Bystrumcaps is an oral non-steroidal anti-inflammatory drug (NSAID).

Release form and composition

Dosage form - capsules of prolonged action: size No. 1, hard gelatinous, with a transparent body and a white or creamy-white cap; the contents of the capsule are almost white or white pellets (in a blister 10 pcs., in a cardboard box 2 blisters and instructions for the use of Bystrumkaps).

Composition of 1 capsule:

  • active substance: ketoprofen - 200 mg;
  • auxiliary components: neutral granules (sugar and corn starch in a ratio of 3: 1), copolymer of ammonium methacrylate, stearic acid, ethyl cellulose, macrogol 4000, talc.

Pharmacological properties

Pharmacodynamics

Ketoprofen is an NSAID from the group of propionic acid derivatives. It has anti-inflammatory, analgesic, antipyretic effects. Also, the drug inhibits platelet aggregation.

The mechanism of action of ketoprofen is due to its ability to nonselectively inhibit the activity of COX-1 and COX-2, which leads to a reduction in prostaglandin synthesis.

Pharmacokinetics

Thanks to the dosage form of Bystrumcaps, ketoprofen in the gastrointestinal tract is released from the capsule gradually, and is absorbed almost completely. Has the effect of the first passage through the liver. After oral administration of a dose of 200 mg, the maximum plasma concentration (C max) is observed after 6–8 hours. The drug does not accumulate in the body. With simultaneous ingestion of food, bioavailability does not change.

Ketoprofen is characterized by a high bond with plasma proteins - 99%. Penetrates into synovial fluid, where it creates a concentration higher than in plasma. In small quantities, it penetrates the placental barrier.

The drug is metabolized in two ways: the main one is a compound with glucuronic acid, an auxiliary one is hydroxylation. No more than 1% of the accepted dose of ketoprofen is determined unchanged in the urine, the rest of the drug is metabolites, among them metabolites of glucuronic acid prevail - 65-75%.

Ketoprofen is excreted from the body by the kidneys. The half-life (T ½) is approximately 8 hours. Within 5 days after taking Bystrumkaps, about 70-90% of the dose is excreted in the urine, from 1 to 8% is found in feces.

In the elderly, the elimination period of the drug decreases, and T ½ increases. T ½ also increases in patients with impaired renal function.

Indications for use

Bystrumcaps is used to relieve pain syndrome of various origins and symptomatic treatment of diseases of the musculoskeletal system:

  • postoperative pain;
  • pain syndrome with uncomplicated injuries, in particular with damage to ligaments and tendons, bruises, etc.;
  • spine and lower back pain: myalgia, neuralgia, lumbago, sciatica;
  • dysmenorrhea pain;
  • articular syndrome, in particular osteoarthritis, rheumatoid arthritis, ankylosing spondylitis;
  • inflammatory and degenerative diseases of the musculoskeletal system, including bursitis, periarthritis, capsulitis, synovitis, tendonitis.

Quickcaps reduces pain and inflammation for the period of use, but does not affect the progression of the disease.

Contraindications

Absolute:

  • III trimester of pregnancy;
  • lactation period;
  • age up to 15 years;
  • condition after coronary artery bypass grafting;
  • a history of bronchial asthma, urticaria or rhinitis caused by ketoprofen, acetylsalicylic acid or other NSAIDs;
  • severe liver failure or active liver disease;
  • severe renal failure [CC (creatinine clearance) <30 ml / min] or progressive kidney disease;
  • confirmed hyperkalemia;
  • active cerebrovascular, gastrointestinal, or other bleeding (or suspected bleeding);
  • hemophilia and other blood clotting disorders;
  • inflammatory bowel disease;
  • diverticulitis;
  • Crohn's disease;
  • peptic ulcer;
  • ulcerative colitis in the acute phase;
  • peptic ulcer of the stomach and duodenum in the acute phase;
  • hypersensitivity to ketoprofen, other NSAIDs, or to any auxiliary component of the drug.

Bystrumkaps capsules are used with caution in the following cases (due to the risk of complications):

  • long-term use of NSAIDs;
  • I and II trimesters of pregnancy;
  • elderly age;
  • peripheral artery disease, ischemic heart disease, arterial hypertension, chronic heart failure, edema;
  • a history of Helicobacter pylori infection or gastrointestinal ulcers;
  • alcoholism;
  • alcoholic cirrhosis of the liver, liver failure;
  • renal failure with CC 30-60 ml / min;
  • blood diseases (including leukopenia, anemia);
  • severe somatic diseases;
  • cerebrovascular diseases;
  • diabetes;
  • dyslipidemia or hyperlipidemia;
  • hyperbilirubinemia;
  • stomatitis;
  • bronchial asthma;
  • dehydration;
  • sepsis;
  • smoking;
  • the simultaneous use of the following drugs: selective serotonin reuptake inhibitors (including paroxetine, citalopram, sertraline, fluoxetine), anticoagulants (including warfarin), antiplatelet agents (including clopidogrel), oral glucocorticosteroids (including prednisolone).

Bystrumcaps, instructions for use: method and dosage

Bystrumcaps capsules should be taken orally with meals.

The recommended dose for adolescents from 15 years old and adults is 1 capsule 1 time per day.

Side effects

  • disorders of the digestive system: stomatitis, flatulence, diarrhea, constipation, nausea, loss of appetite, heartburn, pain in the abdomen and in the epigastric region, vomiting, changes in the activity of hepatic transaminases, NSAID gastropathy, change in taste, hepatitis; with prolonged use of Bystrumkaps in high doses - gingival bleeding, ulceration of the mucous membrane of the gastrointestinal tract, intestinal perforation, hemorrhoidal bleeding;
  • phenomena from the cardiovascular system: increased blood pressure, heart failure; rarely - tachycardia;
  • reactions from the nervous system: sleep disturbance, tinnitus, headache, nervousness, insomnia, agitation, asthenia, drowsiness, dizziness, depression; rarely - migraine, forgetfulness, confusion or loss of consciousness, peripheral neuropathy;
  • effects from the urinary system: rarely - edema (especially in persons with arterial hypertension), urethritis, cystitis, interstitial nephritis, hematuria, nephrotic syndrome, impaired renal function, acute renal failure;
  • disorders of the hematopoietic organs: agranulocytosis, thrombocytopenia, leukopenia, pancytopenia, anemia; rarely - hemolytic anemia;
  • phenomena from the senses: double vision, blurred vision, reversible toxic amblyopia; rarely - hearing loss, eye pain, dryness of the mucous membrane of the eye, conjunctivitis, conjunctival hyperemia;
  • dermatological and allergic reactions: alopecia, photosensitization, erythema multiforme, exudative erythema multiforme, including Stevens-Johnson syndrome, toxic epidermal necrolysis, rash, urticaria, rhinitis, pruritus, anaphylactic shock, angioedema (especially with increased asthmatic sensitivity to acetylsalicylic acid or other NSAIDs);
  • others: increased sweating; rarely - thirst, nosebleeds, hemoptysis, shortness of breath, muscle twitching, myalgia; with long-term use of Bystrumkaps in large doses - vaginal bleeding.

Overdose

An overdose of ketoprofen can manifest itself with symptoms of acute poisoning such as epigastric pain, nausea, diarrhea, bleeding from the gastrointestinal tract, vomiting, drowsiness, dizziness, decreased blood pressure, headache, bronchospasm.

After taking an excessive dose of Bystrumkaps, if no more than 1 hour has passed, gastric lavage with activated charcoal should be done (dose for adults - 60-100 g, for children - 1-2 g / kg). Overdose treatment is symptomatic.

special instructions

In patients receiving ketoprofen, the peripheral blood picture, the functional state of the kidneys and liver should be monitored. More careful monitoring during therapy is required in patients with impaired renal and / or liver function.

NSAIDs can cause adverse reactions from the digestive system. To minimize the risk, it is recommended to use ketoprofen at the lowest dose for the shortest possible course.

Bystrumcaps can mask the signs of an infectious disease.

Patients who are prescribed a test for the determination of 17-ketosteroids should stop taking Bystrumcaps 48 hours before the study.

Influence on the ability to drive vehicles and complex mechanisms

Bystrumcaps is capable of causing disturbances on the part of the central nervous system, therefore, vehicle drivers and persons employed in potentially hazardous industries are advised to be careful during the treatment period.

Application during pregnancy and lactation

Taking Bystrumkaps is contraindicated in the third trimester of pregnancy. The drug should be used with caution in the I and II trimesters.

Breastfeeding women should not take ketoprofen. If a course of treatment during lactation is necessary, breastfeeding should be discontinued.

Pediatric use

Patients under the age of 15 years are not prescribed Bystrumcaps.

With impaired renal function

In case of functional impairment of the kidneys (CC 30-60 ml / min) in patients receiving Bystrumcaps, the state of renal function should be monitored. A dose reduction is required.

In patients with progressive kidney disease and severe renal failure (CC <30 ml / min), Fastcaps is contraindicated.

For violations of liver function

With alcoholic cirrhosis of the liver, alcoholism and moderate hepatic insufficiency, Bystrumcaps should be used with caution, under close monitoring of liver function.

Ketoprofen is contraindicated in patients with active liver disease and severe hepatic impairment.

Use in the elderly

Use Bystrumcaps in the elderly with caution.

Drug interactions

  • other NSAIDs (including salicylates in high doses), heparin, ticlopidine, oral anticoagulants: the risk of bleeding increases (if necessary, taking such a combination requires careful monitoring of the patient's condition);
  • verapamil, nifedipine: their plasma concentrations increase;
  • beta-blockers: their hypotensive effect decreases, the risk of developing a nephrotoxic effect increases;
  • myelotoxic drugs: manifestations of hematotoxicity of ketoprofen increase;
  • antiplatelet agents, anticoagulants, fibrinolytics, ethanol: their effects are enhanced;
  • intrauterine devices: their contraceptive effect decreases;
  • thrombolytics, pentoxifylline, cefotetan, cefamandol, cefoperazone: the risk of bleeding increases;
  • zidovudine: the likelihood of developing anemia increases;
  • estrogens, glucocorticosteroids, mineralocorticoids: their side effects increase;
  • cyclosporine, tacrolimus: may increase the nephrotoxic effect, especially in elderly patients;
  • uricosuric drugs: their effectiveness decreases;
  • glucocorticosteroids, corticotropin, ethanol: the formation of ulcers and the appearance of gastrointestinal bleeding, the occurrence of functional renal disorders;
  • oral hypoglycemic agents, insulin: their effect is enhanced (recalculation of doses is required);
  • diuretics, angiotensin-converting enzyme inhibitors: it is possible to reduce the hypotensive and diuretic effect, in patients with a deficit in circulating blood there is a risk of developing renal failure;
  • sodium valproate: platelet aggregation is disturbed;
  • cholestyramine, antacids: the absorption of ketoprofen in the gastrointestinal tract decreases;
  • methotrexate in a weekly dose of more than 15 mg: hematotoxicity may develop (the intervals between doses of drugs should be at least 12 hours);
  • lithium preparations: the toxic level of lithium in the plasma increases (with the simultaneous use of the lithium preparation together with ketoprofen and for some time after the end of the combination therapy, it is necessary to intensively monitor the lithium content in the blood plasma);
  • barbiturates, tricyclic antidepressants, phenylbutazone, ethanol, phenytoin, rifampicin: the production of hydroxylated active metabolites by inducers of microsomal oxidation in the liver increases.
