Release form and composition

Dosage form Buserelin-long is a lyophilisate for preparing a suspension for intramuscular (i / m) administration of prolonged action (3.75 mg in vials, 1 vial complete with a solvent in a cardboard box).

Composition of the preparation:

• active substance: buserelin - 3.75 mg;
• auxiliary components: dl-lactic and glycolic acid copolymer - 200 mg, D-mannitol - 85 mg, sodium carmellose - 30 mg, polysorbate-80 - 2 mg.

Solvent: 0.8% mannitol solution in 1 ml of water for injection.

Pharmacological properties

Pharmacodynamics

Buserelin-long is an antineoplastic agent, analogue of gonadotropin-releasing hormone. The mechanism of its action is due to the ability to cause a short-term increase in the level of sex hormones in the blood plasma due to competitive binding with cellular receptors located in the anterior lobe of the pituitary gland.

When using the drug in therapeutic doses for 12-14 days, the gonadotropic pituitary function is blocked.

Buserelin lowers the production of testosterone, follicle-stimulating hormone and luteinizing hormone.

In women, the concentration of estradiol reaches values characteristic of the postmenopausal period. The testosterone content in men decreases to the values observed in the post-castration period.

Pharmacokinetics

Bioavailability after intramuscular injection is high.

Indications for use

• mammary cancer;

Contraindications

According to the instructions, Buserelin-Long is contraindicated in women during pregnancy and lactation, as well as in all patients in case of hypersensitivity to any component of the drug.

Instructions for use of Buserelin-Long: method and dosage

Buserelin-long is administered intramuscularly.

A suspension for injection is prepared from the lyophilisate just before administration, using the solvent supplied with the kit.

Recommended dosage according to indications:

• hormone-dependent prostate cancer: 1 dose (3.75 mg) 1 time per 4 weeks;
• endometriosis and endometrial hyperplastic processes: 1 dose every 4 weeks, treatment begins in any of the first 5 days of the menstrual cycle, the course of therapy is from 4 to 6 months;
• uterine fibroids: 1 dose 1 time in 4 weeks, treatment begins in any of the first 5 days of the menstrual cycle, before surgery, the drug is used for 3 months, in other cases - 6 months;
• infertility (IVF treatment): 1 dose is administered at the beginning of the follicular or in the middle of the luteal phase (2 or 21-24 days, respectively) of the menstrual cycle preceding the stimulation of ovulation. After reaching a blockade of the pituitary function, which is confirmed by a study of the concentration of estrogen in the blood serum (this indicator should be at least 50% less than the initial level), usually 12-15 days after the administration of Buserelin-Long, provided that there are no ovarian cysts (established using ultrasound) and the thickness of the endometrium is not more than 5 mm, they begin to stimulate superovulation using gonadotropic hormones (under the control of ultrasound and the level of estradiol in the blood serum).

Side effects

Possible reactions in patients of both sexes:

• from the central nervous system: sleep disorders, headache, frequent mood swings, depression;
• on the part of the musculoskeletal system: in the case of long-term treatment - bone demineralization, which can lead to the development of osteoporosis;
• allergic reactions: skin flushing, urticaria; rarely - Quincke's edema;
• others: in some cases - dyspeptic disorders, pulmonary embolism (the relationship of their development with the use of buserelin has not been established).

In women, in addition, the following undesirable phenomena are possible: sweating, pain in the lower abdomen, dryness of the vaginal mucosa, changes in libido; rarely (during the first weeks of treatment) - menstrual bleeding.

In men, in the treatment of prostate cancer in the first 2-3 weeks after the first injection of the drug, in addition, the following side effects are possible: exacerbation and progression of the underlying disease (due to stimulation of the synthesis of gonadotropins and, accordingly, testosterone), increased sweating, hot flashes, gynecomastia, decreased potency, muscle weakness in the lower extremities, renal edema (swelling of the legs, eyelids and face), urinary retention, a transient increase in the concentration of androgens in the blood, a temporary increase in bone pain (eliminated by symptomatic therapy); in some cases - obstruction of the ureters and compression of the spinal cord.

Overdose

Overdose cases of Buserelin-Long have not been reported to date.

Before starting treatment, women should exclude pregnancy, cancel hormonal contraceptives if they are used, and replace them with another reliable (non-hormonal) method of contraception.

Ovulation induction should be performed in a hospital setting under the strict supervision of a physician.

At the beginning of the use of the drug, there is a possibility of ovarian cyst formation.

After the cancellation of Buserelin-Long, ovarian function is restored. Menses usually return after 3 months.

For the prevention of side effects, men are recommended to prescribe antiandrogens: within 2 weeks before and 2 weeks after the first administration of buserelin.
