Dosage form Siofor 1000 - film-coated tablets: white, oblong, with a line on one and a wedge-shaped snap-tab on the other side (in blisters of 15 pcs., In a cardboard box of 2, 4 or 8 blisters).

Composition of 1 tablet:

  • active substance: metformin hydrochloride - 1 g;
  • auxiliary components: magnesium stearate - 0.005 8 g; povidone - 0.053 g; hypromellose - 0.035 2 g;
  • shell: titanium dioxide (E 171) - 0.009 2 g; macrogol 6000 - 0.002 3 g; hypromellose - 0.011 5 g.

Pharmacological properties

Pharmacodynamics

Metformin, the active substance of the drug, belongs to the biguanide group.

Siofor 1000's actions caused by metformin:

  • has an antihyperglycemic effect;
  • provides a decrease in the basal and postprandial blood plasma glucose concentration;
  • does not stimulate the secretion of insulin, and therefore does not cause hypoglycemia;
  • reduces the production of glucose in the liver by inhibiting glycogenolysis and gluconeogenesis;
  • increases muscle sensitivity to insulin, resulting in improved utilization and absorption of glucose in the periphery;
  • inhibits the absorption of glucose in the intestine;
  • stimulates intracellular synthesis of glycogen through the effect on glycogen synthetase;
  • increases the transport capacity of all currently known membrane transport proteins of glucose;
  • has a beneficial effect on lipid metabolism, reduces the concentration of total cholesterol, triglycerides and low density cholesterol.

Pharmacokinetics

Characteristics of metformin:

  • absorption: after oral administration, it is absorbed from the gastrointestinal tract, C max (maximum concentration in blood plasma) is achieved after 2.5 hours and when taking the maximum dose does not exceed 4 μg per 1 ml. During a meal, absorption decreases and slows down slightly;
  • distribution: accumulates in the kidneys, liver, muscles, salivary glands, penetrates into erythrocytes. The absolute bioavailability in healthy patients varies from 50 to 60%. It practically does not bind to blood plasma proteins, V d (average volume of distribution) - 63–276 l;
  • excretion: unchanged excreted by the kidneys, renal clearance - more than 400 ml in 1 min. T 1/2 (half-life) - about 6.5 hours. Clearance of metformin with a decrease in renal function decreases in proportion to creatinine clearance, respectively, the half-life is lengthened, and the concentration of the substance in the blood plasma increases.

Indications for use

Siofor 1000 is prescribed for patients with type 2 diabetes mellitus, especially with overweight, when diet therapy and physical activity are ineffective.

The drug is used in children over the age of 10 years as monotherapy or in combination with insulin, in adults - as monotherapy or as part of a combination treatment with other oral hypoglycemic drugs and insulin.

Contraindications

Absolute:

  • diabetic precoma, diabetic ketoacidosis;
  • impaired renal function or renal failure (creatinine clearance <60 ml per minute);
  • acute conditions that can negatively affect renal function, for example, severe infectious diseases, dehydration, intravascular administration of an iodine-containing contrast agent;
  • acute or chronic pathologies that can serve the development of tissue hypoxia, for example, a recent myocardial infarction, shock, respiratory or heart failure;
  • liver failure;
  • chronic alcoholism, acute alcohol poisoning;
  • lactic acidosis, including history;
  • stay on a low-calorie diet (<1000 kcal per day);
  • simultaneous intake of alcohol, ethanol-containing drugs;
  • age under 10;
  • pregnancy;
  • lactation period;
  • individual intolerance to the components that make up the drug.

Relative (conditions in the presence of which the appointment of Siofor 1000 requires caution):

  • age 10-12 years;
  • performing heavy physical work by persons over the age of 60 (the likelihood of developing lactic acidosis is high).

Instructions for the use of Siofor 1000: method and dosage

The tablets are taken orally.

The dosage of Siofor 1000 and the duration of the course of therapy are determined by the doctor individually, taking into account the concentration of glucose in the blood plasma.

Monotherapy in adult patients

The initial dose is 0.5 g (½ pcs.) 1 time per day during or after meals.

After 10-15 days from the start of treatment, a further gradual increase in the dose (taking into account the concentration of glucose in the blood plasma) is allowed to an average daily dose of 2 g (2 pcs.). With a gradual increase in the dose, the tolerance of the drug by the gastrointestinal tract improves.

The maximum daily dose is 3 g (1 pc. 3 times a day).

When the patient is transferred to a drug from another antidiabetic drug, the second is stopped and Siofor 1000 is taken in the above doses.

Insulin combination therapy in adults

Siofor 1000 can be combined with insulin to improve glycemic control. The initial dose is 0.5 g (½ pcs.) 1-2 times a day with a gradual increase in the dose at intervals of about 7 days to an average dose of 2 g (2 pcs.) Per day. The insulin dose is set based on the concentration of glucose in the blood plasma.

The maximum daily dose is 3 g (1 pc. 3 times a day).

Monotherapy and combined use with insulin in children 10-18 years old

The initial dose is 0.5 g (½ pcs.) 1 time per day during or after meals.

After 10-15 days from the start of treatment, a further gradual increase in the dose is allowed (taking into account the concentration of glucose in the blood plasma). With a gradual increase in the dose, the tolerance of the drug by the gastrointestinal tract improves.

The maximum dose is 2 g (2 pcs.) Per day, divided into 2 doses.

The insulin dose is set individually, depending on the concentration of glucose in the blood plasma.

Side effects

Possible adverse reactions (> 10% - very common;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rare):

  • nervous system: often - taste disturbance;
  • digestive system: very often - metallic taste in the mouth, loss of appetite, abdominal pain, diarrhea, vomiting, nausea;
  • skin: very rarely - urticaria, itching, hyperemia;
  • metabolism: very rarely - lactic acidosis (requires discontinuation of therapy);
  • liver and biliary tract: in some cases - reversible liver dysfunction (hepatitis or increased activity of hepatic transaminases, passing after the abolition of Siofor 1000).

Overdose

The main symptoms: lactic acidosis (reflex bradyarrhythmia, decreased blood pressure, hypothermia, abdominal pain, diarrhea, vomiting, nausea, drowsiness, respiratory disorders, severe weakness). Loss and confusion of consciousness, muscle pain are possible.

Therapy: hemodialysis.

special instructions

The accumulation of metformin can cause the development of lactic acidosis (an extremely rare pathological condition associated with the accumulation of lactic acid in the blood). There are reports of the development of this condition in patients, mainly with diabetes mellitus with severe renal failure, receiving metformin. For the prevention of lactic acidosis, it is recommended to take into account all associated risk factors (excessive alcohol consumption, prolonged fasting, ketosis, uncompensated diabetes, liver failure and any condition associated with hypoxia). In cases of suspicion of the development of lactic acidosis, Siofor 1000 is canceled, and the patient is hospitalized. Due to the fact that metformin is excreted by the kidneys, before starting therapy, and then regularly determine the concentration of creatinine in the blood plasma. Particular care is taken when there is a risk of impaired renal function, for example, at the beginning of treatment with non-steroidal anti-inflammatory drugs, diuretics or antihypertensive drugs.

2 days before and 2 days after the X-ray examination with intravenous administration of iodine-containing contrast agents, instead of the drug, another hypoglycemic agent (for example, insulin) is taken.

2 days before the planned surgical intervention under general anesthesia, epidural or spinal anesthesia, the drug is stopped. Treatment with Siofor 1000 is continued after the resumption of oral nutrition or no earlier than 2 days after the operation, provided that normal renal function is confirmed.

The drug is not a substitute for diet and daily exercise, patients are advised to combine these therapies in accordance with the recommendations of the attending physician. Patients during the period of treatment should adhere to a diet with a uniform intake of carbohydrates throughout the day. If you are overweight, it is important to follow a low-calorie diet.

During the treatment period, standard laboratory tests for diabetes mellitus are carried out regularly.

Before prescribing the drug for children under the age of 18, the diagnosis of type 2 diabetes should be confirmed.

With Siofor 1000 monotherapy, the development of hypoglycemia is not observed, but it is recommended to be careful when taking the drug in combination with sulfonylurea derivatives or insulin.

Influence on the ability to drive vehicles and complex mechanisms

Since when Siofor 1000 is taken together with other hypoglycemic drugs, hypoglycemic conditions may develop, patients in this case should be careful when driving vehicles or conducting potentially hazardous activities.

Application during pregnancy and lactation

The drug is contraindicated for use during pregnancy and lactation.

Patients should warn the doctor about pregnancy. When planning or the onset of pregnancy against the background of type 2 diabetes, Siofor 1000 is canceled, through insulin therapy, the concentration of glucose in the blood plasma is normalized or as close to normal as possible to reduce the risk of developing fetal defects due to the pathological effects of hyperglycemia.

Pediatric use

Siofor 1000 is contraindicated in children under 10 years of age. With caution, the drug is prescribed to children 10-12 years old.

With impaired renal function

The drug is contraindicated in renal failure, impaired renal function, as well as in cases of acute conditions that can negatively affect renal function.

For violations of liver function

According to the instructions, Siofor 1000 is contraindicated in patients with hepatic impairment.

Use in the elderly

Since elderly patients may have impaired renal function, the dose of Siofor 1000 in patients of this age group is selected taking into account the concentration of creatinine in the blood plasma. Regular assessment of the functional state of the kidneys is required.

Drug interactions

With the combined use of Siofor 1000 with some drugs / substances, the following effects may develop:

  • Danazol: possible development of a hyperglycemic effect;
  • nicotinic acid, phenothiazine derivatives, thyroid hormones, glucagon, epinephrine, oral contraceptives: the concentration of glucose in the blood plasma may increase;
  • nifedipine: absorption increases, maximum concentration in blood plasma and elimination of metformin is prolonged;
  • cationic agents secreted in the tubules (vancomycin, triamterene, ranitidine, quinine, quinidine, procainamide, morphine, amiloride): with prolonged treatment, an increase in the maximum concentration of metformin in blood plasma is possible;
  • cimetidine: excretion of metformin slows down, which increases the risk of developing lactic acidosis;
  • furosemide: its maximum concentration and half-life decreases;
  • indirect anticoagulants: possibly weakening of their action;
  • angiotensin-converting enzyme inhibitors and other antihypertensive drugs: it is possible to reduce the concentration of glucose in the blood plasma;
  • salicylates, acarbose, insulin, sulfonylurea derivatives: hypoglycemic effect may increase.
