Release form and composition
Xonef is produced in the form of eye drops: a solution from colorless to light yellow in color with a transparent structure (5 ml in plastic dropper bottles, in a cardboard box 1 dropper bottle) and instructions for use of Xonef.
1 ml of solution contains:
- active substance: betaxolol (in the form of betaxolol hydrochloride) - 5 mg;
- auxiliary components: hypromellose, benzalkonium chloride, sodium chloride, disodium edetate, sodium hydroxide, water for injection.
Pharmacological properties
Pharmacodynamics
Xonef is an antiglaucoma drug. Its active ingredient, betaxolol, is a selective beta 1 -adrenergic blocker. When applied topically, it reduces the production of intraocular fluid, helping to reduce intraocular pressure. Does not possess internal sympathomimetic activity and local anesthetic (membrane stabilizing) action.
Unlike other beta-blockers, betaxolol does not cause a decrease in blood flow in the optic nerve. In comparison with miotic drugs, its use is not accompanied by the occurrence of accommodation spasm, miosis, hemeralopia.
After a single instillation, the hypotensive effect occurs after 0.5 hours, the maximum therapeutic effect is achieved after 2 hours, and the effect on ophthalmotonus lasts 12 hours.
Pharmacokinetics
The high lipophilicity of betaxolol allows it to penetrate well into the anterior chamber of the eye through the cornea. The maximum concentration (C max) of betaxolol in the anterior chamber is reached in 1/3 h after instillation and is 1.17 mg / ml.
The drug enters the systemic circulation.
The half-life (T 1/2) from intraocular fluid is about 1 hour, from blood plasma - 16-22 hours.
Betaxolol is excreted mainly through the kidneys.
Indications for use
It is recommended to use drops of Xonef to reduce intraocular pressure in open-angle glaucoma or ocular hypertension, as monotherapy or as part of a complex treatment with other drugs.
Contraindications
Absolute:
- hypotension;
- severe heart failure;
- sinus bradycardia;
- atrioventricular block (AV block) of II – III degree;
- cardiogenic shock;
- age up to 18 years;
- hypersensitivity to the components of the drug.
Xonef should be used with caution in thyrotoxicosis, mild and moderate heart failure, grade I AV block, myasthenia gravis, diabetes mellitus, pheochromocytoma, Raynaud's syndrome, chronic obstructive bronchitis, bronchial asthma, and also simultaneously with psychotropic drugs.
During pregnancy or breastfeeding, the use of Xonef is allowed only in cases where, in the opinion of the doctor, the intended effect of therapy for the mother is higher than the potential risk for the fetus or child.
Xonef, instructions for use: method and dosage
Xonef eye drops are intended for topical use by instillation into the conjunctival sac.
Recommended dosage: 1-2 drops, 2 times a day. It may take some time for intraocular pressure to stabilize, so it should be monitored regularly during the first 30 days.
In the absence of a sufficient clinical effect from monotherapy, treatment should be supplemented with the appointment of other agents.
After opening the bottle, the drops are suitable for use within 45 days.
Side effects
- on the part of the organ of vision: often - lacrimation, short-term discomfort in the eyes after instillation; in some cases - redness and / or a feeling of dry eyes, itching, decreased sensitivity of the cornea, punctate keratitis, anisocoria, photophobia, photophobia, allergic reactions;
- on the part of the cardiovascular system: rarely - impaired cardiac conduction, bradycardia, heart failure (including fatal);
- from the central nervous system: rarely - nausea, drowsiness, headache, dizziness, insomnia, increased symptoms of myasthenia gravis, depression;
- from the respiratory system: rarely - dyspnea, respiratory failure, bronchial asthma, bronchospasm (including fatal).
Overdose
Against the background of an overdose of beta 1 -adrenergic blockers, including betaxolol, a decrease in blood pressure (BP), the development of bradycardia, acute heart failure is possible.
If a large amount of solution gets into the eyes, they should be immediately rinsed with warm running water. Careful monitoring of the patient's condition and the appointment of symptomatic therapy are recommended.

It should be borne in mind that the use of beta-blockers in patients with diabetes mellitus can hide the symptoms of acute hypoglycemia, and therefore the drug is prescribed to diabetic patients with caution.
With thyrotoxicosis, Xonef can mask tachycardia and other manifestations of thyroid hyperfunction. In case of suspicion of thyrotoxicosis, the drug should not be abruptly canceled, this can cause an increase in symptoms.
The use of Xonef may be accompanied by a state of general weakness, diplopia or ptosis, symptoms similar to those of myasthenia gravis.
Betaxolol in ophthalmic dosage form does not affect the function of external respiration. But in patients with severe respiratory system disorders, the likelihood of developing hypersensitivity to the drug cannot be ruled out, so they should be careful.
Before conducting a major elective surgery, 48 hours before the start of general anesthesia, it is necessary to begin gradual withdrawal of the beta-blocker, this will avoid a decrease in the sensitivity of the myocardium to sympathetic stimulation.
When treating patients who have a history of anaphylactic reactions or atopy, there is a risk of lack of sensitivity to usual doses of epinephrine (adrenaline) when stopping anaphylaxis resulting from the use of Xonef.
Despite the fact that Xonef drops have a minimal effect on heart rate and blood pressure, patients with mild to moderate heart failure or grade I AV block should be careful and stop using the drug immediately when the first symptoms of cardiovascular disorders appear.
Due to the fact that benzalkonium chloride, which is part of the solution, can accumulate on soft contact lenses and have a toxic effect on the eye tissue, it is recommended to remove contact lenses before each instillation procedure. They can be installed back no earlier than 1/3 hour after instillation.
Influence on the ability to drive vehicles and complex mechanisms
Patients who, when using drops, reduce the clarity of vision, it is recommended to temporarily, until it is restored, refrain from driving and performing other potentially hazardous activities that require increased reaction and attention.
Application during pregnancy and lactation
Due to the lack of sufficient experience in the use of Xonef during pregnancy and lactation, it is indicated in exceptional cases, provided that the expected benefit from therapy for the mother will exceed the potential threat to the fetus or child.
Pediatric use
The use of Xonef is contraindicated in children and adolescents under the age of 18 years.
Drug interactions
- oral beta-blockers: increase the risk of local and systemic adverse events due to an additive effect. If it is necessary to use beta-blockers in the form of a combination of drugs for oral administration and drops, patients should be under medical supervision;
- reserpine: it should be borne in mind that drugs that deplete the reserves of catecholamines, in combination with betaxolol, can lower blood pressure and cause bradycardia;
- muscle relaxants, hypoglycemic agents: it is possible to enhance their pharmacological action;
- sympathomimetics: betaxolol enhances their vasoconstrictor effect;
- psychotropic drugs: the simultaneous use of Xonef increases the risk of increasing their effect on the mental functions of the body.
