Sedalgin Plus is a combined analgesic non-narcotic drug that includes an analgesic, a psychostimulant and a B vitamin.
Release form and composition
Release form Sedalgin Plus - tablets: from almost white to white, flat, round, with a risk on one side, with a chamfer on both sides (in blisters of 10 pcs., In a cardboard box 1 or 2 blisters).
1 tablet contains:
- active substances: metamizole sodium - 0.5 g; caffeine - 0.05 g; thiamine hydrochloride - 0.038 75 g;
- auxiliary components: magnesium stearate - 0.006 g; talc - 0.012 g; colloidal silicon dioxide - 0.002 5 g; microcrystalline cellulose - 0.045 75 g; wheat starch - 0.011 g; gelatin - 0.004 g.

Pharmacodynamics
Sedalgin plus is a combined agent with anti-inflammatory, antipyretic and analgesic effects.
The actions of the active components that make up Sedalgin plus:
- metamizole sodium: a pyrazolone derivative with a weak anti-inflammatory, antipyretic, analgesic effect, the mechanism of which is associated with inhibition of prostaglandin synthesis by inhibiting cyclooxygenase;
- caffeine: an alkaloid that has a stimulating effect on the central nervous system, mainly on the vasomotor and respiratory centers, cerebral cortex; reduces the feeling of fatigue, drowsiness, increases physical and mental performance; increases the bioavailability of non-narcotic analgesics and the permeability of histohematogenous barriers, thereby enhancing their therapeutic effect;
- thiamine: water-soluble vitamin B 1, helps to improve neuro-reflex conduction, takes part in metabolism.
Pharmacokinetics
Characteristic of metamizole sodium:
- absorption: quickly and completely absorbed in the gastrointestinal tract; TC max (time to reach maximum plasma concentration) varies from 60 to 90 minutes after oral administration;
- distribution: binds to plasma proteins;
- metabolism: metabolized in the liver; in the course of non-enzymatic hydrolysis, it is cleaved to 4-methylaminoantipyrine with further metabolism through active 4-aminoantipyrine to the main metabolite - 4-acetylaminoantipyrine;
- excretion: 90% of the main metabolite is excreted through the kidneys, 10% - with bile; T ½ (half-life) is approximately 10 hours.
Characteristic of caffeine:
- absorption: almost completely absorbed; C max (maximum concentration in blood plasma) is determined after 30–40 minutes;
- distribution: quickly passes through the placental and blood-brain barriers, enters breast milk;
- excretion: mainly (including metabolites) is excreted through the kidneys.
Thiamine characteristic:
- absorption: absorption of the substance is high, throughout the small intestine; released before absorption from the bound state by digestive enzymes; the concentration in the blood is relatively low;
- distribution: found in plasma mainly free thiamine, in leukocytes and erythrocytes - its phosphorus esters; half of the total amount is contained in the striated muscles (including the myocardium), approximately 40% - in the internal organs;
- phosphorylation: phosphorylated in the liver; thiamine diphosphate (the most active phosphoric ester) has coenzyme activity; accumulates mainly in the spleen, kidneys, brain, heart and liver;
- excretion: excreted through the kidneys and intestines.
Indications for use
- pain syndrome of various origins, moderately or mildly expressed: algodismenorrhea (menstrual pain), radicular syndrome, arthralgia (joint pain), myalgia (muscle pain), headache (including migraine), neuralgia, toothache (for therapy);
- febrile syndrome with colds and other infectious and inflammatory pathologies (with an established diagnosis).
Contraindications
Absolute:
- severe renal and / or hepatic impairment;
- uncontrolled arterial hypertension;
- pathology of the blood system;
- increased intraocular pressure;
- bronchospasm;
- severe atherosclerosis;
- insomnia;
- syndrome of congenital deficiency of the enzyme glucose-6-phosphate dehydrogenase (deficiency of glucose-6-phosphate dehydrogenase);
- age under 12;
- pregnancy;
- lactation period;
- individual intolerance contained in the composition of Sedalgin plus components, other derivatives of pyrazolone phenylbutazone.
Relative (diseases / conditions in the presence of which the administration of the drug requires caution): liver dysfunction; impaired renal function.
Instructions for use of Sedalgin Plus: method and dosage
Sedalgin Plus tablets are taken orally after meals, in the morning.
The recommended dose is 1 pc. 2-3 times a day.
Maximum single doses:
- adults and children over the age of 16: 2 pcs. per day; the maximum daily dose is 6 pcs. within 1–2 days (no more);
- children 12-16 years old: ½ – 1 pc. 2-3 times a day; the maximum daily dose is 4 pcs.
Duration of taking pills in cases of using the drug as an anesthetic - 5 days; as an antipyretic agent - 3 days.
If, while taking Sedalgin Plus, the symptoms of the disease continue or worsen, it is important to consult a doctor for advice.
Increasing the daily dose of the drug or using it for a longer period is permissible only under medical supervision.
Side effects
- gastrointestinal tract: jaundice, cholestasis, vomiting, nausea, loss of appetite;
- hematopoietic organs: purpura, hemolytic anemia, agranulocytosis, leukopenia, thrombocytopenia;
- central nervous system: irritability, dizziness, insomnia;
- cardiovascular system: heart rhythm disturbances, palpitations;
- allergic reactions: bronchospasm, anaphylactic shock, Quincke's edema, redness, pruritus, rash.
Overdose
The main symptoms: epileptic seizures, ringing in the ears, muscle twitching or tremors, increased pain or tactile sensitivity, headache, hyperthermia, dehydration, delirium, confusion, anxiety, convulsions, decreased / increased urine output, heart rhythm disturbances, tachycardia, motor and mental agitation, pain in the stomach, nausea and vomiting, sometimes with blood; in acute overdose - tonic-clonic seizures.
It is recommended to carry out gastric lavage, intake of adsorbents (for example, activated carbon), laxatives. If necessary, symptomatic treatment is carried out: against the background of epileptic seizures - intravenous administration of diazepam, phenobarbital or phenytoin, and maintaining the balance of salts and fluid; against the background of hemorrhagic gastritis - administration of antacids, gastric lavage with ice-cold 0.9% sodium chloride solution, and maintenance of oxygenation and ventilation of the lungs.

The development of symptoms of an overdose may be excessive consumption of coffee or tea (caffeinated products) while taking Sedalgin Plus.
It is not recommended to drink alcohol during treatment.
The risk of developing hypersensitivity reactions is increased with pollinosis and atopic bronchial asthma.
Influence on the ability to drive vehicles and complex mechanisms
Patients during the period of taking Sedalgin Plus tablets should be careful when driving and conducting potentially hazardous activities (in view of the possible development of dizziness).
Application during pregnancy and lactation
According to the instructions, Sedalgin Plus is contraindicated for use during pregnancy and lactation.
Pediatric use
For children under 12 years of age, the appointment of Sedalgin Plus is contraindicated.
With impaired renal function
- severe renal failure: Sedalgin Plus is contraindicated;
- renal dysfunction: use with caution.
For violations of liver function
- severe hepatic impairment: Sedalgin Plus is contraindicated;
- liver dysfunction: used with caution.
Drug interactions
With the combined use of metamizole sodium with some drugs / substances, the following effects may develop:
- other non-narcotic analgesics, non-steroidal anti-inflammatory drugs: mutual enhancement of the toxic effects of drugs;
- allopurinol, oral contraceptives, tricyclic antidepressants: increase its toxicity and disrupt liver metabolism;
- butadione (phenylbutazone), barbiturates and other inducers of microsomal liver enzymes: weaken its effect;
- cyclosporine: metamizole sodium reduces its concentration in the blood;
- tranquilizers, sedatives: enhance its analgesic effect.
