disease and the severity of its course.
Side effects
When taking Tolperisone-OBL, the development of such adverse reactions as headache, muscle weakness, arterial hypotension, gastralgia, nausea, vomiting is possible (usually disappear when the dose is reduced).
In rare cases, hypersensitivity reactions were observed, manifested by pruritus, erythema, urticaria, angioedema, bronchospasm, anaphylactic shock.
Overdose
Symptoms of an overdose of tolperisone are respiratory depression, a drop in blood pressure, and a weakening of cardiac activity.
In case of proven intoxication, symptomatic treatment is recommended.

During the period of taking Tolperisone-OBL, patients should be careful when driving and engaging in other potentially hazardous activities that require increased concentration of attention and high speed of psychomotor reactions.
Tolperisone-OBL tablets are contraindicated during pregnancy and lactation.
Pediatric use
In pediatric practice, the use of Tolperisone-OBL is contraindicated for the treatment of children under 18 years of age.
Drug interactions
Tolperisone affects the central nervous system, while it does not have a sedative effect, which makes it possible to use it in combination with hypnotics, sedatives and tranquilizers. Moreover, it does not exacerbate the effect of ethanol on the central nervous system.
Since the drug increases the severity of the effect of niflumic acid, with their simultaneous use, the dose of the latter can be reduced.
The simultaneous use of tolperisone with drugs for general anesthesia, peripheral muscle relaxants, psychoactive substances, clonidine enhances its clinical effect.
