Release form and composition

The drug is available in the form of a syrup: pink liquid, transparent, has a sweet taste and fruity smell (100 or 200 ml in brown glass bottles, in a cardboard box 1 bottle and instructions for use of Remebrox).

5 ml of syrup contains:

• active substance: ambroxol hydrochloride - 30 mg;
• auxiliary components: propylene glycol, propyl parahydroxybenzoate, methyl parahydroxybenzoate, disodium edetate, menthol, citric acid monohydrate, crimson dye (Ponso 4R), sorbitol, blackcurrant flavor, purified water.

Pharmacological properties

Pharmacodynamics

Remebrox is a mucolytic drug with an expectorant effect. Its active ingredient, ambroxol, is an active metabolite of bromhexine and has a secretolytic, secretomotor and expectorant effect. It potentiates the activity of serous cells of the glands and bronchial mucosa. In the alveoli and bronchi, it increases the content of mucous secretions and the release of surfactant (surfactant). Promotes restoration of the disturbed ratio of serous and mucous components of sputum. As a result of the activation of hydrolyzing enzymes and an increase in the release of lysosomes from Clarke cells, the viscosity of the sputum decreases. The motor function of the cilia of the ciliated epithelium increases, the mucociliary transport of sputum increases.

After taking the syrup inside, the therapeutic effect is observed after 0.5 hours. Depending on the size of the dose taken, it can persist from 6 to 12 hours.

Pharmacokinetics

After oral administration, Ambroxol is absorbed from the gastrointestinal tract (GIT) almost completely. Bioavailability is 70-80%. The maximum concentration in blood plasma is reached after 2 hours.

Plasma protein binding - 80%.

It overcomes the blood-brain and placental barriers, penetrates into breast milk.

Ambroxol is metabolized in the liver to form dibromantranilic acid and glucuronic conjugates.

T _1/2 (half-life) is 7-12 hours. In severe chronic renal failure, T _1/2 increases, with impaired liver function - remains unchanged.

The drug is excreted mainly through the kidneys: 90% - in the form of water-soluble metabolites, 5% - in unchanged form.

Indications for use

The use of Remebrox is indicated in the treatment of the following respiratory diseases, accompanied by the production of viscous sputum:

• acute or chronic bronchitis;
• pneumonia;
• bronchial asthma with difficult sputum discharge;
• chronic obstructive pulmonary disease;
• bronchiectasis.

Contraindications

Absolute:

• fructose intolerance, sucrose or isomaltase deficiency, glucose or galactose malabsorption syndrome;
• I trimester of pregnancy;
• breast-feeding;
• age up to 6 years;
• hypersensitivity to the components of the drug.

Care should be taken to take Remebrox syrup for cough in renal and / or hepatic failure, gastric ulcer and duodenal ulcer, in the II and III trimesters of pregnancy.

Remebrox, instructions for use: method and dosage

The syrup is taken orally after a meal with plenty of liquid to enhance the mucolytic effect of the drug.

Recommended dosage:

• adults, children over 12 years old: 5 ml (1 teaspoon) 2-3 times a day;
• children aged 6 to 12 years: 2.5 ml (½ teaspoon) 3 times a day.

The duration of the course of treatment depends on the clinical indications and the course of the disease.

If it is necessary to use the drug for more than 5 days, medical supervision is required.

Side effects

• from the immune system: allergic reactions (including skin rash, urticaria, angioedema); very rarely - contact allergic dermatitis, anaphylactic shock;
• others: rarely - dryness of the mucous membrane of the mouth and respiratory tract, weakness, increased salivation, headache, rhinorrhea, exanthema, impaired urination (dysuria), constipation; with prolonged use of high doses - nausea, vomiting, gastralgia.

Overdose

Symptoms: dyspepsia, nausea, vomiting, diarrhea.

Treatment: during the first two hours after an overdose, immediate gastric lavage or provocation of artificial vomiting is indicated, in the future - the appointment of symptomatic therapy. It is advisable to use fatty foods.

During the period of Remebrox's use, patients are not recommended to engage in breathing exercises.

Patients with diabetes mellitus should take into account that in 5 ml of syrup, the amount of sorbitol is equivalent to 0.2 XE (bread unit).

With bronchial asthma, taking syrup can intensify the cough.

Do not take the drug just before bedtime.

Application during pregnancy and lactation

The use of Remebrox is contraindicated during the first trimester of pregnancy.

If it is necessary to take ambroxol during lactation, breastfeeding for the period of treatment of the mother should be stopped.

Pediatric use

The use of Remebrox syrup for the treatment of children under the age of 6 is contraindicated.

With impaired renal function

Drug interactions

• amoxicillin, erythromycin, cefuroxime, doxycycline: the concentration in the lung tissue of each of these antibiotics increases.
