Release form and composition

1 capsule contains:
- active substance: tamsulosin - 0.4 mg (pellets - 267 mg, contain 0.15% tamsulosin hydrochloride);
- auxiliary components: sucrose, starch, ethyl cellulose, hypromellose, povidone K-30, copovidone (plasdon S-630), hypromellose phthalate, cetyl alcohol, diethyl phthalate, talc;

Pharmacological properties
Pharmacodynamics
Tamzelin is a drug for the treatment of dysuric disorders caused by benign prostatic hyperplasia. Its active substance, tamsulosin, is an alpha1-adrenergic blocker that selectively inhibits the functional activity of postsynaptic alpha1A-adrenergic receptors located in the smooth muscles of the prostate gland, the bladder neck, and the prostatic urethra. The selective and competitive action of tamsulosin helps to reduce smooth muscle tone and improves urine flow. In addition, the severity of symptoms associated with benign prostatic hyperplasia, such as obstruction and irritation of the urinary tract, decreases.
The clinical effect occurs after 14 days of continuous use of Tamzelin.
The high selectivity of tamsulosin makes it possible to avoid a clinically significant decrease in systemic blood pressure (blood pressure) in patients with normal baseline values and in patients with arterial hypertension. What is associated with the tropism of tamsulosin to the alpha1A-adrenergic receptors located in the bladder, which is 20 times greater than its ability to interact with the alpha1B-adrenergic receptors located in the smooth muscles of the vessels.
Pharmacokinetics
After taking Tamzelin capsules, more than 90% of tamsulosin is absorbed internally. Against the background of a single dose of 0.4 mg, the maximum plasma concentration is reached after 6 hours. Plasma protein binding is 94–99%. The volume of distribution is 0.2 l / kg.
It is metabolized in the liver, as a result of slow biotransformation, pharmacologically active metabolites are formed. In the blood, tamsulosin is present mainly unchanged.
It is excreted through the kidneys, including in unchanged form - 4-9%. The half-life is 10-12 hours.
Indications for use
The use of Tamzelin is indicated for benign prostatic hyperplasia for the treatment of dysuric disorders.
Contraindications
Absolute:
- age up to 18 years;
- hypersensitivity to drug components.
Tamzelin should be used with caution in chronic renal failure with creatinine clearance (CC) less than 10 ml / min, arterial hypotension (including orthostatic hypotension), severe hepatic failure.
Tamzelin, instructions for use: method and dosage
Tamzelin capsules are taken orally without chewing (swallowing whole) and drinking plenty of water after the first meal.
Recommended dosage: 0.4 mg once a day.
Side effects
- on the part of the male reproductive system: decreased libido, retrograde ejaculation, priapism;
- from the nervous system: sleep disorders (insomnia or drowsiness), asthenia, headache, dizziness;
- from the cardiovascular system: chest pain, palpitations, tachycardia, orthostatic hypotension, fainting;
- from the digestive system: constipation, diarrhea, nausea, vomiting;
- from the immune system: skin rash, itching, urticaria, angioedema;
- others: rhinitis, back pain.
Overdose
Symptoms of a tamsulosin overdose: a significant decrease in blood pressure, tachycardia.
For the treatment of intoxication, immediate gastric lavage, intake of activated carbon, sodium sulfate or other osmotic laxative is recommended. Giving the patient a horizontal position. Appointment of symptomatic therapy, careful monitoring of renal function. If necessary, the use of drugs that increase the volume of circulating blood and vasoconstrictor drugs is indicated.
Dialysis is ineffective.

During the period of treatment with Tamzelin, patients should take into account the possible lowering of blood pressure, which is characteristic of all alpha1-blockers, which in some cases can lead to fainting. In this connection, when dizziness or weakness appears, which are the first signs of orthostatic hypotension, it is necessary to sit or lie down and maintain this body position until blood pressure is completely normalized.
The appointment of Tamzelin should be based on the results of the examination, confirming the presence of benign prostatic hyperplasia in the patient. Before starting treatment and regularly during the period of therapy, a digital rectal examination is required and, if necessary, the determination of a prostate specific antigen (PSA).
Influence on the ability to drive vehicles and complex mechanisms
During the period of Tamzelin application, patients are advised to be careful when driving vehicles, complex mechanisms and performing other potentially dangerous types of work that require a high speed of psychomotor reactions and concentration of attention.
Application during pregnancy and lactation
The use of the drug in women is not provided.
Pediatric use
The use of Tamzelin for the treatment of patients under the age of 18 is contraindicated.
With impaired renal function
It is recommended to take Tamzelin with caution in patients with chronic renal failure (CC less than 10 ml / min). No dosage changes are required.
For violations of liver function
It is recommended to take Tamzelin with caution in patients with severe hepatic impairment.
Drug interactions
With simultaneous use with Tamzelin:
- cimetidine: the combination with cimetidine causes an increase in the concentration of tamsulosin in the blood plasma;
- furosemide: helps to lower the level of tamsulosin in the blood plasma;
- diclofenac, indirect anticoagulants: against the background of concomitant therapy with these agents, there is a slight increase in the rate of elimination of tamsulosin;
- diazepam, chlormadinone, propranolol, trichloromethiazide, amitriptyline, diclofenac, simvastatin, glibenclamide, warfarin: the combination with each of the listed agents in vitro does not affect the free fraction of tamsulosin in plasma. However, tamsulosin does not alter the free fractions of trichloromethiazide, diazepam, propranolol or chlormadinone;
- amitriptyline, salbutamol, glibenclamide, finasteride: no interaction with these agents at the level of hepatic metabolism in vitro was detected;
- beta-blockers, alprostadil, anesthetics, acetylcholinesterase inhibitors, diuretics, antidepressants, levodopa, slow calcium channel blockers, nitrates, other alpha1-blockers, ethanol: a combination with one of the listed agents increases the risk of increased severity of the hypotensive effect of tamsulosin.
