Ringer-SOLOpharm is a remedy that restores the balance of electrolytes.

Release form and composition

Dosage form - solution for infusion: clear, colorless liquid [in a cardboard box, instructions for use of Ringer-SOLOpharm and one polypropylene bottle (PolyFlak or PolyFlak EN) containing 100, 200, 250, 400, 500 or 1000 ml of solution; packing for hospitals - in a box of 1–40 bottles of 100, 200, 250, 400, 500 or 1000 ml together with the corresponding number of medical instructions for the preparation].

Composition of 100 ml solution:

  • active substances: sodium chloride - 860 mg; potassium chloride - 30 mg; calcium chloride hexahydrate - 25 mg (in terms of anhydrous substance);
  • auxiliary component: water for injection - up to 100 ml.

Ionic composition of the preparation per 1 liter: chloride ion (Cl -) - 155.7 mmol; sodium ion (Na +) - 147.2 mmol; potassium ion (K +) - 4.0 mmol; calcium ion (Ca ++) - 2.25 mmol. The theoretical osmolarity is 309 mOsm / L.

Pharmacological properties

Pharmacodynamics

Ringer-SOLOpharm is a rehydrating drug that stabilizes the electrolyte and water composition of the blood and has a detoxifying effect.

Properties of the active ingredients of the medicinal product:

  • Chlorine: is the main extracellular anion that closely correlates with sodium metabolism and is involved in the regulation of the acid-base balance of the body;
  • sodium: the main cation of extracellular fluid, which controls water distribution, water balance, osmotic pressure of body fluids. In the regulation of the acid-base state of liquids, it is associated with bicarbonate and chlorine;
  • potassium: is the main cation of the intracellular fluid and is involved in the synthesis of proteins, the metabolism of carbohydrates, is necessary for the normal conduction of nerve impulses and muscle contraction;
  • calcium: in an ionized form, it takes part in the mechanisms of blood coagulation and the regulation of neuromuscular excitability, it is needed for the normal function of the heart.

When Ringer-SOLOpharm is prescribed, the solution is used only for a short time as a means of replenishing the circulating blood volume, since it quickly leaves the bloodstream into the intercellular space, which is why the effect lasts no more than 40 minutes.

Pharmacokinetics

Pharmacokinetic information refers to the ions contained in the drug.

The Na + and Cl - ions introduced with the Ringer-SOLOpharm solution are characterized by the same pharmacokinetics as the ions entering the body with food. They are easily distributed to all organs, tissues and intercellular spaces. With glomerular filtration, they are secreted in the kidneys, where they are significantly reabsorbed in the tubules (mainly in the distal tubules and loop of Henle).

K + ions are freely filtered in the renal glomeruli, and are reabsorbed almost completely in the proximal tubules, therefore, no more than 10% of the filtered ions are excreted from the body. Elimination of K + can be significantly increased by secretion in the collecting ducts and distal tubules of the nephron. The ability of the kidneys to maintain potassium levels is limited. In this regard, at a high concentration of this electrolyte in the distal tubules, a significant loss of potassium is possible, which leads to the development of hypokalemia. That is why K + ions are included in the Ringer-SOLOpharm.

Homeostasis of Ca ++ ions is well controlled by human hormones, so intravenous infusion of the solution is rarely necessary.

The drug is not metabolized. It is quickly excreted from the body: 80% - within 4 hours, complete excretion occurs within 12-24 hours.

Indications for use

Ringer-SOLOpharm solution is used to stabilize the water and electrolyte composition of blood, and as a detoxifying agent in the treatment of the following conditions / diseases:

  • electrolyte imbalance and dehydration (with diarrhea, prolonged vomiting, frostbite, thermal burns of III – IV degree);
  • intestinal obstruction and acute diffuse peritonitis (to correct the water-salt balance);
  • acute intestinal infection, intestinal fistula;
  • acute blood loss, hypovolemic shock, therapeutic plasmapheresis.

Ringer-SOLOpharm is also used for diluting other electrolyte solutions with high concentration.

Contraindications

Absolute:

  • hypervolemia;
  • hyperchloremia, hypernatremia;
  • disorders of acid-base balance: metabolic alkalosis, acidosis;
  • swelling of the brain;
  • pulmonary edema;
  • oliguria / anuria;
  • severe chronic renal failure;
  • hypercoagulation;
  • thrombophlebitis;
  • chronic heart failure (NYHA functional class III – IV);
  • simultaneous use of glucocorticosteroids;
  • lactation period;
  • hypersensitivity to any component of Ringer-SOLOfarm solution.

Relative:

  • arterial hypertension or chronic heart failure (NYHA class I-II), especially in the elderly and postoperative patients;
  • impaired liver function;
  • mild to moderate chronic renal failure;
  • concomitant use of blood preparations or cardiac glycosides;
  • period of pregnancy.

Ringer-SOLOfarm, instructions for use: method and dosage

Ringer-SOLOpharm solution is administered intravenously by jet or drip using sterile equipment. The intravenous system should be changed at least once a day.

The rate of administration of the drug with the drip method for adults is 60–80 drops per minute and 70–90 drops per minute for severe conditions, for children - 30–60 drops per minute. The required volume of solution is determined individually, depending on the type of dehydration and the degree of dehydration of the body, the patient's body weight and the cause of shock.

Before the introduction, a medical professional should perform a visual examination of the drug. It can be used only when the bottle is completely tight and the solution is transparent.

Recommended daily doses:

  • adults: 5–20 mg / kg, maximum 50 ml / kg;
  • children: 5–10 mg / kg, with shock dehydration - 20–30 mg / kg.

The maximum volume of the injected solution is 3 liters per day. The duration of therapy is 3 to 5 days.

In case of therapeutic plasmapheresis, the volume of the Ringer-SOLOpharm solution administered is twice the volume of the removed plasma and is usually 1.2–2.4 liters. With severe hypovolemia, the simultaneous administration of colloidal solutions is indicated.

For patients with acute intestinal infections of mild and moderate severity, intravenous administration of Ringer-SOLOpharm is prescribed only if it is impossible to rehydrate with oral medications.

With long-term treatment with Ringer-SOLOpharm in large doses, it is recommended to control the content of electrolytes in urine and plasma.

Rules for the use of bottles PolyFlak EH (with a head for connecting a standard infusion system):

  1. Place the bottle vertically on a horizontal surface.
  2. Remove the cap.
  3. Disinfect the port of the bottle with a disinfectant.
  4. Prepare the infusion set.
  5. Insert the injection needle / spike into the chamfer marked on the port at a right angle until it touches the butterfly of the injection needle / dropper skirt.
  6. Turn the bottle over and fix the system on a tripod (infusion rack).

The drug can be injected only through the bevel marked on the bottle port. Do not insert the infusion set into the vial on a tripod (i.e., suspended). When forming a "lake" in the infusion system, press on the dropper, not on the bottle.

Side effects

Ringer-SOLOpharm is well tolerated. In some cases, the development of allergic reactions, hyperkalemia, overhydration, chloride acidosis is noted.

Overdose

Exceeding the recommended doses of a rehydrating agent can cause a violation of the acid-base state, water and electrolyte balance (hypercalcemia, hypernatremia, hypervolemia, hyperchloremia).

In most cases, it is sufficient to stop the administration of Ringer-SOLOfarm. If necessary, symptomatic treatment is carried out.

special instructions

Rehydration therapy causes a decrease in blood pH, which leads to an increase in serum potassium ions. In this regard, with the rapid introduction of a large volume of Ringer-SOLOpharm solution and with prolonged parenteral therapy, laboratory parameters and the patient's clinical condition should be monitored in order to monitor the electrolyte content and acid-base state.

Intravenous fluids can lead to fluid and / or drug overload, overhydration, congestion, and pulmonary edema. Between the concentration of electrolytes and the development of dilution, a direct proportionality was noted, an inverse proportionality was observed between the content of electrolytes and the risk of overloading the body with fluid (leads to the appearance of congestion with peripheral edema and pulmonary edema).

Ringer-SOLOfarm contains sodium ions, which should be used with caution in conditions accompanied by edema and sodium retention, as well as when corticotropin is administered together.

Due to the potassium content, treatment with Ringer-SOLOpharm requires taking precautions for conditions accompanied by potassium retention in the body, hyperkalemia, severe renal failure, concomitant heart disease.

The drug can cause retention of potassium and sodium in patients with reduced renal excretory function.

Solutions containing calcium are administered under the control of an electrocardiogram, especially for patients undergoing treatment with digitalis drugs (for example, digitalis). It should be borne in mind that the concentration of calcium in the serum does not always correspond to its level in the tissues. With the simultaneous administration of blood preparations, there is a risk of developing coagulation. Parenteral calcium administration should be done with caution in persons receiving cardiac glycosides.

Ringer-SOLOpharm is not intended for long-term use due to the presence of a high level of chlorine in its composition.

If an undesirable reaction occurs during infusion, the drug should be discontinued and the clinical condition of the patient should be assessed, if necessary, appropriate therapeutic measures should be taken.

When other medicines are used together, their possible side effects must be taken into account.

To reduce the risk of incompatibility that may appear when mixing Ringer-SOLOfarm with other solutions, the final infusion solution should be checked for turbidity and precipitation: immediately after mixing, before administration and periodically during the infusion.

Influence on the ability to drive vehicles and complex mechanisms

Studies evaluating the degree of influence of Ringer-SOLOpharm on human cognitive and motor functions have not been conducted.

Application during pregnancy and lactation

Separate studies to study the possible undesirable effects of Ringer-SOLOpharm during pregnancy and lactation have not been conducted. During gestation, the drug is used only in exceptional cases, when the need for administration of the solution is clinically justified, while the benefits of therapy for the mother are higher than the potential risks for the fetus.

Due to the lack of clinical data on the safety of Ringer-SOLOfarm during lactation, you should refrain from breastfeeding during its use.

Pediatric use

In children, the drug is used according to indications, adhering to the recommended dosage regimen.

With impaired renal function

Ringer-SOLOfarm is contraindicated in severe chronic renal failure.

The drug should be used with caution in patients with mild to moderate renal impairment.

For violations of liver function

Liver dysfunction is a relative contraindication to the use of Ringer-SOLOpharm, therefore, treatment is carried out only after assessing the benefits / risks under close medical supervision.

Drug interactions

When using Ringer-SOLOfarm solution and other drugs together, their compatibility should be visually checked.

Potassium preparations, potassium-sparing diuretics and angiotensin-converting enzyme (ACE) inhibitors, when used simultaneously with Ringer-SOLOpharm solution, increase the likelihood of hyperkalemia. Vasodilators, ganglion blockers, corticotropin, nonsteroidal anti-inflammatory drugs, mineralocorticoids, anabolic hormones, androgens, and estrogens can increase sodium retention in the body.

Ringer-SOLOfarm may increase the toxicity of cardiac glycosides.
