Prestans is a drug of a combination of amlodipine and perindopril, used for the treatment of primary and secondary hypertension, angina pectoris, chronic ischemic heart disease. Prestans refers to ACE inhibitors in combinations and calcium channel blockers in combinations.
Pharmacological action Prestans
Prestans consists of two active active components of different directions - antianginal and antihypertensive. The action of each component separately and the drug as a whole is considered.
Perindopril in Prestans inhibits kininase II, an enzyme that converts angiotensin I to vasoconstrictor angiotensin II and destroys vasodilating bradykinin to an inactive heptapeptide.
By inactivating bradykinin, ACE increases the activity of not only the circulating, but also the tissue kinin-kallikrein system, simultaneously with the activation of the prostaglandin system.
The therapeutic effect of perindopril is due to the inhibition of ACE by the active metabolite perindoprilat. Prestans is used to treat arterial hypertension of any degree of complexity. Its use helps to reduce diastolic and systolic blood pressure, both in the supine position and in the standing position. By decreasing the OPSS, perindopril improves peripheral blood flow, lowers blood pressure without affecting heart rate. By increasing renal blood flow, the drug does not alter the glomerular filtration rate.

In hypertension, the degree of hypertrophic changes in the left ventricular myocardium is significantly reduced.
With angina pectoris, a single daily dose of amlodipine in Prestans allows you to increase the total time of physical activity, delay the onset of angina attacks, reducing their frequency, as a result of which to reduce the dose of nitroglycerin consumed.
With arterial hypertension, a single daily dose of the drug will provide a significant decrease in blood pressure for 24 hours in the standing and lying position, without causing acute hypotension due to the slow onset of action.
