Release form and composition

Pharmacodynamics

Metformin, the active substance of the drug, belongs to the biguanide group.

Siofor 1000's actions caused by metformin:

  • has an antihyperglycemic effect;
  • provides a decrease in the basal and postprandial blood plasma glucose concentration;
  • does not stimulate the secretion of insulin, and therefore does not cause hypoglycemia;
  • reduces the production of glucose in the liver by inhibiting glycogenolysis and gluconeogenesis;
  • increases muscle sensitivity to insulin, resulting in improved utilization and absorption of glucose in the periphery;
  • inhibits the absorption of glucose in the intestine;
  • stimulates intracellular synthesis of glycogen through the effect on glycogen synthetase;
  • increases the transport capacity of all currently known membrane transport proteins of glucose;
  • has a beneficial effect on lipid metabolism, reduces the concentration of total cholesterol, triglycerides and low density cholesterol.

Pharmacokinetics

Characteristics of metformin:

  • absorption: after oral administration, it is absorbed from the gastrointestinal tract, C max (maximum concentration in blood plasma) is achieved after 2.5 hours and when taking the maximum dose does not exceed 4 μg per 1 ml. During a meal, absorption decreases and slows down slightly;
  • distribution: accumulates in the kidneys, liver, muscles, salivary glands, penetrates into erythrocytes. The absolute bioavailability in healthy patients varies from 50 to 60%. It practically does not bind to blood plasma proteins, V d (average volume of distribution) - 63–276 l;

Contraindications

Absolute:

  • diabetic precoma, diabetic ketoacidosis;
  • impaired renal function or renal failure (creatinine clearance <60 ml per minute);
  • acute conditions that can negatively affect renal function, for example, severe infectious diseases, dehydration, intravascular administration of an iodine-containing contrast agent;
  • acute or chronic pathologies that can serve the development of tissue hypoxia, for example, a recent myocardial infarction, shock, respiratory or heart failure;
  • liver failure;
  • chronic alcoholism, acute alcohol poisoning;
  • lactic acidosis, including history;
  • stay on a low-calorie diet (<1000 kcal per day);
  • simultaneous intake of alcohol, ethanol-containing drugs;

  • pregnancy;
  • lactation period;

Instructions for the use of Siofor 1000: method and dosage

The tablets are taken orally.

The dosage of Siofor 1000 and the duration of the course of therapy are determined by the doctor individually, taking into account the concentration of glucose in the blood plasma.

Monotherapy in adult patients

The initial dose is 0.5 g (½ pcs.) 1 time per day during or after meals.

After 10-15 days from the start of treatment, a further gradual increase in the dose (taking into account the concentration of glucose in the blood plasma) is allowed to an average daily dose of 2 g (2 pcs.). With a gradual increase in the dose, the tolerance of the drug by the gastrointestinal tract improves.

The maximum daily dose is 3 g (1 pc. 3 times a day).

  • indirect anticoagulants: possibly weakening of their action;
  • angiotensin-converting enzyme inhibitors and other antihypertensive drugs: it is possible to reduce the concentration of glucose in the blood plasma;
  • salicylates, acarbose, insulin, sulfonylurea derivatives: hypoglycemic effect may increase.
