Vikanol Life is a drug with antiulcer action.

Release form and composition

Dosage form - film-coated tablets: biconvex, rounded, with a slight smell of ammonia or odorless, the shell is creamy white, the core is almost white or white (30, 60, 120 or 240 tablets each in polymer polyethylene cans, in a cardboard box 1 jar, 8, 10, 15 or 20 tablets in blisters, in a carton box 3, 4, 6-8, 10, 12, 14 or 24. Each pack also contains instructions for the use of Vicanol Life).

Composition of 1 tablet:

  • active substance: Vicanol, granular substance - 324.47 mg [active substance: bismuth tripotassium dicitrate - 304.6 mg (corresponds to bismuth oxide Bi2CO3 - 120 mg); excipients: macrogol 6000 - 3.86 mg; povidone K30 - 16.01 mg];
  • auxiliary components: magnesium stearate - 2 mg; corn starch - 70.6 mg; potassium polyacrylate - 23.6 mg; macrogol 6000 - 2.13 mg; povidone K30 - 1.7 mg;
  • film shell: hypromellose (viscosity 5 mPas) - 3.2 mg; macrogol 6000 - 1.1 mg.

Pharmacological properties

Pharmacodynamics

Bismuth tripotassium dicitrate - the active substance of Vicanol Life, is an antiulcer agent with bactericidal activity against Helicobacter pylori. It also has anti-inflammatory and astringent effects. Insoluble bismuth oxychloride and citrate are deposited in the acidic environment of the stomach, while chelating compounds with a protein substrate in the form of a protective film are formed on the surface of ulcers and erosions.

By increasing the biosynthesis of prostaglandin E, the formation of mucus and the secretion of bicarbonate, the drug stimulates the activity of cytoprotective mechanisms, increases the resistance of the mucous membrane of the gastrointestinal tract to the effects of hydrochloric acid, pepsin, enzymes and bile salts. Other effects of Vicanol Life: accumulation of epidermal growth factor in the defect area, decreased activity of pepsinogen and pepsin.

Pharmacokinetics

Bismuth subcitrate is practically not absorbed from the gastrointestinal tract. Excretion is carried out mainly with feces. Bismuth, which entered the plasma in small amounts, is excreted from the body by the kidneys.

Indications for use

  • chronic gastroduodenitis and gastritis, gastric ulcer and duodenal ulcer (including those associated with Helicobacter pylori) in the acute phase;
  • functional dyspepsia, which is not associated with organic diseases of the gastrointestinal tract;
  • irritable bowel syndrome, which is accompanied mainly by symptoms of diarrhea.

Contraindications

  • severe renal impairment, including severe renal failure (in patients with creatinine clearance less than 30 ml / min) and chronic renal failure;
  • combined therapy with preparations containing bismuth;
  • pregnancy and lactation;
  • age up to 4 years;
  • individual intolerance to the components of the drug.

Vikanol Life, instructions for use: method and dosage

Vicanol Life tablets are taken orally, 30 minutes before meals. They should be swallowed whole, without crushing or chewing, drinking plenty of water. It is not recommended to drink pills with milk.

Dosing regimen:

  • adults and children from 12 years old: 4 times a day, 1 tablet or 2 times a day, 2 tablets;
  • children 8-12 years old: 2 times a day, 1 tablet;
  • children 4–8 years old: 8 mg / kg per day in 1 or 2 doses (the daily dose should be closest to the calculated one).

The duration of the course of treatment ranges from 4 to 8 weeks. Over the next 8 weeks, bismuth-containing preparations (Vikalin, Vikair) should not be used.

For the eradication of Helicobacter pylori, it is advisable to carry out combination therapy with other antibacterial agents that have anti-Helicobacter activity.

Side effects

With the use of Vicanol Life, the following adverse events may develop [> 10% - very often; (> 1% and 0.1% and 0.01% and <0.1%) - rarely; <0.01% - very rare]:

  • digestive system: very often - stool staining black; infrequently - vomiting, nausea, constipation, or diarrhea;
  • nervous system: very rarely (with prolonged use in high doses) - encephalopathy associated with the accumulation of bismuth in the central nervous system;
  • allergic reactions: infrequently - itching, skin rash; very rarely - anaphylactic reactions.

Adverse reactions are reversible and quickly disappear after discontinuation of the drug.

Overdose

When using Vikanol Life in doses significantly higher than recommended, or with prolonged treatment in doses exceeding therapeutic ones, bismuth poisoning may develop.

The main symptoms of a bismuth overdose: dyspepsia, rash, inflammation of the oral mucosa, darkening of the gums in the form of blue lines. Long-term use of high doses can lead to impaired renal function.

Therapy: gastric lavage is prescribed, the use of enterosorbents (for example, activated carbon), symptomatic treatment is carried out aimed at maintaining kidney function. The use of saline laxatives is shown. In case of impaired renal function, accompanied by a high plasma level of bismuth in the blood, it is possible to introduce complexing agents - dimercaptosuccinic and dimercaptopropanesulfonic acids. With the development of severe renal dysfunction, hemodialysis is prescribed. There is no specific antidote.

When the drug is discontinued, the symptoms of overdose are completely reversible.

special instructions

Do not use Vicanol Life for more than 8 weeks. During the period of therapy, other medicines containing bismuth should not be taken. The plasma concentration of the active substance in the blood at the end of the course of therapy using the recommended doses does not exceed 3–58 μg / l (the development of intoxication occurs at a concentration of more than 100 μg / l).

Drinking alcohol during therapy is not recommended.

While taking Vicanol Life tablets, feces may turn dark, which is associated with the formation of bismuth sulfide. Some patients have a slight darkening of the tongue.

Application during pregnancy and lactation

During pregnancy / lactation, Vicanol Life is not prescribed.

Pediatric use

For patients under 4 years of age, the drug is contraindicated.

With impaired renal function

Contraindications to taking Vicanol Life are severe renal dysfunction, including severe renal failure (in patients with creatinine clearance less than 30 ml / min) and chronic renal failure.

Drug interactions

To avoid affecting the effectiveness of Vicanola Life, with the use of other drugs, as well as with the intake of liquids and food (antacids, milk, fruit juices and fruits), a break of at least 30 minutes must be observed.

Vicanol Life reduces the absorption of tetracyclines.

Combined use with bismuth-containing drugs is not recommended, as this can lead to the development of adverse reactions, including encephalopathy.
