Teraligen is a neuroleptic, antipsychotic drug.

Release form and composition

Dosage forms of Teraligen's release:

  • film-coated tablets: dark pink, with a strip on one side and with an embossed symbol on the other side (10 or 25 pieces in a blister strip, 1, 2 or 5 packs in a cardboard box);
  • solution for intramuscular administration and infusion: slightly colored or colorless, slightly opalescent or transparent liquid (5 ml in ampoules made of light-protective glass, 5 ampoules in a blister strip made of polyvinyl chloride film, 1 or 2 packs in a cardboard box).

Composition of 1 tablet:

  • active substance: alimemazine tartrate - 5 mg;
  • auxiliary substances: lactose - 90 mg, wheat starch - 42 mg, sucrose (refined sugar) - 14 mg, talc - 2 mg, tapioca starch (tapioca) - 2 mg, magnesium stearate - 1.5 mg;
  • shell: hypromellose - 4 mg, titanium dioxide - 0.1 mg, talc - 1 mg, macrogol 6000 - 0.4 mg, pink dye Osprey R110 - 0.3 mg.

Composition of 1 ml solution:

  • active substance: alimemazine tartrate (in terms of dry matter) - 5 mg;
  • auxiliary substances: ascorbic acid - 1 mg, sodium sulfite - 0.5 mg, water for injection - up to 1 ml.

Pharmacological properties

Pharmacodynamics

Pills

Teraligen is an antipsychotic drug (neuroleptic) that has an antispasmodic, antihistamine, serotonin-blocking and moderate alpha-adrenergic blocking effect, as well as a hypnotic, sedative, antiemetic and antitussive effect. The antipsychotic effect is due to the blockade of dopamine D2 receptors in the mesocortical and mesolimbic systems. Sedation is due to blockade of adrenergic receptors of the reticular formation of the brain stem; antiemetic effect - blockade of B2 receptors of the trigger zone of the vomiting center; hypothermic effect - blockade of dopamine receptors in the hypothalamus.

Teraligen has a low antipsychotic activity, therefore it is ineffective in acute psychotic conditions.

It is used in pediatric, adolescent and gerontological practice due to its good tolerance.

Solution

It is a phenothiazine derivative. In connection with the antihistaminic activity, alimemazine is prescribed for allergic reactions, especially the respiratory tract, and for itching of the skin. Alimemazine is more active in sedative and antihistamine action than diprazine. It has antitussive and antiemetic activity. It also acts as a mild sedative and anti-anxiety agent, and has a positive effect on senestopathy, phobia and obsession.

It is used for psychosomatic manifestations that develop as a result of neurovegetative disorders, for traumatic, vascular and infectious disorders of the functions of the central nervous system.

Sedation helps to normalize sleep in this category of patients.

The antipruritic effect of Teraligen is due to the effect on type I histamine receptors.

Vegeto-stabilizing and antiemetic action is caused by blockade of dopamine D2-receptors of the trigger zone of the vomiting center.

Anxiolytic and sedative action is caused by blockade of adrenergic receptors of the reticular formation of the brain stem.

Pharmacokinetics

Pills

Absorbed completely and quickly, after 1-2 hours after ingestion, the maximum concentration in plasma is observed. The connection with plasma proteins is 20-30%, the half-life reaches 3.5-4 hours. About 70-80% is excreted by the kidneys in the form of sulfoxide (metabolite) within 48 hours. The beginning of the effect - after 15–20 minutes, the duration of the effect is 6–8 hours.

Solution

With any route of administration, it is absorbed completely and quickly. The action of alimemazine begins 15–20 minutes after administration and lasts about 6–8 hours. The connection with plasma proteins is 20-30%. Metabolized in the liver. 70-80% is excreted by the kidneys in the form of sulfoxide (metabolite).

Indications for use

Teraligen is indicated for symptomatic treatment of allergic conditions of various origins (conjunctivitis, rhinitis, urticaria, pruritus, angioedema).

Additionally for tablets:

  • sleep disorders of various origins;
  • somatized mental disorders, senestopathic depression;
  • psychopathies with psychoasthenic and asthenic disorders;
  • neurosis-like states and neuroses of organic and endogenous genesis with a predominance of hypochondriacal, senestopathic, psychovegetative and phobic disorders;
  • anxiety-depressive states within the borderline vascular and endogenous diseases;
  • states of anxiety and excitement in case of somatic diseases.

Additionally for mortar:

  • postoperative period, and also as a sedative for premedication;
  • relief of symptoms in psychovegetative and neurotic disorders - as an anxiolytic and sedative drug;
  • combined therapy of alcohol withdrawal syndrome.

Contraindications

  • hypersensitivity to any component of the medication;
  • one-time therapy with monoamine oxidase inhibitors (you can use the drug no earlier than 2 weeks after stopping therapy with MAO inhibitors);
  • angle-closure glaucoma;
  • pregnancy and the period of breastfeeding.

Additionally for tablets:

  • severe kidney and liver disease;
  • hyperplasia of the prostate;
  • myasthenia gravis, parkinsonism, Reye's syndrome;
  • age up to 7 years.

Additionally for mortar:

  • apnea;
  • acute intoxication with narcotic analgesics and hypnotics, alcohol intoxication;
  • coma and deep depression of the central nervous system;
  • episodic manifestations of vomiting of unknown origin in children.

Teraligen should be used with caution in the following conditions / diseases:

  • inhibition of bone marrow function;
  • epilepsy;
  • jaundice;
  • predisposition to urinary retention;
  • alcoholism (in the event that there are indications of complications in the history of the use of drugs of the phenothiazine series);
  • obstruction of the bladder neck;
  • open-angle glaucoma;
  • arterial hypotension.

Instructions for the use of Teraligen: method and dosage

Pills

Teraligen is administered orally (by mouth). The daily dose should be divided into 3-4 doses.

For adult patients it is recommended to take 5-10 mg per day (hypnotic effect), 60-80 mg per day (anxiolytic effect). In psychotic conditions, you should take 200-400 mg per day.

Children from 7 years old Teraligen tablets are prescribed:

  • 2.5-5 mg per day (hypnotic effect);
  • 5–20 mg per day (as symptomatic therapy for allergic reactions);
  • 20-40 mg per day (anxiolytic effect).

An increase in the daily dose of up to 60 mg is allowed (for psychotic conditions).

Solution

Teraligen is administered intramuscularly. Care should be taken to avoid subcutaneous injection, which may result in local necrosis at the injection site.

The dosage is determined individually by the attending physician, depending on the degree of the disease.

Symptomatic treatment of allergic conditions - 1-2 times a day, 25 mg.

In psychiatric practice and in acute cases, adults are prescribed 100-200 mg once a day.

In the complex therapy of alcohol withdrawal syndrome, 50 mg is prescribed 1-2 times a day.

During premedication as a sedative - 1–2 hours before surgery 25–50 mg.

The maximum daily dose of Teraligen is 200 mg.

Side effects

Teraligen is usually well tolerated, side effects are mild and extremely rare.

Possible reactions:

  • sense organs: blurred visual perception (paresis of accommodation), ringing or tinnitus;
  • respiratory system: dry throat and / or nose, increased viscosity of bronchial secretions;
  • cardiovascular system: lowering blood pressure, dizziness, tachycardia;
  • digestive system: atony of the gastrointestinal tract, constipation, dry mouth, loss of appetite;
  • urinary system: urinary retention, atony of the bladder;
  • nervous system: fatigue, lethargy, drowsiness, usually occurring in the first days of admission and not always requiring discontinuation of the drug; paradoxical reaction (agitation, irritability, anxiety, nightmares); rarely - confusion, extrapyramidal disorders (hypokinesia, akathisia, tremor); increased seizure activity, increased frequency of sleep apnea (in children);
  • others: increased sweating, allergic reactions, muscle relaxation, inhibition of bone marrow hematopoiesis, photosensitization.

Overdose

In case of an overdose of Teraligen, an increase in the manifestations of the above side effects, in addition to allergic reactions, is possible.

special instructions

It is recommended to refrain from drinking alcohol during treatment.

During the therapy period, false positive pregnancy test results are possible.

The drug can mask the ototoxic effect (dizziness and tinnitus) when used in combination with other drugs. Also, alimemazine increases the body's need for riboflavin.

In order to prevent distortion of the results of skin scarification tests for allergens, the drug should be canceled 72 hours before allergy testing.

With prolonged treatment, liver function should be systematically evaluated, as well as a complete blood count.

Solution

It should be borne in mind that the composition of the solution for injection includes sulfites, which can enhance anaphylactic-type reactions.

Influence on the ability to drive vehicles and complex mechanisms

Application during pregnancy and lactation

  • beta-blockers: lower blood pressure, increase the risk of arrhythmias;
  • tricyclic antidepressants and anticholinergic drugs: enhance the m-anticholinergic activity of alimemazine;
  • hepatotoxic drugs: increase the manifestations of drug hepatotoxicity;
  • phenothiazine derivatives and MAO inhibitors: increase the risk of arterial hypotension and extrapyramidal disorders;
  • antiepileptic drugs and barbiturates: reduce the threshold of seizure activity (dose adjustment is necessary).

The effect of Teraligen on drugs / substances in combined use:

  • narcotic analgesics, hypnotics, anxiolytic (tranquilizers) and antipsychotic (neuroleptics) drugs, as well as antihypertensive drugs, drugs for general anesthesia, m-anticholinergics: enhances their effects (dose adjustment is necessary);
  • amphetamine derivatives, m-anticholinergics, ephedrine, guanethidine, dopamine: weakens their effect;
  • bromocriptine: weakens the effect of the latter and increases the concentration of prolactin in the blood serum.
