Diprosalik lotion is a combined external preparation with glucocorticosteroid and keratolytic action.

Release form and composition

Dosage form - solution for external use: viscous translucent colorless liquid without visible particles with a characteristic smell of isopropyl alcohol (30 ml in plastic dropper bottles, each bottle is placed in a cardboard box).

Active ingredients in 1000 mg solution:

  • salicylic acid - 20 mg;
  • betamethasone dipropionate - 0.64 mg, which is equivalent to the content of betamethasone - 0.5 mg.

Additional components: isopropanol - 392 mg, sodium hydroxide - 5 mg, hypromellose - 5 mg, disodium edetate - 0.3 mg, purified water - up to 1000 mg.

Pharmacological properties

Pharmacodynamics

Diprosalik lotion is a combined preparation, the action of which is due to the properties of its active substances.

Salicylic acid, when applied topically, has a keratolytic, bacteriostatic and some fungicidal effect.

Betamethasone dipropionate is a synthetic fluorinated glucocorticosteroid (GCS) that has anti-inflammatory, vasoconstrictor and antipruritic effects.

Indications for use

  • seborrhea, including the scalp;
  • psoriasis, including scalp;
  • other dermatoses sensitive to the action of glucocorticosteroids.

Contraindications

Absolute:

  • perioral dermatitis;
  • rosacea;
  • post-vaccination skin reactions;
  • trophic ulcers against the background of chronic venous insufficiency;
  • fungal, bacterial and viral skin infections (blastomycosis, actinomycosis, sporotrichosis, pyoderma, chickenpox, herpes simplex, shingles, skin tuberculosis, syphilis);
  • formations / tumors of the skin (atheroma, nevus, melanoma, hemangioma, epithelioma, xanthoma, skin cancer, sarcoma);
  • pregnancy and lactation;
  • children under 2 years old;
  • hypersensitivity to any component of Diprosalic lotion.

The drug should be used with caution for the treatment of children aged 2–12 years, with hepatic insufficiency, as well as if long-term therapy is necessary.

Instructions for use of Diprosalik lotion: method and dosage

Diprosalic lotion is applied externally.

The solution must be applied in a thin layer to the affected skin 2 times a day - in the morning and at night. In some cases, a supportive effect can be achieved with less frequent applications.

The duration of treatment is determined individually depending on the effect, the tolerability of the drug, and the presence and severity of its side effects.

Side effects

Local reactions: dry skin, itching, burning, irritation, perioral dermatitis, acne, folliculitis, pain, striae, hypopigmentation, prickly heat, skin maceration, hypertrichosis, skin cracks, purpura, dermatitis, allergic contact dermatitis, telangiectasia, secondary local hirsutism, skin atrophy.

With prolonged use of Diprosalic lotion or in high doses, especially in children, there is a risk of developing side effects inherent in systemic GCS, including such as Cushing's syndrome, suppression of the function of the hypothalamic-pituitary-adrenal system, increased intracranial pressure, insufficient body weight gain, growth retardation.

Salicylic acid can cause dermatitis.

Overdose

With long-term local use of the drug in high doses, the dipropionate, which is part of Diprosalik, betamethasone lotion, can cause suppression of the pituitary-adrenal system, which is fraught with the development of hypercortisolism, including Cushing's syndrome, and secondary adrenal insufficiency.

Salicylic acid applied topically for a long time can cause salicylate poisoning, the early symptoms of which are nausea and vomiting.

Overdose treatment is symptomatic. If necessary, the electrolyte imbalance is corrected. In case of poisoning with salicylic acid, urgent measures should be taken to remove it from the body, for example, to appoint sodium bicarbonate inside (it alkalizes urine and forces diuresis).

Diprosalic lotion is not intended for the treatment of ophthalmic diseases.

Avoid contact with eyes and mucous membranes.

Do not apply the drug under occlusive dressings, on open wounds and areas with damaged skin.

Avoid use in the anogenital area.

Do not apply on face skin for a long time.

If dryness or irritation of the skin appears, the development of hypersensitivity reactions, the drug should be canceled.

Systemic absorption of the active ingredients of Diprosalic lotion when applied topically is enhanced when the solution is applied to large areas of the body. Appropriate precautions should be taken if long-term treatment is required, especially in children.

In case of infection, appropriate therapy is indicated.

With prolonged therapy, gradual withdrawal of Diprosalik lotion is recommended.

Pediatric use

According to the instructions, Diprosalik lotion is contraindicated in children under 2 years of age.

When treating children from 2 to 12 years old, the drug should be used with caution. It should be borne in mind that children, compared with adults, are more at risk of developing systemic side effects of GCS, including suppression of the function of the hypothalamic-pituitary-adrenal system and increased intracranial pressure. This is due to the large ratio of body surface area to weight, and, therefore, increased absorption of the drug.

Increased intracranial pressure in children is manifested by headaches, protrusion of the fontanelle, bilateral edema of the optic nerve head. Suppression of the function of the hypothalamic-pituitary-adrenal system is determined by low plasma cortisol levels and lack of response to stimulation of adrenocorticotropic hormone.

Cases of negative interaction of Diprosalik lotion with its simultaneous use with other drugs have not been registered.
