Spazgan is a combined drug with analgesic (non-narcotic), antipyretic, antispasmodic action.

Release form and composition

Dosage forms:

  • tablets: round flat shape, with a beveled edge, white with a yellowish tinge, on one side there is a dividing line, on the other - an engraving of the emblem (10 pcs. in a blister, in a cardboard box 2 or 10 blisters);
  • solution for intravenous (i / v) and intramuscular (i / m) administration: clear liquid, colorless or light yellow (5 ml in a brown glass ampoule, 5 ampoules in a plastic tray, 1 or 5 pallets in a cardboard box).

1 tablet contains:

  • active ingredients: metamizole sodium - 500 mg, pitofenone hydrochloride - 5 mg, fenpiverinium bromide - 0.1 mg;
  • auxiliary components: lactose, magnesium stearate, colloidal silicon dioxide.

1 ml of solution contains:

  • active ingredients: metamizole sodium (analgin) - 500 mg, pitofenone hydrochloride - 2 mg, fenpiverinium bromide - 0.02 mg;
  • auxiliary component: water.

Indications for use

  • symptomatic short-term therapy for neuralgia, arthralgia, myalgia, sciatica;
  • pain syndrome with spasms of smooth muscles of internal organs, including hepatic colic, biliary dyskinesia, biliary colic, spastic pain along the intestines, renal colic, algomenorrhea;
  • adjunctive treatment of pain syndrome after surgery and diagnostic procedures.

In addition, tablets, if necessary, can be used as an antipyretic agent for diseases of infectious-inflammatory or colds.

Contraindications

  • unstable and stable angina pectoris;
  • decompensation stage of chronic heart failure;
  • tachyarrhythmias;
  • severe renal or liver dysfunction;
  • oppression of bone marrow hematopoiesis;
  • deficiency of glucose-6-phosphate dehydrogenase;
  • angle-closure glaucoma;
  • hyperplasia of the prostate (with clinical manifestations);
  • intestinal obstruction, megacolon;
  • the period of pregnancy and breastfeeding;
  • hypersensitivity to pyrazolone derivatives;
  • individual intolerance to the components of the drug.

In addition, additional contraindications:

  • tablets: granulocytopenia, lactose intolerance, lactase deficiency, glucose-galactose malabsorption syndrome, age up to 8 years;
  • solution: acute intermittent porphyria, collapse, age up to 3 months of life or body weight up to 5 kg.

Method of administration and dosage

The tablets are taken orally, preferably after meals.

The dose and period of treatment is prescribed by the attending physician based on clinical indications, taking into account the patient's age.

Recommended dosage:

  • patients over 14 years old: 1–2 pcs. 3-4 times a day no more than 5 days. The daily dose should not exceed 8 pcs.;
  • adolescents 12-14 years old: 1-1.5 pcs. no more than 4 times a day;
  • children 8-11 years old: 1 pc. no more than 4 times a day.

Solution for i / v and i / m administration

Spazgan solution is intended for parenteral intravenous slow (1 ml per minute) or intramuscular injection; the ampoule should be warmed in the hand before administration.

The solution should not be mixed in the same syringe with other drugs.

The dose and duration of use is prescribed by the doctor based on clinical indications, taking into account the age and weight of the patient.

For patients over 15 years of age with acute colic, 2 ml is administered intravenously; to achieve a sufficient therapeutic effect, the procedure can be repeated in the same dose after 6-8 hours. The intramuscular drug is prescribed in a dose of 2 ml 2 times a day. The maximum daily dose is 4 ml.

Recommended single dosage for children:

  • 12-15 years old with a body weight of 46-53 kg: i / m - 0.8-1 ml or i / v - 0.8-1 ml;
  • 8-12 years old with a weight of 31-45 kg: i / m - 0.6-0.7 ml or i / v - 0.5-0.6 ml;
  • 5-7 years old with a weight of 24-30 kg: i / m - 0.4-0.5 ml or i / v - 0.3-0.4 ml;
  • 3-4 years old, weighing 16-23 kg: i / m - 0.3-0.4 ml or i / v - 0.2-0.3 ml;
  • 1-2 years with a weight of 9-15 kg: i / m - 0.2-0.3 ml or i / v - 0.1-0.2 ml;
  • 3-11 months of life with a weight of 5-8 kg: i / m - 0.1-0.2 ml, i / v administration is contraindicated.

If necessary, repeated administration is indicated at the same dose. In the treatment of pain syndrome, the duration of the drug should not exceed 5 days.

Side effects

Against the background of the use of tablets and Spazgan's solution, dry mouth, allergic reactions, malignant exudative erythema (including Stevens-Johnson syndrome), burning sensation in the epigastric region, headache, dizziness, tachycardia, lowering blood pressure, cyanosis, urine staining in red color, toxic epidermal necrolysis (Lyell's syndrome).

Long-term therapy can cause a violation of the hematopoietic system in the form of leukopenia, thrombocytopenia, agranulocytosis.

In addition, the use of the solution can cause the following side effects:

  • possibly: the risk of developing bronchospasm in prone patients;
  • rarely: usually with the appointment of high doses or prolonged therapy, renal dysfunction occurs: anuria, oliguria, interstitial nephritis, proteinuria;
  • very rare: decreased sweating, paresis of accommodation, difficulty urinating;
  • local reactions: with intramuscular injection, an infiltrate may appear at the injection site.

special instructions

The use of the solution is contraindicated in acute abdominal pain of unknown etiology.

Parenterally, the drug is recommended to be used to provide emergency care for renal or hepatic colic and if absorption from the gastrointestinal tract is impaired or ingestion is impossible.

When treating patients prone to arterial hypotension (whose systolic pressure is below 100 mm Hg), with impaired liver or kidney function, individual hypersensitivity to non-steroidal anti-inflammatory drugs or non-narcotic analgesics, care should be taken and medical supervision should be provided to them.

Due to the risk of a significant decrease in blood pressure with the introduction of 2 ml of solution or more, intravenous injection should be carried out in the supine position, monitoring blood pressure, heart rate and respiration.

If the drug is used for more than seven days, monitoring of the functional state of the liver and the picture of peripheral blood (including the content of leukocytes) is required.

In case of unmotivated fever, chills, sore throat, difficulty swallowing, stomatitis, the development of proctitis or vaginitis, and the appearance of other symptoms of side effects, the drug should be discontinued and consult a doctor.

During the period of Spazgan's use, you should not drink alcohol or take ethanol-containing drugs.

The drug should be taken with caution in patients with complete or incomplete combination of bronchial asthma, recurrent polyposis of the nose and paranasal sinuses.

With strict adherence to therapeutic doses, as a rule, the drug does not cause undesirable effects in the patient.

During the period of treatment, it is recommended to refrain from driving vehicles and mechanisms.

Drug interactions

With the simultaneous use of Spazgan:

  • non-narcotic analgesics - can cause mutual amplification of toxic effects;
  • allopurinol, tricyclic antidepressants, oral contraceptives - increase the toxicity of metamizole sodium, disrupting its metabolism in the liver;
  • barbiturates, inducers of liver microsomal enzymes (phenylbutazone) - reduce the effect of sodium metamizole;
  • cyclosporine - reduces its level of concentration in the blood;
  • tranquilizers, sedatives - enhance the effect of sodium metamizole;
  • H 1 -histamine blockers, butyrophenones, phenothiazines, quinidine, amantadine - can enhance the M-cholinolytic effect;
  • ethanol - promotes the development of mutual enhancement of effects;
  • chlorpromazine and other phenothiazine derivatives - can cause the development of severe hyperthermia;
  • glucocorticosteroids, oral hypoglycemic agents, indomethacin, indirect anticoagulants - are displaced by sodium metamizole from the connection with plasma proteins and can increase the severity of their action;
  • cytostatics, thiamazole - increase the likelihood of developing leukopenia;
  • codeine, propranolol, H 2 -histamine blockers - slow down the process of inactivation of sodium metamizole and enhance the effect of the drug.

The combination of Spazgan with X-ray contrast agents and colloidal blood substitutes is contraindicated.
