Ammifurin is a photosensitizing agent of plant origin.
Release form and composition
- Tablets: round, biconvex, light yellow with a greenish tinge of color with darker blotches, with a weak specific odor (10, 30 or 50 pcs. In blister contoured packs, in a cardboard box 3 or 5 packs of 10 tabs., 1 pack 30 or 50 tabs each);
- Solution for external use 0.3%: transparent liquid, light yellow with a greenish tinge, with the smell of alcohol, sensitive to light (50 and 100 ml in dark glass bottles, in a cardboard box 1 bottle; 1000 and 5000 ml in orange glass bottles).
The active substance is furocoumarins (bergapten, isopimpinellin and xanthotoxin) of large ammonium fruits:
- 1 tablet - 20 mg;
- 1 ml of solution - 3 mg.
Excipients of tablets: milk sugar (lactose), calcium stearate, potato starch.
Additional components of the solution: ethanol 95%, glycerin and purified water.
Indications for use
Ammifurin is used as a photosensitizing agent in combination with ultraviolet irradiation (UV):
- Vitiligo;
- Psoriasis;
- Fungal mycosis;
- Alopecia areata and alopecia totalis.
Contraindications
- Diseases accompanied by increased sensitivity to light, such as albinism and systemic lupus erythematosus;
- Benign and malignant tumors;
- Multiple pigmented nevi;
- Decompensated endocrine diseases;
- Thyrotoxicosis;
- Diseases of the central nervous system (cachexia);
- Kidney disease (nephritis);
- Liver disease (hepatitis, liver cirrhosis);
- Pancreatitis;
- Acute gastritis, exacerbation of gastric ulcer and 12 duodenal ulcer;
- Diabetes;
- Tuberculosis;
- Cataract;
- Diseases of the cardiovascular system (hypertension II-III degree, chronic heart failure);
- Diseases of the blood;
- Lactase deficiency, lactose intolerance, glucose-galactose malabsorption (for tablets);
- Pregnancy and breastfeeding period;
- Age up to 18 years (for tablets), up to 5 years (for solution);
- Intolerance to UFOs and psoralens.
Ammifurin should be used with caution when treating children and the elderly (over 60 years old) people.
Method of administration and dosage
Ammifurin is prescribed in combination with UFO.
In the form of tablets, the drug should be taken orally after meals, washed down with milk, 2 hours before irradiation. A single dose - 0.8 mg / kg, but not more than 80 mg (4 tablets) per dose. The course dose is 100-150 tab. (2000-3000 mg).
In the form of a solution, the agent is applied externally, applying to the lesions, 1 hour before irradiation. Heading dose is 1-2 bottles of 50 ml (150-300 mg).
At intervals of 1-1.5 months, repeated courses are carried out.
With limited lesions, the drug is prescribed only externally. With common forms of the disease, the external application of Ammifurin is combined with its intake.
Ultraviolet irradiation procedures for psoriasis are carried out 4 times a week, the irradiation starts with ½ biodose and gradually increases to 5-6 biodoses, which corresponds to the duration of irradiation from 1 to 15 minutes (depending on the individual sensitivity to ultraviolet radiation). The general course of treatment consists of 15-20 procedures. If necessary, treatment is carried out in combination with long-wave UV radiation.
In other cases, irradiation is carried out 3 times a week. The general course consists of 20-25 procedures, 4-6 courses are shown in total.
In all cases, in the summer, irradiation with a quartz lamp can be replaced with dosed solar insolation.
Side effects
- Skin reactions: secondary skin pigmentation, pruritus, moderate erythema;
- When combined with UV: nausea, dizziness, a feeling of heaviness in the stomach, headache, cardialgia, palpitations (in this case, the drug should be canceled);
- In case of an overdose of artificial or solar radiation: acute dermatitis (edema and hyperemia of the skin, blisters).
special instructions
In case of local exposure, in order to avoid irradiation of apparently healthy skin surrounding the lesions, at a distance of 1-2 cm from the boundaries of the lesion, it should be lubricated with a photoprotective cream or zinc paste, and after the UFO procedure, removed with a cotton swab moistened with vegetable oil.
Care must be taken when using cosmetics and eating products that contain photosensitizing substances.
Treatment should be carried out under medical supervision.
Each Ammifurin tablet contains 0.079 g of carbohydrates, which corresponds to 0.01 bread units.
When taking pills, it is recommended to avoid being in direct sunlight, wear light-protective glasses during the day, and also be careful when driving and performing potentially dangerous types of work that require quick psychophysical reactions and increased attention.
Drug interactions
The specified therapy can be carried out simultaneously with conventional drug treatment, including the use of vascular drugs, penicillin, lidase and external drugs.
Ammifurin enhances the action of other photosensitizers (including photosensitizing cosmetics and food products).
