Release form and composition
Derinat is available in the following dosage forms:
- Solution for intramuscular injection: colorless, transparent, without foreign inclusions (2 or 5 ml in glass vials, 5 (5 ml) or 10 (2 ml) vials in a pallet, 1 pallet in a cardboard box);

Pharmacological properties
Pharmacodynamics
Derinat activates the processes of humoral and cellular immunity. Immunomodulatory action is provided by stimulation of B-lymphocytes and activation of T-helpers. The drug activates nonspecific resistance of the body, optimizes inflammatory reactions, as well as the immune response to viral, fungal and bacterial antigens. Promotes stimulation of regenerative and reparative processes. Increases the body's resistance to infections, regulates hematopoiesis (ensures the normalization of the number of lymphocytes, leukocytes, granulocytes, platelets, phagocytes).
Due to the pronounced lymphotropicity, taking Derinat stimulates the drainage and detoxification functions of the lymphatic system. The drug significantly reduces the sensitivity of cells to the effects of radiation therapy and chemotherapy drugs. It has no embryotoxic, teratogenic and carcinogenic effects.
Pharmacokinetics
It is rapidly absorbed, distributed in tissues and organs through the endolymphatic transport path. It has a high tropism for the organs of the hematopoietic system, is embedded in cellular structures, due to which it actively participates in cellular metabolism. In the phase of intensive entry into the blood, in parallel with the processes of metabolism and excretion, the drug is redistributed between the blood plasma and its formed elements. After a single injection, all pharmacokinetic curves of changes in the concentration of sodium deoxyribonucleate in the studied tissues and organs show rapid phases of increase and decrease in concentration in the time interval from 5 to 24 hours. When administered intramuscularly, the half-life is 72.3 hours.
It is quickly distributed in the body, during the daily course of treatment it accumulates in tissues and organs (mainly in the lymph nodes, bone marrow, thymus, spleen). To a lesser extent, the drug is accumulated in the brain, liver, stomach, large and small intestines. The time to reach the maximum concentration in the bone marrow is 5 hours, and in the brain - 30 minutes. Penetrates the blood-brain barrier.
It is metabolized in the body. It is excreted by biexponential dependence in the form of metabolites in the urine, to a lesser extent in the feces.
Indications for use
According to the instructions, Derinat in the form of a solution for intramuscular administration is used in the treatment of diseases / conditions such as:
- Violation of hematopoiesis;
- Radiation damage;
- Resistance and myelodepression to cytostatics in cancer patients associated with radiation and / or cytostatic therapy (stabilization of hematopoiesis, lowering of myelo- and cardiotoxicity of chemotherapy drugs);
- Stomatitis induced by cytostatic drugs;
- Erosive gastroduodenitis, gastric ulcer and duodenal ulcer;
- Obliterating vascular diseases of the lower extremities (stage II-III);
- Ischemic heart disease (CHD);
- Purulent septic complications, odontogenic sepsis;
- Long-term non-healing wounds, trophic ulcers;
- Pulmonary tuberculosis, inflammatory diseases of the respiratory tract;
- Burn disease;
- Rheumatoid arthritis;
- Endometritis, endometriosis, salpingo-oophoritis, fibroids;
- Ureaplasmosis, chlamydia, mycoplasmosis;
- Chronic obstructive pulmonary disease;

As monotherapy Derinat in the form of a solution for local and external use is used for the following diseases:
- Dystrophic and inflammatory eye diseases;

Contraindications
Derinat is contraindicated in the presence of hypersensitivity to the drug components.
Instructions for the use of Derinat: method and dosage
For adults, Derinat is administered in the form of a solution for intramuscular administration for 1-2 minutes at an average single dose of 75 mg (5 ml of a solution for intramuscular administration of 15 mg / ml). The interval of administration is 24-72 hours.
Depending on the indications, the following treatment regimens are used:
- Ischemic heart disease - 5 ml of a 15 mg / ml solution, the interval between injections is 48-72 hours. Treatment course - 10 injections;
- Oncological diseases - 5 ml (75 mg per day), the interval between injections is 48-72 hours. Treatment course - 10 injections;
- Peptic ulcer and duodenal ulcer - 5 ml of a solution of 15 mg / ml, the interval between injections is 48 hours. Treatment course - 5 injections;
- Tuberculosis - 5 ml of a solution of 15 mg / ml, the interval between injections is 24-48 hours. Treatment course - 10-15 injections;
- Benign prostatic hyperplasia, prostatitis - 5 ml of a 15 mg / ml solution, the interval between injections is 24-48 hours. Treatment course - 10 injections;
- Chlamydia, endometriosis, endometritis, mycoplasmosis, ureaplasmosis, fibroids, salpingo-oophoritis - 5 ml of a solution of 15 mg / ml, the interval between injections is 24-48 hours. Treatment course - 10 injections;
- Chronic inflammatory diseases - 5 ml of a 15 mg / ml solution: the first 5 injections with a break of 24 hours each, the next - with an interval of 72 hours. Treatment course - 10 injections;
- Acute inflammatory diseases - 5 ml of a 15 mg / ml solution, the interval between injections is 24-72 hours. The treatment course is 3-5 injections.
When using a solution of 15 mg / ml, 2 ml injections should be carried out every day, recalculating, until a dose of 375-750 mg per course is reached.
The frequency of intramuscular administration of the drug in children is the same as in adults. The drug is used in the following doses:
- Up to 2 years: the average single dose is 7.5 mg (0.5 ml of a solution for intramuscular administration of 15 mg / ml);
- 2-10 years: a single dose is determined at the rate of 0.5 ml of the drug per year of life;
- Over 10 years: average single dose - 75 mg (5 ml of solution for intramuscular injection of 15 mg / ml), course dose - up to 5 injections of the drug.
Derinat in the form of a solution for external and local use is used depending on the localization of the ongoing process.
The drug can be used by adults and children from the first day of life.
For the prevention of ARVI, Derinat is dripped into the nose: in each nasal passage, 2 drops of the solution 2-4 times a day. The duration of therapy is 7-14 days. With the development of symptoms of a respiratory disease during the first day, Derinat is instilled into the nose with 2-3 drops in each nasal passage every 1-1.5 hours, then - 3-4 times a day, 2-3 drops. The duration of the treatment course can vary from 5 to 30 days.
Depending on the disease, Derinat is used according to the following schemes:
- Inflammatory diseases of the paranasal sinuses and nasal cavity - 4-6 times a day, 3-5 drops are instilled into each nasal passage. The duration of the treatment course is 7-15 days;
- Inflammatory diseases of the oral cavity - 4-6 times a day, you should rinse the mouth (1 bottle for 2-3 rinses). The duration of the treatment course is 5-10 days;
- Chronic inflammatory diseases, fungal, bacterial and other infections in gynecological practice - irrigation of the vagina and cervix or intravaginal administration of tampons with a solution is shown. For the procedure - 5 ml, frequency of application - 1-2 times a day. The duration of the treatment course is 10-14 days;
- Severe inflammatory and dystrophic processes in ophthalmic practice - Derinat 2-3 times a day should be instilled into the eyes, 1-2 drops. The duration of the treatment course is 14-45 days;
- Hemorrhoids - rectal administration of the drug is shown using microclysters of 15-40 ml. The duration of the treatment course is 4-10 days;
- Post-radiation necrosis of mucous membranes and skin, long-term non-healing wounds, burns, frostbite, gangrene, trophic ulcers of various etiology - 3-4 times a day, application dressings (gauze in 2 layers) with the applied solution should be applied to the affected areas. Also, the affected surface can be treated 4-5 times a day with a drug from a nebulizer, 10-40 ml. The duration of the treatment course is 1-3 months;
- Obliterating diseases of the lower extremities - to achieve a systemic effect, Derinat is instilled 6 times a day into each nasal passage, 1-2 drops. The duration of the treatment course is up to 6 months.
Side effects
With intravenous administration of Derinat in patients with diabetes mellitus, a hypoglycemic effect may be noted, which must be taken into account when controlling the level of glucose in the blood.
With local and external use of the drug, side effects have not been identified.

No negative effects from overdose have been identified.
