Release form and composition
Dosage form Lavasepta is a concentrate for the preparation of a solution for external and local use: a colorless, transparent, odorless liquid (2 or 5 ml in vials made of hydrolytic glass of class 1, sealed with rubber stoppers with aluminum rolling and plastic colorless plugs; 100 ml in vials from hydrolytic glass of class 2, sealed with rubber stoppers with aluminum rolling; in a cardboard box 1 bottle of 5, 20 or 100 ml).
Composition of 1 ml of concentrate:
- active ingredient: polyhexanide - 200 mg;
- auxiliary ingredients: macrogol-4000 - 10 mg, water for injection up to 1 ml (pH 5.0-7.0).
Pharmacological properties
Pharmacodynamics
The active component of Lavasept - polymeric biguanide polyhexanide, has a disinfecting effect of a broad antimicrobial spectrum, is active against both gram-positive and gram-negative bacteria, including Pseudomonas aeruginosa, and fungi.
An auxiliary component of the concentrate - polyethylene glycol (macrogol-4000), reduces surface tension, helping to improve the performance of polyhexanide.
The effect of the drug in the presence of protein (albumin 0.2%) is weakened slightly; when conducting quantitative tests, it shows high fungicidal and bactericidal effectiveness.
The composition of Lavasept does not contain irritating substances such as halogen- and mercury-containing antiseptics, iodine, heavy metals, polyvinylpyrrolidone and aldehydes.
Polyhexanide is a chemically stable, odorless, non-volatile, soluble in water, glycerin, ethanol, propylene glycol, and other solvents. It has low toxicity, and the polyethylene glycol contained in the Lavasept concentrate is not enough to increase the toxicity of polyhexanide.
Pharmacokinetics
Systemic absorption of a solution prepared from Lavasept concentrate is not observed when applied externally. The concentration of polyhexanide recorded in blood and urine was lower than the possible detection level - 10 μg / ml.
The substance was also not detected in the systemic circulation and urine when washing or constant irrigation of extensive surgical wounds during operations without the use of a tourniquet, lasting 2 hours or more, and when dressing with Lavasept for dressing wounds for several weeks (the maximum recorded period - 58 weeks).
Indications for use
- antiseptic treatment of superficial injuries of soft tissues and infected wounds during their surgical treatment;
- washing and sanitizing cavities using active drainage through drainage tubes;
- irrigation (irrigation) of the operating field in the process of surgical intervention during reconstructive operations, immediately before bone and / or soft tissue transplantation;
- irrigation (irrigation) of the operating field during surgical interventions with the existing threat of infection;
- therapy of infected wounds during hip replacement surgery (according to Schneider);
- antiseptic treatment of open wounds and burn surfaces;
- antiseptic treatment in maxillofacial surgery;
- disinfection of surgical instruments, dressings and patient care items.
Contraindications
- surgical interventions on uninfected joints;
- neurosurgical operations (surgery of the brain and spinal cord);
- surgical interventions in the middle and inner ear;
- ophthalmic surgery;
- lavage of intraperitoneal organs;
- hypersensitivity to the components of Lavasept.
Instructions for the use of Lavasept: method and dosage
Lavasept concentrate is intended only for external use in the form of a diluted solution.
The concentration of the solution used according to the indications:
- washing and draining wounds - 0.1%;
- treatment of wound surfaces infected with gram-negative flora - 0.2%.
To prepare a working solution with a concentration of 0.1% or 0.2%, add 1 or 2 ml (respectively) of Lavasept concentrate to Ringer's solution in a volume of 1000 ml.
Mixing the concentrate with other solutions is allowed provided that the liquid becomes cloudy and a precipitate is avoided.
Polyhexanide, which precipitates at low temperatures, can be dissolved by heating the solution to 60 ° C.
Side effects
The negative side effects observed when using Lavasept are manifested in the form of the following allergic reactions: skin rash and / or urticaria.

Allergic reactions that appear when using Lavasept require immediate discontinuation of therapy and the appointment of antiallergic drugs (antihistamines).
When using Lavasept, it is necessary to avoid its retention in body tissues.
In the course of experimental studies on animals, it was revealed that polyhexanide inhibits the growth of cartilage tissue in embryos. Prolonged use of the drug can lead to the destruction of intact cartilage, and therefore do not wash uninfected joints with Lavasept. In case of unintentional contact of the solution during surgery on the intact cartilage tissue of the uninfected joint, the entire joint should be flushed with saline or Ringer's solution.
According to the instructions, Lavasept, when used on significantly affected areas, inflamed / infected joints, when planning arthrodesis or total joint replacement, is able to provide an optimal level of disinfection, which guarantees a favorable outcome of the operation.
Application during pregnancy and lactation
There is currently insufficient data on the use of Lavasept during pregnancy and lactation.
In the first trimester, it is not recommended to use the solution. In later stages of pregnancy and during breastfeeding, a careful assessment of the ratio of benefits to the mother with the risk to the fetus / child is required before use.
Pediatric use
In pediatric practice, it is allowed to use a solution at a concentration of 0.1%. Since there is no experience with the use of Lavasept in children under 3 years of age, its use in patients of this age group is allowed only after a thorough assessment of the risk / benefit ratio.
Drug interactions
To date, no cases of drug interaction of Lavasept with other drugs / preparations have been recorded.
