Yumex is a drug from the group of antiparkinsonian drugs, type B monoamine oxidase inhibitors. The active ingredient is selegiline. Participates in the metabolism of dopamine and other catecholamines. The drug inhibits the reuptake of serotonin in presynaptic dopamine receptors, increasing its concentration in the nuclei of both the extrapyramidal system and other parts of the brain.

Yumex is able to potentiate, prolong the effect of levodopa in the treatment of parkinsonism.

Umex is not used to treat patients with extrapyramidal symptoms not accompanied by dopamine deficiency (Huntington's chorea, essential tremor).

When taken orally, Yumex is quite intensively and quickly absorbed. The maximum concentration is reached after 0.5-2 hours. Binds to plasma proteins in the amount of 94%. Yumex is rapidly metabolized in the body. The main metabolites are L-amphetamine, L-methamphetamine, N-dismethylselegiline. Both the active substance and metabolites are able to penetrate the blood-brain barrier. Metabolites are excreted in the urine (about 73%) and through the intestines (about 15%).

Indications for the use of Yumex

According to the instructions for Yumex, the drug is indicated in the treatment of Parkinson's disease, symptomatic parkinsonism.

As monotherapy, Yumex can be used in the early stages of Parkinson's disease to delay the use of levodopa (with or without a peripheral decarboxylase inhibitor), and can also be used with levodopa (with or without PID).

Contraindications to the use of Yumex

The following contraindications are indicated in the instructions for Yumex:

  • Hypersensitivity to selegiline;
  • Extrapyramidal pathology not associated with dopamine deficiency (Huntington's chorea, essential tremor);
  • Pregnancy, lactation.

With extreme caution, Yumex should be prescribed for such a pathology:

  • Peptic ulcer, duodenal ulcer;
  • Lability of blood pressure;
  • Tachycardia;
  • Severe angina;

  • Tardive dyskinesia;
  • Coarse tremor;
  • Thyrotoxicosis;
  • Pheochromocytoma;
  • Hyperplasia of the prostate;
  • Angle-closure glaucoma.

In combination with levodopa, Yumex should not be prescribed for:

  • Open-angle glaucoma;
  • Renal, liver failure;
  • Melanoma;
  • Depression of the central nervous system;
  • Convulsive seizures;
  • HZL;
  • Bronchial asthma;
  • Myocardial infarction.

Method of application, dosage

According to the instructions, Yumex is administered orally at a daily dosage of 5-10 mg (with monotherapy and in conjunction with levodopa). You can take Yumex once in the morning. With a daily dosage of 10 mg, the dose can be divided into two doses. With the simultaneous appointment of Yumex with levodopa, the dosage of levodopa can be reduced by about 30%.

Side effect

The drug Yumex with monotherapy is well tolerated.

  • Allergic reactions: shortness of breath, rash, photosensitivity, bronchospasm;
  • from the gastrointestinal tract: dry mouth, nausea, constipation, diarrhea, loss of appetite, dysphagia;
  • From the CVS: orthostatic hypotension, less often - increased blood pressure, arrhythmias;
  • From the side of the central nervous system: mood changes, insomnia, dizziness, psychosis, dyskinesia;
  • Others: increased levels of hepatic transaminases, urinary retention, nocturia, headache, diplopia, impaired visual acuity, increased sweating, hair loss, hypoglycemia.

Since Yumex is able to potentiate the clinical effects of levodopa, the side effects of levodopa (hyperkinesis, agitation, abnormal movements, confusion, postural hypotension, arrhythmias, hallucinations) are also enhanced with combination therapy.

Interaction of Yumex with other drugs

Combinations of Yumex with the following drugs are contraindicated:

  • Sympathomimetics: danger of increased blood pressure;
  • Pethidine: Non-selective MAO inhibitors and pethidine are incompatible. The mechanism of this interaction is unknown, however, the use of Yumex, pethidine, or another opioid analgesic is absolutely contraindicated;
  • Serotonin reuptake inhibitors: Yumex should not be taken with fluoxetine due to the risk of hypertension, agitation, convulsions, ataxia, hallucinations, and manic psychosis. Fluoxetine should not be taken for two weeks after discontinuation of Yumex. Due to the long half-life of fluoxetine after its cancellation and before using Yumex, more than 5 weeks should pass;
  • Tricyclic antidepressants: the phenomena of severe toxic damage to the central nervous system (tremor, dizziness, seizures), accompanied by hypotension, hypertension, profuse sweating in patients who took this combination, have been described;

It is not recommended to combine Yumex with MAO inhibitors (risk of severe hypotension).

Yumex enhances the clinical effects of levodopa, agents that depress the central nervous system, ethanol, and increases the frequency of side effects of amantadine.

Interaction with food

When monotherapy with Yumex (as opposed to treatment with traditional MAO inhibitors), a special diet should not be followed. However, when combined therapy with MAO or MAO type A inhibitors, it is necessary to avoid foods containing a large amount of tyramine: yeast products, mature cheese.

In connection with the property of this drug to potentiate the action of levodopa and enhance its side effects, constant monitoring of the patient's condition is necessary. Involuntary movements, arousal that occur when taking this combination of drugs, disappear when the dosage of levodopa is reduced. The maximum daily dosage of Yumex (10 mg) should not be exceeded, since in this case the drug loses its selectivity, which increases the likelihood of developing hypertension.

Studies showing the effect of Yumex on the ability to drive vehicles have not been conducted. However, you should refrain from potentially dangerous activities that require increased attention, speed of motor, mental reactions.

The drug Yumex is contraindicated in pregnancy, lactation.
