Alphadol-Ca is a combined drug that regulates calcium-phosphorus metabolism and replenishes vitamin D3 and calcium deficiencies.

Release form and composition

Alphadol-Ca is produced in the form of capsules: soft gelatinous, oblong, have a double color (light blue / dark blue); the capsules contain an almost white pasty mass (10 pcs. in blisters, 3 blisters in a cardboard box).

Active ingredients in 1 capsule:

• Alfacalcidol - 250 mg;
• Calcium Carbonate 500 mg (Elemental Calcium 200 mg)

Auxiliary components: hydrogenated castor oil - 6 mg, hydrogenated vegetable oil - 29 mg, LTG mixture: butylhydroxyanisole - 0.063 mg, corn oil - 314.937 mg.

Sheath (dark blue / light blue part): titanium dioxide - 0.32 / 0.32 mg, gelatin - 46.14 / 46.19 mg, 70% sorbitol solution - 2.31 / 2.31 mg, glycerol 13.84 / 13.86 mg, propyl parahydroxybenzoate - 0.13 / 0.13 mg, methyl parahydroxybenzoate - 0.22 / 0.22 mg, water - 36.91 / 36.95 mg, brilliant blue dye - 0.13 / 0.02 mg.

Indications for use

• Osteoporosis (steroid / senile / idiopathic / menopausal), as well as its complications (bone fractures);
• Chronic renal failure
• Hyperparathyroidism and hypoparathyroidism (with damage to the bones);
• Renal osteodystrophy.

Contraindications

Absolute:

• Urolithiasis (the formation of calcium stones);
• Hypersensitivity to vitamin D3;
• Increased calcium in the blood (with increased function of the parathyroid glands, osteochondrosis, hypervitaminosis D, tumor metastases in the bone);
• Hyperphosphatemia (except when it develops against the background of hypoparathyroidism);
• Hypermagnesemia;
• Soy or peanut intolerance;
• Zollinger syndrome;
• Age under 18;
• Pregnancy and lactation (the safety profile for this group of patients has not been studied. The appointment of Alfadol-Ca is possible only for absolute indications and in cases where the potential benefit of its use is higher than the possible risk to the fetus. The maximum daily dose during pregnancy is 1500 mg of calcium carbonate);
• Hypersensitivity to drug components.

Alphadol-Ca is prescribed with caution in the presence of renal failure, as well as in patients on hemodialysis.

When prescribing a long course of treatment, you need to check the amount of calcium excreted by the kidneys.

Method of administration and dosage

Alphadol-Ca is taken orally.

Recommended dosing regimen (daily dose):

• Osteoporosis, hypoparathyroidism, hyperparathyroidism, chronic renal failure - 1-2 capsules;
• Osteodystrophy in chronic renal failure - 2 capsules.

Side effects

Alfacalcidol is an active derivative of vitamin D3. Vitamin D3 and its derivatives in therapeutic doses should not be prescribed during treatment (the likelihood of an increased risk of hypercalcemia and the development of additive effects).

Alphadol-Ca enhances intestinal absorption of phosphate and calcium, the concentration of which must be controlled, especially in renal failure. During the period of therapy, the concentrations of alkaline phosphatase, creatinine, phosphate, calcium and magnesium should be monitored.

Hypercalciuria or hypercalcemia is quickly eliminated by abolishing Alphadol-Ca (until the plasma calcium concentration normalizes). With hypercalciuria, in particular with urolithiasis, the drug should be prescribed with caution.

Drug interactions

With the combined use of Alfadol-Ca with certain substances / preparations, the following effects may develop:

• Digitalis glycosides: increased cardiac arrhythmias (the combination is recommended to be avoided);
• Barbiturates and anticonvulsants, which are inducers of microsomal liver enzymes (systematic use): development of interaction (an increase in the dose of alfacalcidol is necessary);
• Mineral oils (with prolonged use), colestipol, cholestyramine, sucralfate, as well as a large number of antacids containing aluminum: impaired absorption of alfacalcidol;
• Antacids and laxatives containing magnesium (in hemodialysis patients): development of hypermagnesemia;
• Thiazide diuretics: increased risk of hypercalcemia;
• Tetracycline, ciprofloxacin: impaired absorption of calcium carbonate.
