Release form and composition
The drug is produced in the form of a solution for injection: a clear, colorless liquid (5 ml each in an ampoule of neutral glass, 5 or 10 ampoules in a blister, 10 ampoules in a cardboard box, 1 or 2 blister packs equipped with a scarifier / ampoule knife for opening ampoules, and instructions for the use of Angiocardil; if the ampoules have a fracture ring or a break point, the scarifier / ampoule knife is not put into the box).
1 ml of solution for injection contains:
- active substance: meldonium dihydrate (in terms of anhydrous substance) - 100 mg;
- additional components: water for injection.
Pharmacological properties
Pharmacodynamics
Meldonium is a structural analogue of gamma-butyrobetaine, a substance present in every cell of the human body. The active component of Angiocardil leads to suppression of gamma-butyrobetaine hydroxylase, a decrease in carnitine production and transport of long-chain fatty acids across cell membranes, prevents the accumulation of acyl-coenzyme A and acylcarnitine derivatives in cells - activated forms of unoxidized fatty acids. Due to a decrease in carnitine levels, gamma-butyrobetaine is intensively produced, which exhibits vasodilating properties.
Against the background of acute ischemic damage to the heart muscle, meldonium inhibits the formation of a necrotic zone, shortens the rehabilitation period. With angina pectoris, it helps to reduce the frequency of attacks, and in chronic heart failure (CHF) - increase exercise tolerance. In the case of both acute and chronic ischemic cerebral circulation disorders, it improves blood circulation in the ischemic zone and redistributes blood flow in favor of the affected area.
The drug also demonstrates effectiveness in vascular and dystrophic pathology of the fundus. Thanks to the effect of the active substance, physical performance increases, the severity of symptoms of physical and mental overstrain decreases, and cellular and humoral immunity is activated. Angiocardil helps to eliminate vegetative manifestations against the background of chronic alcoholism and abstinence.
Pharmacokinetics
When administered intravenously, the bioavailability of meldonium is 100%. Almost immediately after administration, its maximum concentration (C max) in blood plasma is noted.
As a result of the metabolic transformation of the active substance in the body, two main metabolites are formed, which are excreted by the kidneys. The half-life (T 1/2) varies from 3 to 6 hours.
Indications for use
- CHF, cardialgia against the background of dyshormonal cardiomyopathy, ischemic heart disease (CHD), including angina pectoris and myocardial infarction (as part of a combination treatment);
- disorders of cerebral circulation, including cerebrovascular insufficiency and ischemic stroke (as part of a combination treatment);
- alcohol withdrawal syndrome (in combination with specific therapy for alcoholism);
- thrombosis of the central retinal vein and its branches, hemorrhages in the retina and hemophthalmos of various etiologies, retinopathy of various origins, including hypertensive, diabetic (as part of a combination treatment).
Also, Angiocardil is recommended for use with reduced performance, mental and physical overload (including in athletes).
Contraindications
Absolute:
- increased intracranial pressure, including against the background of impaired venous outflow and intracranial tumors;
- age up to 18 years;
- pregnancy and lactation;
- hypersensitivity to meldonia.
Angiocardil should be used with extreme caution in patients with liver and / or kidney disease.
Angiocardil, instructions for use: method and dosage
Angiocardil is administered intravenously (intravenously), intramuscularly (intramuscularly) or parabulbar. Since the drug can have a stimulating effect, it is recommended to use it in the morning.
The route of administration of the solution, dose and duration of treatment are set individually, taking into account the indications, severity of the condition and other factors.
Recommended dosage regimen of Angiocardil (as part of a combination treatment):
- myocardial infarction: 500–1000 mg (5–10 ml of solution) intravenously in a stream, 1 time per day or divided into 2 injections, followed by a switch to oral forms of meldonium;
- stable angina pectoris, CHF accompanying dyshormonal cardiomyopathy cardialgia: 500–1000 mg (5–10 ml) intravenously, once a day, or divided into 2 injections, or 500 mg intramuscularly 1–2 times a day; course - 10-14 days, followed by a transition to oral forms of meldonium; general course - 28–42 days;
- violation of cerebral circulation (acute phase and chronic insufficiency of cerebral circulation): 500 mg (5 ml) intravenously or additionally in case of chronic insufficiency - intramuscularly, once a day for 10 days, followed by a switch to oral administration, general course - 28–42 days; on the recommendation of the attending physician in the chronic form, it is possible to carry out repeated courses 2-3 times a year;
- alcohol withdrawal syndrome: 500 mg (5 ml) intravenously or intramuscularly 2 times a day; course - 7-10 days;
- hemorrhages in the retina and hemophthalmus of various etiology, retinopathy, thrombosis of the central retinal vein and its branches: 50 mg (0.5 ml) parabulbar, course - 10 days;
- excessive physical and mental stress: 500 mg intravenously or intramuscularly once a day, course - 10-14 days, if necessary, after 14-21 days, a second course is possible.
Side effects
- central nervous system: rarely - insomnia, psychomotor agitation;
- cardiovascular system: rarely - decrease / increase in blood pressure (BP), tachycardia;
- allergic reactions: rarely - itchy skin, flushing of the skin, rash, angioedema;
- digestive system: rarely - dyspeptic disorders;
- laboratory parameters: eosinophilia;
- other reactions: extremely rarely - general weakness.
Overdose
Symptoms of an overdose of Angiocardil may include a decrease in blood pressure, accompanied by dizziness, general weakness, headache, tachycardia.
If an overdose is suspected, symptomatic treatment is performed.

Angiocardil does not belong to the first-line drugs in acute coronary syndrome and its use is not urgently needed.

Angiocardil is contraindicated in pregnant women.

- cardiac glycosides, coronary dilator drugs, some antihypertensive drugs: the therapeutic effect of these drugs may increase;
- antianginal and antiarrhythmic drugs, antiplatelet agents, anticoagulants, bronchodilators, diuretics: the combined use of these drugs with meldonium is allowed.
