Release form and composition
Dosage form - solution for intracavitary and external use: transparent liquid of greenish-yellow color [5 and 10 ml in colorless glass ampoules, 5 ampoules in cell contour packs or pre-made cardboard forms (grays) with cells for ampoules, in a cardboard box 1-2 packs / warming, complete with or without an ampoule knife / scarifier and instructions for use of Dixin].
Composition of 1 ml solution:
- active substance: hydroxymethylquinoxalindioxide - 10 mg;
- auxiliary component: water for injection.
Pharmacological properties
Pharmacodynamics
Dixin is a broad-spectrum antibacterial agent. It belongs to the group of quinoxaline derivatives. Effective for infections caused by the following microorganisms: Escherichia coli, Klebsiella pneumoniae, Salmonella spp., Shigella flexneri, Pseudomonas aeruginosa, Staphylococcus spp., Shigella dysenteriae, Shigella sonnei, Proteus vulgaris, Streptococcus pathogenic spp. … The drug has an effect on bacterial strains that are resistant to other antimicrobial agents, including antibiotics.
The mechanism of action of Dixin is due to its ability to damage the DNA biosynthesis of microbial cells.
When treating purulent-necrotic and burn wounds with the drug, Dixin promotes faster cleaning of the wound surface, stimulates reparative regeneration and marginal epithelization, and in general has a beneficial effect on the course of the wound process.
As with other antibacterial drugs, bacteria can develop drug resistance to hydroxymethylquinoxalindoxide.
Pharmacokinetics
Hydroxymethylquinoxaline dioxide is applied topically on wound and burn surfaces, therefore it is absorbed in small amounts. It is excreted from the body by the kidneys.
Indications for use
Dixin is intended for the treatment of bacterial infections caused by sensitive microorganisms, with the ineffectiveness of other chemotherapeutic agents or with their poor tolerance:
- external use: wound and burn infections of various localization, including superficial and deep wounds, purulent wounds with osteomyelitis, phlegmon of soft tissues;
- intracavitary administration: purulent processes in the abdominal and chest cavity, including peritonitis, pleural empyema, purulent pleurisy, wounds with deep purulent cavities (purulent mastitis, phlegmon of pelvic tissue, soft tissue abscess, postoperative wounds of the biliary and urinary tract), cystitis, prevention of infectious complications after bladder catheterization.
Contraindications
Absolute:
- adrenal insufficiency (currently or in history);
- age up to 18 years;
- period of pregnancy and lactation;
- hypersensitivity to any component of Dixin or other drugs from the quinoxaline group.
The drug should be used with caution in patients with renal insufficiency.
Dixin, instructions for use: method and dosage
Dixin solution is intended for external and intracavitary use.
External use
Topically apply 0.1-1% solutions of the drug, for this the original Dixin solution is diluted with a sterile solution of sodium chloride 0.9% or water for injection.
For superficial purulent wounds, after preliminary cleansing from purulent-necrotic masses, use wipes moistened with a 0.5-1% solution. They are applied to the wound surface and changed daily or every other day, depending on the condition of the wound and response to treatment. Deep wounds after treatment must be loosely tamped with tampons moistened with a 1% solution of the drug, 1 time per day.
The maximum daily dose of the drug is 2500 mg (250 ml of 1% solution). The duration of treatment is determined individually, up to 3 weeks.
For deep purulent wounds of the feet and hands caused by osteomyelitis, 0.5–1% solutions are prescribed. The drug is used in the form of trays. You can also inject the drug for 15–20 minutes into the wound, and then apply bandages moistened with a 1% solution.
In order to prevent the development of an infectious process after surgery, 0.1–0.5% solutions are used. According to indications (for osteomyelitis) and if Dixin is well tolerated, the drug can be used daily for 1.5–2 months.
Intracavitary introduction
Depending on the size of the purulent cavity, 10–50 ml (maximum 70 ml) of 1% solution is injected into it per day. The drug is administered with a syringe, through a catheter or drainage tube, usually once a day. Depending on the indication, the daily dose may be divided into two administrations. With good tolerance, the drug can be administered daily for 3 or more weeks. If necessary, it is possible to conduct repeated courses at intervals of 1–1.5 months.
Dose adjustment
The dose of Dixin is halved in patients with functional impairment of the kidneys.
In elderly patients, dose reduction is required in case of impaired renal function.
Side effects
- external use: itching, peri-wound dermatitis;
- intracavitary administration: headache, hyperthermia, chills, muscle spasms, dyspeptic disorders, photosensitivity (the appearance of pigmented spots on the body when exposed to sunlight), allergic reactions.
Overdose
With an overdose of Dixin, acute adrenal insufficiency may develop. In this case, the drug is urgently canceled and appropriate hormone replacement therapy is prescribed.

Dixin treatment is carried out in a hospital under the strict supervision of a physician.
The solution is unstable if stored at low temperatures, therefore, intravenous administration of the drug is prohibited.
Before using hydroxymethylquinoxalindioxide, its tolerance is checked by the patient: 10 ml of a 1% solution is injected into the cavity. If there are no adverse events (fever, chills, dizziness) within 3-6 hours, treatment is started. With the development of adverse reactions, the use of the solution is contraindicated.
The drug is used only for the treatment of severe forms of infectious diseases, as well as in cases of ineffectiveness of other antibacterial agents, including carbapenems, fluoroquinolones, cephalosporins of II – IV generations.
It is recommended to use Dixin together with antihistamines and calcium preparations in order to avoid the development of side effects. If they occur, the dose of the drug should be reduced and / or antihistamine therapy should be prescribed, hydroxymethylquinoxalindioxide may need to be canceled.
In case of the appearance of pigmented spots, it is necessary to increase the duration of administration of the solution to 1.5–2 hours, reduce the dose of the drug, prescribe an antihistamine or cancel Dixin.
Crystals may precipitate in solution during storage. In this case, they are dissolved with shaking until the crystals are completely dissolved, by heating the ampoules in a boiling water bath (the solution should become transparent). Then the ampoules are cooled to 36–38 ° C. If no re-precipitation of crystals occurs, the preparation can be used.
Influence on the ability to drive vehicles and complex mechanisms
During the use of Dixin, it is recommended to be careful when driving a car and performing potentially hazardous work requiring speed of psychomotor reactions and concentration.
Application during pregnancy and lactation
According to preclinical studies, hydroxymethylquinoxaline dioxide has teratogenic, embryotoxic and mutagenic properties. In this regard, Dixin is contraindicated for use during pregnancy. If antimicrobial therapy is needed during lactation, women should transfer the baby to bottle feeding.
Pediatric use
Dixin is not used to treat children and adolescents under 18 years of age.
With impaired renal function
The drug should be used with caution in patients with renal insufficiency. Dose reduction by 2 times is required.
For violations of liver function
There is no evidence of the need to adjust the dose for patients with hepatic impairment.
Use in the elderly
Under the condition of normal renal function in elderly patients, there is no need to adjust the dose of Dixin. In case of age-related decrease in renal function, a decrease in the dose of the drug is required.
