Paracetamol MS is an analgesic non-narcotic drug.

Release form and composition

Paracetamol MS is available in the form of tablets: flat-cylindrical, white with a cream shade or white, with a dividing line and a chamfer (200 mg and 500 mg each: 10 pcs. In blisters or bezelzheykovyh packaging, in a cardboard box 1, 2, 5 or 10 packs).

1 tablet contains:

  • active substance: paracetamol - 200 mg or 500 mg;
  • auxiliary components: potato starch, stearic acid, low molecular weight polyvinylpyrrolidone (povidone), talc.

Pharmacological properties

Pharmacodynamics

Paracetamol is a non-narcotic analgesic, the mechanism of action of which is due to the inhibition of the synthesis of prostaglandins and a decrease in their effect on the center of thermoregulation in the hypothalamus. The tablets have an analgesic and antipyretic effect, exhibit insignificant anti-inflammatory activity.

Pharmacokinetics

When taken orally, paracetamol is rapidly absorbed from the gastrointestinal tract. Its maximum concentration in blood plasma is reached in 20-60 minutes and is 0.005-0.02 mg / ml.

Plasma protein binding - 15% of the dose taken.

Paracetamol crosses the blood-brain and placental barriers, is excreted in breast milk (less than 1% of the dose taken by the mother).

The optimal therapeutic effect is provided by the concentration of paracetamol in plasma, achieved from taking a single dose of 10–15 mg per 1 kg of patient weight.

Paracetamol metabolism occurs with the participation of microsomal liver enzymes (90–95%), including the CYP2E1 isoenzyme. Most (80%) of metabolites as a result of conjugation with glucuronic acid and sulfates are converted into inactive metabolites, 17% as a result of hydroxylation form 8 active metabolites, which after conjugation with glutathione are converted into inactive metabolites. With a deficiency of glutathione, active metabolites, blocking the enzyme systems of hepatocytes, can cause their necrosis.

It is excreted through the kidneys in unchanged form - 3%, the rest - in the form of metabolites, mainly conjugates.

The half-life (T 1/2) is 1–4 hours.

In elderly patients, the clearance of the drug decreases, T 1/2 - increases.

Indications for use

According to the instructions, Paracetamol MS is indicated for symptomatic therapy aimed at reducing pain and inflammation in the following conditions:

  • mild or moderate pain (sore throat, toothache and headache, migraine pain, neuralgia and myalgia);
  • increased body temperature with infectious and inflammatory diseases, including colds.

The drug does not affect the progression of the disease.

Contraindications

  • genetic absence of glucose-6-phosphate dehydrogenase;
  • severe liver dysfunction;
  • blood diseases;
  • severe renal impairment;
  • confirmed hyperkalemia;
  • period after coronary artery bypass grafting;
  • active phase of gastrointestinal bleeding;
  • erosive and ulcerative lesions of the mucous membrane of the stomach and duodenum;
  • intestinal diseases of inflammatory etiology;
  • period of pregnancy;
  • breast-feeding;
  • age up to 3 years;
  • hypersensitivity to acetylsalicylic acid or nonsteroidal anti-inflammatory drugs (NSAIDs), accompanied by rhinitis, bronchial obstruction, urticaria (including history);
  • individual intolerance to the components of the drug.

Paracetamol MS should be taken with caution in case of liver dysfunction, Gilbert's syndrome (constitutional hyperbilirubinemia), renal dysfunction with creatinine clearance (CC) less than 60 ml / min, diabetes mellitus, ischemic heart disease, chronic heart failure, cerebrovascular pathologies, peripheral arterial diseases, dyslipidemia or hyperlipidemia, history of ulcerative lesions of the gastrointestinal tract, Helicobacter pylori infection, long-term treatment with NSAIDs, severe somatic diseases, smoking, frequent alcohol consumption, in old age.

In addition, caution should be exercised in patients who are on concomitant therapy with oral glucocorticosteroids (including prednisolone), anticoagulants (including warfarin), selective serotonin reuptake inhibitors (including citalopram, paroxetine, fluoxetine, sertraline), antiaggregalicilic acid (including acidylacetylulophenyl), antiaggrellicylates (including acidic acidogrellulopidoglycelium).

Instructions for the use of Paracetamol MS: method and dosage

Paracetamol MS tablets are taken orally.

The recommended dosage has the following age restrictions:

  • adults: 200 mg or 500 mg 1-4 times a day. The maximum single dose is no more than 1000 mg, the daily dose is 4000 mg;
  • children: at the rate of 10-15 mg per 1 kg of the child's weight 4 times a day with an interval of 6 hours. Maximum daily dose: under the age of 6 years (weight up to 22 kg) - 1000 mg, up to 9 years (weight up to 30 kg) - 1500 mg, up to 12 years (weight up to 40 kg) - 2000 mg.

Duration of use: as an anesthetic - no more than 5 days, to lower high body temperature - no more than 3 days.

Side effects

  • from the digestive system: it is possible (with prolonged use of high doses of Paracetamol MS) - toxic liver damage;
  • from the hematopoietic system: in rare cases - leukopenia, agranulocytosis, thrombocytopenia;
  • on the part of the excretory system: possibly (with prolonged use of high doses of the drug) - nephrotoxic effect;
  • on the part of the immune system: it is possible - the development of allergic reactions of a systemic nature or in the form of skin rashes.

Overdose

Symptoms: acute period - nausea, stomach pain, vomiting; after a few days - the appearance of signs of liver damage in the form of liver failure, encephalopathy, coma.

Treatment: immediate gastric lavage, intake of activated charcoal, after which you should seek medical attention.

The use of alcoholic beverages and oral administration of ethanol-containing drugs during the treatment period are contraindicated. In persons prone to chronic alcoholism, taking paracetamol can cause toxic liver damage.
