Release form and composition

Dosage forms of Ceresil Canon:

1 ampoule contains:

  • active ingredient: citicoline - 500 or 1000 mg (in the form of sodium citicoline - 522.5 or 1045 mg, respectively);
  • additional substances: 1M hydrochloric acid solution or 1M sodium hydroxide solution - up to pH 6.5–7.1; water for injection - up to 4 ml.

1 ml of oral solution contains:

  • active ingredient: citicoline - 100 mg (in the form of sodium citicoline - 104.5 mg);
  • additional substances: sorbitol - 200 mg; glycerin - 50 mg; sodium citrate - 6 mg; methyl parahydroxybenzoate - 1.45 mg; potassium sorbate - 3 mg; banana flavor (Banana essence) - 0.4 mg; propyl parahydroxybenzoate - 0.25 mg; sodium saccharinate - 0.2 mg; 50% citric acid solution - up to pH 5.9-6.1; purified water - up to 1 ml.

Pharmacological properties

Pharmacodynamics

Citicoline is a natural endogenous compound, an intermediate metabolite in the synthesis of phosphatidylcholine, which acts as one of the main structural components of the cell membrane.

Citicoline, as a precursor of key ultrastructural components of the cell membrane (mainly phospholipids), demonstrates a wide spectrum of action - it restores damaged cell membranes, inhibits the action of phospholipases, prevents the increased formation of free radicals, and also prevents cell death by affecting the mechanisms of apoptosis. In the acute phase of stroke, the substance improves cholinergic transmission, reduces the amount of damage to the brain tissue. With traumatic brain injury, it reduces the severity of neurological symptoms and the duration of post-traumatic coma, and reduces the duration of the recovery period.

In patients with chronic hypoxia of the brain, citicoline is effective in the treatment of cognitive disorders (difficulties arising in the performance of daily activities and self-care, lack of initiative, memory impairment). Reception of Ceresil Canon helps to increase the level of attention and consciousness, as well as reduce the manifestations of amnesia.

The effectiveness of citicoline has been proven in the treatment of sensory and motor neurological disorders of vascular and degenerative etiology.

Pharmacokinetics

Pharmacokinetic parameters of citicoline:

  • absorption: citicoline is well absorbed both by ingestion and by intravenous and intramuscular administration; after oral administration, absorption is almost complete, bioavailability is approximately the same as after intravenous administration;
  • distribution: citicoline is largely distributed in the structures of the brain with the rapid introduction of the cytidine fraction into nucleic acids and cytidine nucleotides, and choline fractions into structural phospholipids. Penetrates into the brain and actively integrates into cellular, mitochondrial and cytoplasmic membranes, while forming part of the fraction of structural phospholipids;
  • metabolism: when administered intravenously and intramuscularly, citicoline is metabolized in the liver, when taken orally - in the liver and intestines, with the formation of cytidine and choline during the metabolism. Plasma choline concentrations increase significantly after parenteral / oral administration;
  • excretion: only 15% of the administered dose of citicoline is excreted from the human body (of which less than 3% - with urine and feces, about 12% - with exhaled CO 2); there are 2 phases in the excretion of citicoline with urine: the rate of excretion in the first phase (its duration is about 36 hours) decreases rapidly, the rate of excretion in the second phase decreases much more slowly. Similar processes are observed in the exhaled CO 2 - the rate of excretion decreases rapidly after about 15 hours, after which the decrease occurs much more slowly.

Indications for use

Ceresil Canon is prescribed in the following cases:

  • cognitive and behavioral disorders in degenerative and vascular diseases of the brain;
  • acute period of ischemic stroke - as part of complex therapy;
  • an acute period with craniocerebral trauma - as part of complex therapy;
  • recovery period in ischemic and hemorrhagic strokes, traumatic brain injury.

Contraindications

Absolute:

  • severe vagotonia (predominance of the tone of the parasympathetic part of the autonomic nervous system);
  • children and adolescents up to 18 years of age (since there is insufficient clinical data on the safety and efficacy of the drug in this group of patients);
  • breast-feeding;
  • established hypersensitivity to the active substance of the drug or auxiliary components in its composition.

With caution and only on condition that the expected benefit to the mother exceeds the possible risks to the fetus, Ceresil Canon can be used to treat pregnant women.

Ceresil Canon, instructions for use: method and dosage

Solution for i / v and i / m administration

Ceresil Canon is prescribed intravenously (this route of administration is preferable) and intramuscularly.

The intravenous solution is used in the form of a slow intravenous injection (its duration depends on the prescribed dose and varies within 3-5 minutes) or as a drip intravenous infusion (the rate of administration is 40-60 drops / min).

When administered intramuscularly, it is important to avoid repeated administration of the solution in the same place.

Recommended dosage regimen of Ceresil Canon:

  • acute period of ischemic stroke and traumatic brain injury: 1000 mg every 12 hours from the first day after diagnosis, the course of therapy is at least 6 weeks. After 3-5 days after the start of treatment (in the absence of impaired swallowing function), it is allowed to switch to taking Ceresil Canon orally (in the form of an oral solution);
  • recovery period for ischemic and hemorrhagic strokes, traumatic brain injury; cognitive and behavioral disorders in degenerative and vascular diseases of the brain: 500-2000 mg per day; the dosage and duration of therapy depend on the severity of the symptoms of the disease. It is possible to use the drug Ceresil Canon orally (in the form of an oral solution). If there is a suspicion of ongoing intracranial bleeding, it is not recommended to exceed the dose of the drug, which is 1000 mg / day; the introduction is carried out intravenously at a rate of 30 drops / min.

When prescribing a drug for elderly patients, dose adjustment is not required.

Solution in ampoule is intended for single use. After opening the ampoule, it must be used immediately.

Ceresil Canon is compatible with all types of isotonic solutions for intravenous use and dextrose solutions.

Oral solution

Ceresil Kanon oral solution is administered orally. Before use, the drug must be diluted in 100-120 ml (½ glass) of water.

The ready-made solution should be taken between meals or directly with meals.

Recommended dosage regimen of Ceresil Canon:

  • acute period of ischemic stroke and traumatic brain injury: 1000 mg (10 ml) every 12 hours, starting from the first day after diagnosis; the course of therapy is at least 6 weeks;
  • recovery period for ischemic and hemorrhagic strokes, traumatic brain injury; cognitive and behavioral disorders in degenerative and vascular diseases of the brain: 500-2000 mg per day (5-20 ml), if necessary, the daily dose can be divided into two doses; the dosage and duration of therapy depend on the severity of the symptoms of the disease.

Elderly patients do not need to adjust the dose of Ceresil Canon.

Instructions for using the dosing syringe attached to the bottle:

  1. Place the dosing syringe into the vial (the syringe plunger must be fully lowered).
  2. Gently pull on the plunger of the dosing syringe until the solution level rises to the appropriate mark on the syringe.
  3. Dilute the required amount of solution in 100–120 ml (½ cup) of water before taking.
  4. After each use, rinse the dosing syringe thoroughly with water.

Side effects

Adverse reactions from systems and organs when taking Ceresil Canon (develop extremely rarely, with a frequency of less than 1/10 000, including individual episodes):

  • central and peripheral nervous system: dizziness, headache, insomnia, numbness in paralyzed limbs, tremors, stimulation of the parasympathetic nervous system;
  • psyche: agitation, hallucinations;
  • heart and blood vessels: short-term changes in blood pressure (blood pressure);
  • digestive system: decreased appetite, nausea, vomiting, diarrhea, changes in the activity of liver enzymes;
  • allergic reactions: itching, rash, anaphylactic shock;
  • other reactions: shortness of breath, swelling, feeling of heat.

In case of aggravation of any of the above side effects or the appearance of new adverse reactions that are not specified in this instruction, you must urgently inform your doctor.

Overdose

Due to the extremely low toxicity of Ceresil Canon, an overdose is unlikely even if the therapeutic doses are exceeded.

The introduction of the Ceresil Canon solution into a vein is carried out in a slow stream (duration of administration - from 3 to 5 minutes, depending on the dose) or by drip with an infusion rate of 40 to 60 drops / min.

In the case of persistent intravenous hemorrhage, it is not recommended to exceed the dose of the drug, which is 1000 mg / day; the introduction is carried out intravenously at a rate of 30 drops / min.

In a solution for oral administration, when exposed to cold, crystals may form in a small amount due to the temporary partial crystallization of the preservative. Upon further storage under recommended conditions, the crystals dissolve within several months. The presence of crystals in the solution does not affect the quality of Ceresil Canon.

Application during pregnancy and lactation

There is insufficient data on the use of citicoline during gestation. In the course of studies conducted on animals, the negative effect of the drug was not revealed, but for the sake of precaution, Ceresil Canon is prescribed to pregnant women only if the expected benefit to the mother significantly outweighs the potential risks to the fetus.

The use of the drug during breastfeeding is contraindicated, since there is no data on the release of citicoline with human milk. If therapy with Ceresil Canon is required during lactation, it is necessary to resolve the issue of transferring the child to artificial feeding.

Pediatric use
