Release form and composition

Dosage form - tablets: round, flat-cylindrical, light brown interspersed, with a line and a chamfer, have a cocoa smell (in a cardboard box, instructions for the use of Citramon-LekT and 1-2 contour cell / cell-free packages containing 10 tablets each).

Composition of Citramon-LekT for 1 tablet:

  • active substances: acetylsalicylic acid (ASA) - 240 mg, paracetamol - 180 mg, anhydrous caffeine - 27.5 mg (in terms of caffeine monohydrate - 30 mg);
  • auxiliary components: potato starch, talc, calcium stearate, citric acid, cocoa.

Pharmacological properties

Pharmacodynamics

Citramon-LekT is a drug containing three medicinal components as active substances that determine its properties:

  • acetylsalicylic acid is a non-steroidal anti-inflammatory drug (NSAID) with analgesic and antipyretic effects. The drug relieves pain well, especially against the background of inflammation. In the focus of inflammation, it improves microcirculation. Also moderately inhibits platelet aggregation, reduces thrombus formation;
  • paracetamol is an analgesic-antipyretic, with predominantly analgesic and antipyretic effects. Shows anti-inflammatory activity to a weak degree. The mechanism of its action is due to the influence on the center of thermoregulation in the hypothalamus, as well as the weakly expressed ability to inhibit the synthesis of prostaglandins in peripheral tissues;
  • caffeine is a psychostimulant and analeptic agent that stimulates the respiratory and vasomotor centers, increases the reflex excitability of the spinal cord, dilates the blood vessels of the brain, skeletal muscles, heart and kidneys, and also reduces platelet aggregation. As part of Citramon-LekT, the dose of caffeine is small, so it practically does not stimulate the central nervous system (CNS), but regulates the tone of the cerebral vessels, which helps to relieve the feeling of fatigue and reduce drowsiness.

Pharmacokinetics

Pharmacokinetic parameters of active components of Citramon-Lect:

  • acetylsalicylic acid: entering the gastrointestinal tract, it is rapidly and in large quantities absorbed. In the process of absorption, it undergoes presystemic elimination in the intestinal wall and liver. The resorbed part of the drug under the influence of esterases quickly undergoes a hydrolysis process with a half-life (T 1/2) of no more than 20 minutes. As a result, the salicylic acid anion circulates in the body. Passes through the placenta, in small quantities into breast milk. The drug easily penetrates into all fluids and tissues of the body, in the presence of edema and hyperemia, diffusion is accelerated, with inflammation in the proliferative phase, it slows down. Biotransformation undergoes in the liver, resulting in the formation of 4 metabolites. It is excreted by the kidneys, with alkalinization of urine, a significant increase in excretion is noted;
  • paracetamol: rapidly absorbed from the gastrointestinal tract. The maximum plasma concentration (C max) reaches within 30-60 minutes. Biotransformed in the liver to form paracetamol sulfate and glucuronide. T 1/2 is 1-4 hours, excreted mainly by the kidneys, mainly in the form of conjugated products, less than 5% is excreted unchanged;
  • caffeine: well absorbed in the intestines. It is excreted mainly in the form of metabolites by the kidneys; about 10% is excreted unchanged. T 1/2 is on average 5 hours, in individual cases it can increase up to 10 hours.

Indications for use

  • pain syndrome of mild and moderate severity: algomenorrhea, myalgia, neuralgia, arthralgia, migraine, headache and toothache;
  • fever due to influenza and acute respiratory infections.

Contraindications

Absolute contraindications for taking Citramon-LekT are:

  • a history of bronchial asthma induced by the intake of ASA, salicylates or other NSAIDs;
  • complete or incomplete combination of intolerance to ASA or other NSAIDs with recurrent polyposis of the nose or paranasal sinuses (currently or in history);
  • glaucoma;
  • severe liver dysfunction;
  • serious functional impairment of the kidneys;
  • gastrointestinal bleeding, including a history;
  • hypoproteinemia;
  • vitamin deficiency K;
  • exacerbation of erosive and ulcerative lesions of the gastrointestinal tract;
  • surgical interventions accompanied by profuse bleeding;
  • hemophilia, hypocoagulation, hemorrhagic diathesis, hypoprothrombinemia;
  • deficiency of glucose-6-phosphate dehydrogenase;
  • portal hypertension;
  • severe arterial hypertension;
  • organic diseases of the cardiovascular system (including atherosclerosis and acute myocardial infarction);
  • sleep disturbances, hyperexcitability;
  • I and III trimesters of pregnancy;
  • lactation period;
  • age up to 15 years (due to the risk of developing Reye's syndrome against a background of a viral disease);
  • hypersensitivity to any active or auxiliary component of the drug.

Relative contraindications to the use of Citramon-LekT tablets:

  • tendency to seizures or the presence of epilepsy;
  • benign hyperbilirubinemia (including Gilbert's syndrome);
  • gout;
  • increased activity of hepatic transaminases;
  • mild to moderate renal failure;
  • alcoholic liver damage;
  • II trimester of pregnancy;
  • elderly age.

Citramon-LekT, instructions for use: method and dosage

Citramon-Lect tablets should be taken orally during or immediately after a meal. The irritating effect of the drug on the gastrointestinal tract can be reduced by drinking it with alkaline mineral water or milk.

Recommended dosages:

  • fever - 1 tablet with a minimum interval of 4 hours;
  • pain syndrome - 1-2 tablets no more than once every 4 hours.

The average recommended dose of Citramon-LekT per day is 3-4 tablets, the maximum allowable dose is 8 tablets.

Unless otherwise prescribed by a doctor, at elevated body temperature, the drug should be treated for no more than 3 days, with pain syndrome - 5 days.

Side effects

  • on the part of the cardiovascular system: tachycardia, increased blood pressure;
  • from the central nervous system: dizziness;
  • from the gastrointestinal tract: gastralgia, nausea, vomiting, anorexia, erosive and ulcerative lesions of the gastrointestinal tract, gastrointestinal bleeding;
  • from the immune system: allergic reactions, toxic epidermal necrolysis, exudative erythema multiforme, bronchospasm;
  • from the kidneys: renal failure;
  • from the liver: liver failure;
  • hematological disorders: decreased platelet aggregation;
  • others: Reye's syndrome in children.

In some cases, with prolonged use, Citramon-Lect causes dizziness, headaches, tinnitus, deafness, visual impairment, liver dysfunction, vomiting, metabolic acidosis, hyperpyrexia, kidney damage with papillary necrosis, hemorrhagic syndrome (nosebleeds, bleeding gums, purpura, etc.), hypocoagulation, disorders of the nervous system and psyche.

Overdose

An overdose of Citramon-Lect can cause disturbances from various body systems:

  • light intoxication: ringing in the ears, dizziness, stomach pain, nausea, vomiting;
  • severe poisoning: drowsiness, lethargy, shortness of breath, convulsions, bleeding, anuria, collapse.

In case of an overdose of Citramon-LekT, you should rinse the stomach, take activated charcoal and call an ambulance. In severe cases, hospitalization is required. Treatment of an overdose is symptomatic and depends on the patient's symptoms and his general condition. If necessary, to enhance the excretion of ASA, sodium lactate, sodium bicarbonate or sodium citrate is injected.

Citramon-LekT is capable of causing a gout attack in patients with a predisposition to uric acid accumulation.

During the period of therapy, patients are not recommended to consume alcohol, since the drug increases the risk of toxic liver damage and the development of bleeding from the gastrointestinal tract.

During treatment with the drug, the state of the peripheral blood and liver function should be monitored.

Citramon-LekT slows down blood clotting. In case of planned surgical intervention, the patient must warn the doctor that he is taking a drug containing ASA.

Influence on the ability to drive vehicles and complex mechanisms

The caffeine in Citramon-LekT with increased excitability can cause anxiety and insomnia. In this regard, during the period of drug treatment, it is recommended to refrain from performing potentially hazardous work that requires speed of reactions and increased attention.

Application during pregnancy and lactation

Citramon-LekT should not be taken in the first and third trimesters of pregnancy, since there is a risk of teratogenic effects and inhibition of labor (respectively).

In the second trimester, the drug can be prescribed by a doctor, but should be used with extreme caution.

ASA penetrates into breast milk, therefore Citramon-LekT is contraindicated during lactation.

Pediatric use

ASA is prohibited for children under 15 years old, because against the background of a viral infection it can cause the development of Reye's syndrome, manifested by prolonged vomiting, enlarged liver and acute encephalopathy.

With impaired renal function

Citramon-LekT is allowed to be used with caution in case of mild to moderate renal failure. However, in case of severe renal impairment, the drug should not be taken.

For violations of liver function

In case of increased activity of hepatic transaminases, Citramon-LekT should be used with caution.

In case of severe liver disorders, the drug is contraindicated.

Drug interactions

Acetylsalicylic acid:

  • diuretics (furosemide, spironolactone), antihypertensive drugs, uricosuric drugs (sulfinpyrazone, benzbromarone): their effect decreases;
  • indirect anticoagulants, heparin, platelet aggregation inhibitors, thrombolytics, oral antidiabetic drugs, narcotic analgesics (propoxyphene, oxycodone, codeine): their effects are enhanced;
  • methotrexate: its renal clearance increases, toxicity increases;
  • digoxin, lithium salts, barbiturates: their concentration increases;
  • magnesium and aluminum-containing antacids: absorption of ASA slows down and worsens;
  • myelotoxic drugs: the hematotoxic effects of ASA are enhanced;
  • glucocorticoids, ethanol (including drugs containing it): the clearance of ASA increases and its negative effect on the gastrointestinal tract increases.

Paracetamol:

  • chloramphenicol: its T 1/2 increases approximately 5 times;
  • anticoagulants, which are coumarin derivatives: their effect is enhanced with repeated doses of paracetamol;
  • metoclopramide: paracetamol absorption is accelerated;
  • inducers of microsomal liver enzymes, including antiepileptic drugs, salicylamide, rifampicin, barbiturates: the formation of toxic metabolites of paracetamol, which have a negative effect on the liver, increases.

Caffeine:

  • cardiac glycosides: their absorption is accelerated, the effect is enhanced, and toxicity is increased;
  • ergotamine: its absorption is accelerated;
  • lithium preparations: their excretion in the urine increases;
  • sleeping pills and narcotic drugs: their effect decreases;
  • calcium preparations: their absorption in the gastrointestinal tract decreases;
  • theophylline (and possibly other xanthines): their clearance decreases, there is a risk of increased toxic effects;
  • selegiline, furazolidone, procarbazine, monoamine oxidase inhibitors: with high doses of caffeine, the risk of a significant increase in blood pressure and the development of dangerous cardiac arrhythmias increases;
  • nicotine: the rate of excretion of caffeine from the body increases;
  • mexiletine: caffeine excretion is reduced by almost half;
  • adrenergic bronchodilators: there is a possibility of increased stimulation of the central nervous system and the development of other additive toxic effects;
  • beta-blockers: mutual suppression of therapeutic effects is possible;
  • anticonvulsants, barbiturates, norfloxacin, disulfiram, primidone, ciprofloxacin: the metabolism of caffeine in the liver decreases, which increases its concentration in the blood;
  • drugs that stimulate the central nervous system, and caffeine-containing drinks: there is a risk of overstimulation of the central nervous system;
  • adenosine: caffeine is an antagonist of it, so a significant dose increase may be required.
