Alcaine is a local anesthetic used in ophthalmology.

Release form and composition

Alcaine is produced in the form of eye drops: a clear liquid, from slightly yellowish-brownish or slightly yellowish to colorless (15 ml in polyethylene dropper bottles "Drop Tainer", 1 dropper bottle in a cardboard box).

Composition of 1 ml solution:

  • Active ingredient: proxymetacaine hydrochloride - 5 mg;
  • Auxiliary components: glycerol - 24.5 mg; benzalkonium chloride (in the form of benzalkonium chloride solution) - 0.1 mg; concentrated hydrochloric acid and / or sodium hydroxide - in a sufficient amount; purified water - up to 1 ml.

Indications for use

Alcaine is prescribed for local anesthesia in ophthalmology during short-term medical and diagnostic procedures that require quick and short-term pain relief:

  • Conjunctival scraping for diagnostic purposes;
  • Surgery to remove a cataract, suture from the cornea or conjunctiva;
  • Removal of foreign bodies from the cornea;
  • Gonioscopy, tonometry.

Relative (Alcaine is prescribed with caution in the presence of the following diseases / conditions):

  • Heart disease;
  • Low plasma acetylcholinesterase;
  • Allergic reactions;
  • Epilepsy;
  • Myasthenia gravis;
  • Hyperthyroidism and breathing disorders;
  • Combined use with cholinesterase inhibitors (due to the high risk of developing systemic adverse reactions);
  • Age up to 18 years (due to insufficient experience with Alkain in this age group).

Method of administration and dosage

Alcaine is used topically.

Recommended dosage regimen:

  • Tonometry and other short procedures: 1-2 drops immediately before the procedure. If necessary, the dose may be increased;
  • Minor surgical procedures (to remove a foreign body or suture thread): every 5-10 minutes, 1-2 drops (1 to 3 times);
  • Long procedures (for example, to remove cataracts): every 5-10 minutes, 1-2 drops (3 to 5 times).

Side effects

Possible side reactions (often - 1-10% of cases; rarely - 0.1-1% of cases; with an unknown frequency - if it is impossible to estimate the frequency of development according to the available data):

  • Local reactions: often - burning eyes, itching, conjunctival hyperemia; rarely - descemetitis, diffuse stromal edema, iritis, allergic reactions of an immediate type (immediately after instillation), which are characterized by the appearance of epithelial keratitis with the formation of infiltrates and zones of epithelial necrosis;
  • Systemic reactions: rarely - systemic toxicity (in the form of excitation of the central nervous system with its subsequent depression); with an unknown frequency - photophobia, transient blurred vision after instillation of the drug, increased lacrimation, infections / corneal opacities with prolonged loss of vision and corneal perforation (due to an increase in the amount of the drug used).

Alcaine can only be used topically, it is not intended for injection.

After the end of the surgical procedure, you need to cover the eye with a bandage, since the blink reflex is briefly eliminated.

When using Alkain, the use of contact lenses is not recommended.

Long-term therapy can reduce the duration of the effect, and therefore an increase in the amount of the drug may be required to achieve the desired anesthetic effect.

During the entire period of anesthesia, it is necessary to protect the eye from the ingress of foreign bodies, irritating substances into it, and also to rub the eye until the end of the anesthetic effect.

If symptoms of eye irritation or hypersensitivity develop, the drug should be discontinued.

The bottle should be closed after each use. Do not touch the eye with the pipette tip.

If after using Alcaine there is a short blurring of vision, it is not recommended to drive vehicles and perform work that requires an increased reaction and concentration until it is restored.
