Release form and composition
The drug is produced in the form of tablets: flat-cylindrical, with a risk on one side, with a chamfer, from pale yellow with a green tint to yellow with a green tint, marbling is possible (10 pieces in a blister strip, in a cardboard box 2 or 10 packs and instructions for using Spazmonet).
1 tablet contains:
- active substance: drotaverine hydrochloride - 40 mg;
- additional components: croscarmellose sodium, corn starch, anhydrous colloidal silicon dioxide, lactose monohydrate, magnesium stearate, microcrystalline cellulose, talc.
Pharmacological properties
Pharmacodynamics
Drotaverine is a myotropic antispasmodic, close to papaverine in pharmacological properties and chemical structure, but with a longer and more pronounced effect. The active substance helps to reduce the intake of calcium ions into the cells of smooth muscles, to reduce the tone of smooth muscle tissue of internal organs and intestinal motility, as well as to expand blood vessels. Does not penetrate into the central nervous system (CNS), does not affect the function of the autonomic nervous system.
The ability of Spazmoneta to directly affect smooth muscles makes it possible to use it as an antispasmodic in cases where drugs from the group of m-anticholinergics are contraindicated for use (the presence of prostatic hypertrophy, angle-closure glaucoma).
Pharmacokinetics
For the drug when taken orally, high absorption is characteristic, the bioavailability of drotaverine is 100%. Penetrates into smooth muscle cells, is evenly distributed throughout the tissues, does not pass through the blood-brain barrier (BBB).
The maximum concentration (C max) of the active substance in the blood is observed 2 hours after administration, it binds to plasma proteins by 80–90%, the half-life (T ½) is 7–12 hours. The metabolic transformation of the agent proceeds intensively in the liver, is eliminated mostly with urine and less - with bile.
Indications for use
Spazmonet is recommended for use in case of spastic pain and spasms that occur against the background of the following conditions:
- renal colic, biliary colic, intestinal colic (spasm of smooth muscles of internal organs);
- cholecystitis, postcholecystectomy syndrome, dyskinesia of the gallbladder and biliary tract of the hyperkinetic type;
- spastic constipation, spastic colitis, proctitis, tenesmus;
- pyelitis;
- pylorospasm, gastroduodenitis, gastric ulcer and 12 duodenal ulcer (during an exacerbation; as part of a combination therapy to relieve spastic pain syndrome);
- endarteritis;
- spasm of the coronary, peripheral arteries, as well as cerebral arteries with concomitant headaches;
- threatening miscarriage / threatening premature birth (to reduce the excitability of the uterus), prolonged opening of the pharynx, spasm of the pharynx of the uterus during childbirth, postpartum contractions;
- algodismenorrhea.
Also, the drug is used to prevent the appearance of spasm during cholecystography and some other instrumental studies.
Contraindications
Absolute:
- chronic heart failure;
- cardiogenic shock;
- atrioventricular block (AV block) of II and III degrees;
- arterial hypotension;
- severe renal and / or hepatic impairment;
- glucose-galactose malabsorption, lactase deficiency, congenital galactose intolerance (antispasmodic agent contains lactose);
- hypersensitivity to any component of the drug.
Relative (it is required to use Spazmonet tablets with caution):
- severe atherosclerosis of the coronary arteries;
- age up to 6 years;
- hyperplasia of the prostate;
- angle-closure glaucoma.
Spazmonet, instructions for use: method and dosage
Spazmonet is taken orally. The tablets should be taken with a little liquid.
Recommended dosage regimen depending on the patient's age:
- children under 6 years of age: 1–2 times a day, 10–20 mg (- ½ tablets), the maximum daily dose is 120 mg;
- children from 6 to 12 years old: 2 times a day, 20 mg (½ tablet), the maximum daily dose is 200 mg;
- adolescents over 12 years old and adults: 3 times a day, 40–80 mg (1–2 tablets), the daily dose is 120–240 mg.
Side effects
During treatment with the drug, the following side effects may occur: increased sweating, a feeling of heat, dizziness, nausea, allergic skin reactions, palpitations, lowering blood pressure.
Overdose
When very large doses of the drug are taken, atrioventricular conduction is disturbed, the excitability of the myocardium decreases, which can lead to cardiac arrest and paralysis of the respiratory center.
special instructions
In the case of using Spazmonet for the treatment of gastric ulcer and duodenal ulcer, it must be taken in combination with antiulcer drugs.
Influence on the ability to drive vehicles and complex mechanisms
During treatment, it is required to refrain from driving vehicles and other complex, potentially dangerous mechanisms.
Application during pregnancy and lactation
The period of pregnancy and breastfeeding is not a contraindication for taking the drug. It is recommended to take Spazmonet tablets with caution in the first trimester of pregnancy and during lactation.
Pediatric use
Spazmonet is approved for use in pediatric practice, but children under 6 years of age should take it with caution.
With impaired renal function
In the presence of severe renal failure, Spazmonet is contraindicated.
For violations of liver function
With severe violations of the liver, drug therapy is contraindicated.
Drug interactions
- bendazole, papaverine and other antispasmodics (including m-anticholinergics): the effect of these drugs is potentiated;
- levodopa: the antiparkinsonian effect of this substance is weakened;
- procainamide, quinidine, tricyclic antidepressants: decreases high blood pressure caused by taking these drugs;
- morphine: its spasmogenic activity decreases;
- phenobarbital: the severity of the antispasmodic effect of drotaverine increases.
