Gyno-Pevaril is a topical antifungal drug.

Release form and composition

The drug is produced in the form of vaginal suppositories with a dosage of 50 and 150 mg: torpedo-shaped, white or almost white (suppositories with a dosage of 50 mg - 5 pieces in blisters, in a cardboard box 3 blisters; suppositories with a dosage of 150 mg - 3 pieces in blisters, 1 blister in a carton. Each pack also contains instructions for the use of Gino-Pevaril).

Composition for 1 suppository:

  • active substance: econazole nitrate - 50 or 150 mg;
  • auxiliary components: solid fat (type A and type B).

Pharmacological properties

Pharmacodynamics

The spectrum of action of econazole is quite wide. It is active against molds, yeasts and dermatophytes. In addition, clinically significant activity of the drug against gram-positive bacteria was established. It damages cell membranes, which ultimately leads to increased cell permeability of fungi and bacteria. The target for econazole nitrate is the acyl groups of unsaturated fatty acids that are part of the membrane phospholipids.

Pharmacokinetics

The absorption of econazole is negligible. After the introduction of the suppository into the vagina, only about 5% of the active substance is absorbed. The maximum plasma concentrations of econazole, as well as its metabolites, are observed after 1-2 days of using Gino-Pevaril and are 15 ng / ml (for a drug with a dosage of 50 mg) and 65 ng / ml (for a drug with a dosage of 150 mg).

About 98% of econazole and / or its metabolites binds strongly to serum proteins. The drug's metabolism is active: econazole is metabolized by deamination, O-dealkylation and oxidation. Metabolites are excreted through the intestines (with feces) and kidneys (with urine).

Indications for use

Suppositories Gino-Pevaril are used in women and girls over 16 years of age for the treatment of vulvovaginal mycoses (in particular, candidiasis).

Contraindications

The drug is contraindicated in children and adolescents under 16 years of age, women in the first trimester of pregnancy, as well as persons with hypersensitivity to individual components in its composition.

Elderly patients (over the age of 65), women in the second and third trimesters of pregnancy, as well as breastfeeding mothers, Gino-Pevaril is prescribed with caution.

Gino-Pevaril, instructions for use: method and dosage

Gyno-Pevaril suppositories are intended for intravaginal use. Suppositories are inserted deep into the vagina. The patient should be in the supine position. The drug is administered once a day, just before bedtime.

Suppositories with a dosage of 50 mg are used for at least 14 days. A full therapeutic course is required, even if the subjective symptoms of the disease (burning and itching) disappear before the end of treatment.

Suppositories with a dosage of 150 mg are used for 3 days.

In the event of a relapse or a positive result for determining the pathogen, a second course is carried out 7 days after the end of therapy.

Side effects

  • skin and subcutaneous fat: very rarely (less than 1 in 10,000, including isolated cases) - contact dermatitis, erythema, urticaria, skin peeling, angioedema;
  • immune system: very rarely - hypersensitivity reactions;
  • others: very rarely - swelling, irritation and pain at the injection site.

Overdose

Overdose cases of Gino-Pevaril have not been registered. If suppositories are accidentally swallowed, vomiting, nausea and diarrhea may occur (symptomatic treatment is performed).

special instructions

Gyno-Pevaril is intended for local (intravaginal) administration only. Suppositories should not be taken orally.

If the drug accidentally gets into the eyes, they should be urgently rinsed with saline or clean water. If necessary, the patient should seek qualified medical attention.

Vaginal suppositories are not recommended for use in conjunction with latex condoms or diaphragms (due to the possible decrease in the effectiveness of latex contraceptives).

Intravaginal drugs can inactivate spermicidal contraceptives, therefore, before using these contraceptives in conjunction with Gyno-Pevaril, you should consult your doctor.

Suppositories should not be used concurrently with other oral or topical medications for the treatment of genital diseases.

There are no data on the penetration of econazole nitrate into breast milk, therefore, during lactation, Gino-Pevaril is used with caution.

Pediatric use

Gino-Pevaril is contraindicated in children and adolescents under the age of 16.

Use in the elderly

In elderly patients (over 65 years old), vaginal suppositories are used with caution, since there is insufficient data on the use of the drug in this age group.

Drug interactions

The absorption of econazole into the systemic circulation is insignificant, therefore the likelihood of significant drug interactions is very small. Despite this, there have been rare cases of interaction with oral anticoagulants, therefore, patients who take indirect anticoagulants (for example, acenocoumarol and warfarin) should monitor their blood clotting parameters.
