Ringer's solution is a means to restore electrolyte balance in case of blood loss and other shock conditions.

pharmachologic effect

The drug replenishes fluid loss, promotes detoxification, and restores the body's electrolyte balance. Effectively, but short-term, it replenishes the circulating blood volume (BCC), since it quickly leaves the extravascular space. The action of Ringer's solution to restore the BCC lasts no more than 30-40 minutes, which must be taken into account when providing emergency assistance.

The detoxification effect of this agent is based on the effect of thinning and increasing blood volume, which helps to reduce the concentration of toxins in the blood.

Ringer's solution composition

The Ringer's solution contains the following compounds (based on 1 liter of solution):

Active substances:

• Sodium chloride (NaCl) - 8.6 g;
• Calcium chloride (CaCl ₂) - 0.33 g;
• Potassium chloride (KCl) - 0.3 g.

Excipients:

• Sodium hydroxide (NaOH);
• Hydrochloric acid (HCl);
• Water for injections.

In terms of chemical units, the composition of Ringer's solution (1L) contains ions:

• Sodium (Na +) - 147 mmol;
• Potassium (K +) - 4 mmol;
• Calcium (Ca2 +) - 2.25 mmol;
• Chlorine (Cl-) - 155.6 mmol.

Theoretical osmolarity of the substance: 309 mosmol / l.

Release form

The drug is a colorless transparent solution; in case of turbidity and / or precipitation, the agent cannot be used.

Available in the following forms:

Indications for the use of Ringer's solution

In accordance with the instructions, Ringer's solution is intended for use in the practice of providing emergency care in the following conditions:

• Collapse, shock of various etiologies;
• Thermal injuries (burns, frostbite);
• Electrical injury;
• Intestinal fistula;
• Intestinal obstruction;
• Poisoning of any etiology, accompanied by persistent vomiting;
• Severe course of acute intestinal infections with severe diarrheal syndrome, followed by the development of dehydration of the body;
• Acute peritonitis;
• Therapeutic plasmapheresis;
• Alkalosis of metabolic origin, accompanied by dehydration.

Instructions for the use of Ringer's solution

This medicinal substance is intended for parenteral administration. In accordance with the instructions, Ringer's solution is injected intravenously, setting the rate to an average of 60-80 drops per minute (250 ml / h), or jet (during emergency rehydration assistance).

The daily dose for children is set at the rate of 5 to 10 ml per 1 kg of body weight, the rate of administration is from 30 to 60 drops per minute, according to emergency indications, the dosage can be increased to 20-30 ml / kg.

The daily dose for adults is at the rate of 10-20 ml per 1 kg of body weight, according to emergency indications it can be increased to 30-50 ml / kg.

In general, the daily dose ranges from 2 to 6% of body weight.

The course of treatment is set on an individual basis, on average it is 3 to 5 days.

special instructions

• If long-term use of Ringer's solution is necessary, control of urine output and blood plasma electrolytes is required;
• If necessary, the jet injection of a large volume of this medicinal substance requires monitoring of plasma electrolytes and blood pH (risk of acidosis);
• Possible undesirable effects with other medicinal substances, for example NSAIDs, cardiac glycosides, therefore, if it is necessary to use Ringer's solution and other drugs in parallel, the doctor should be informed about the therapy.

Side effects

When used in accordance with the instructions, Ringer's solution is well tolerated. Side effects of the drug are mainly associated with its overdose. The development of the following undesirable effects is possible:

• Hypokalemia (decrease in plasma potassium levels);
• Hyperhydration;
• Allergic reactions.

To eliminate side effects, it is usually sufficient to reduce the dosage of the drug, in case of allergy development, interrupt the administration of Ringer's solution and carry out desensitizing measures.

Contraindications for use

Ringer's solution is contraindicated in the presence of the following pathology:

• Increased individual sensitivity to the drug;
• Hypernatasia (increased plasma sodium levels);
• Hyperchloremia (increased plasma chlorine levels);
• Cerebral edema;
• Pulmonary edema;
• Renal failure;
• Heart failure in the stage of decompensation;
• Acidosis;
• Taking glucocorticosteroids.

During pregnancy and lactation, the use of Ringer's solution is possible strictly for health reasons, there is no information about its safety, as well as about its toxicity in these conditions.

Storage rules and periods

Store at an air temperature of 15-25 ° C, out of direct sunlight, in an airtight container for 2 years. Keep out of the reach of children.
