Release form and composition

Dosage form Sanoxal - chewable tablets: round, flat, light orange interspersed with lighter and darker colors, with a risk on one side (1, 2 or 3 pcs. In blisters, in a cardboard box 1 package).

Composition of 1 tablet:

  • active substance: albendazole - 400 mg;
  • auxiliary components: magnesium stearate, sodium benzoate, lactose, sucrose, menthol (levomenthol), talc, purified water, sunset yellow dye (sunset yellow dye), peppermint flavor (peppermint essence).

Pharmacological properties

Albendazole is a benzimidazole derivative. The mechanism of its action is due to the ability to selectively suppress the polymerization of beta-tubulin, as a result of which the destruction of cytoplasmic microtubules of the cells of the intestinal tract of helminths occurs.

The drug changes the course of biochemical processes (suppresses glucose utilization), prevents the movement of secretory granules and other organelles in the muscle cells of roundworms, which is why they die.

Sanoxal is especially effective against Strongyloides stercoralis nematodes, larval forms of Taenia solium and Echinococcus granulosus cestodes.

Pharmacokinetics

After oral administration, albendazole is poorly absorbed in the gastrointestinal tract, since it is poorly soluble in water. It is rapidly metabolized during the "first pass" through the liver, therefore, it is not detected in the unchanged form in plasma, or is determined in very low concentrations.

It is characterized by low bioavailability. However, fatty foods significantly enhance the absorption of albendazole and increase its plasma concentration by about 5 times.

Albendazole sulfoxide (the main metabolite) reaches its maximum concentration after 2-5 hours.

It binds to plasma proteins by 70%. Well distributed throughout the body. High concentrations are found in the liver, bile, urine, cerebrospinal fluid, the wall and fluid of parasitic cysts.

It is rapidly metabolized in the liver to form albendazole sulfoxide - the primary metabolite, which also has an anthelmintic effect. Albendazole sulfoxide, in turn, is converted to a secondary metabolite of albendazole sulfone and other oxidized products devoid of pharmacological activity.

The half-life (T ½) is 8-12 hours. It is excreted in the form of metabolites mainly in the bile, excretion in the urine is insignificant (less than 1%).

Albendazole is an inducer of CYP1A2 microsomal enzymes of the cytochrome P 450 system in liver cells.

In patients with concomitant impairment of hepatic function, the bioavailability of albendazole increases. The maximum concentration of the primary metabolite is doubled, the period T ½ is lengthened.

With concomitant impairment of renal function, the clearance of albendazole and its metabolites does not change.

Indications for use

  • helminthiasis;
  • trematodes: opisthorchiasis;
  • nematodes: ankylostomiasis, ascariasis, strongyloidiasis, trichocephalosis, enterobiasis;
  • Echinococcosis of the liver, peritoneum, or lungs caused by the larval form of the canine tapeworm (Echinococcus granulosus);
  • neurocysticercosis caused by the larval form of pork tapeworm (Taenia solium);
  • some protozoal infections: microsporidiosis, giardiasis, mixed.

As an adjuvant, Sanoxal is used in the surgical treatment of echinococcosis cysts.

Contraindications

  • age up to 3 years;
  • individual intolerance to the components of the drug or other benzimidazole derivatives.

According to the instructions, Sanoxal is used with extreme caution in liver cirrhosis, liver failure, inhibition of bone marrow hematopoiesis and cysticercosis with the involvement of the retina.

Instructions for use of Sanoxal: method and dosage

Sanoxal is indicated for oral administration. The tablets can be swallowed whole or chewed.

The optimal dose is determined by the doctor depending on the type of helminthic invasion and the patient's body weight.

Recommended dosage regimens for echinococcosis and neurocysticercosis:

  • patients with body weight ≥ 60 kg - 400 mg 2 times a day for 8-30 days;
  • patients with body weight <60 kg - in a daily dose of 15 mg / kg in 2 divided doses, but not more than 800 mg per day, the course of treatment is 28 days. In total, 3 courses are carried out at intervals of 14 days.

For trichocephalosis, ankylostomiasis, enterobiasis and ascariasis for adults and children from 3 years of age, Sanoxal is prescribed once in a dose of 400 mg. If necessary, a single dose is repeated after 3 weeks.

In the case of giardiasis, adults and children over 3 years old are recommended to take 400 mg 1 time per day for 3 days.

With strongyloidosis and cestodosis, the recommended daily dose for adults and children from 3 years of age is 400 mg, the duration of treatment is 3 days. If necessary, a second course of therapy is carried out after 3 weeks.

Side effects

  • from the digestive system: nausea, abdominal pain, vomiting, abnormal liver function and changes in liver function tests (a weak or moderate increase in the activity of hepatic transaminases);
  • from the nervous system: dizziness, headache, meningeal symptoms;
  • on the part of the hematopoietic system: oppression of bone marrow hematopoiesis (agranulocytosis, pancytopenia, leukopenia, thrombocytopenia, granulocytopenia);
  • allergic reactions, including skin rashes, itching;
  • others: increased blood pressure, increased body temperature, reversible alopecia, acute renal failure.

Overdose

In case of an overdose, gastric lavage, the appointment of activated charcoal and symptomatic therapy aimed at eliminating the disturbances that have occurred are indicated.

In neurocysticercosis, Sanoxal is used in combination with the appropriate anticonvulsant and glucocorticosteroid therapy. Glucocorticosteroids (oral or intravenous) are used to prevent a spike in blood pressure during the first week of anti-cyst therapy.

During the period of treatment with albendazole, it is recommended to monitor the cellular composition of the blood: every 2 weeks and at the beginning of each 28-day cycle.

If leukopenia develops, the intake of Sanoxal is suspended. Therapy is resumed if the decrease in the total content of leukocytes and neutrophilic leukocytes is moderate and does not progress.

Patients with concomitant impairment of hepatic function require regular monitoring (every 2 weeks). In case of a significant increase in hepatic transaminases, Sanoxal is canceled. After the return of this indicator to the initial value, treatment can be resumed, but it should be carried out under more careful monitoring of laboratory parameters.

If neurocysticercosis is accompanied by eye damage, Sanoxal is prescribed after examining the retina to assess the feasibility and risks of taking the drug (which is associated with the risk of worsening the pathology).
