Xamiol is a drug for the treatment of psoriasis.

Release form and composition

The dosage form of Ksamiol is a gel for external use: from almost white to colorless, almost transparent, with a possible thin oily film on the surface (in a polyethylene bottle 15, 30 or 60 g, in a cardboard box 1 bottle).

Composition of 1000 mg gel:

  • active substances: betamethasone - 0.5 mg (in the form of betamethasone dipropionate - 0.643 mg), calcipotriol - 0.05 mg (in the form of calcipotriol monohydrate - 0.052 mg);
  • excipients: liquid paraffin (contains alpha-tocopherol - about 10 ppm) - 820 mg, polypropylene glycol stearate (contains butylhydroxytoluene - 0.1%) - 160 mg, hydrogenated castor oil - 20 mg.

Pharmacological properties

Pharmacodynamics

The active substance of Xamiol, betamethasone, belongs to glucocorticosteroids (GCS) for external use. It has anti-inflammatory, antipruritic, vasoconstrictor and immunosuppressive effects. However, there is currently no description of the exact mechanism of the anti-inflammatory action of GCS when applied topically. Better penetration of betamethasone into the skin and enhancement of its action is observed when using occlusive dressings.

Another active component of the gel, calcipotriol, is a synthetic analogue of the active metabolite of vitamin D. The substance inhibits the proliferation of keratinocytes and helps to accelerate their morphological differentiation.

Pharmacokinetics

When Xamiol gel is applied to intact skin, the absorption of betamethasone and calcipotriol is less than 1%. If the drug is applied under occlusive dressings on psoriatic plaques, the absorption of external corticosteroids increases. During the application of the gel, a depot of the active components of the drug is created in the skin, therefore it is eliminated from the skin for a long time - within several days. Metabolism of betamethasone occurs in the kidneys and in the liver with the formation of sulfoesters and glucuronides, the substance is excreted in the urine and feces.

Indications for use

  • psoriasis of the scalp;
  • psoriasis of various parts of the body of mild to moderate severity.

Contraindications

  • diseases and / or conditions accompanied by impaired calcium metabolism;
  • severe renal / liver dysfunction;
  • psoriatic erythroderma;
  • pustular, exfoliative, guttate psoriasis;
  • fungal, viral (shingles, herpes), parasitic and bacterial skin infections, rosacea, rosacea, acne vulgaris, perioral dermatitis, skin atrophy, wounds, ulcers, itching in the genital area and in the perianal region, skin manifestations of syphilis and tuberculosis, striae, ichthyosis, increased fragility of skin vessels;
  • pregnancy, lactation period;
  • age up to 18 years;
  • increased individual sensitivity to any component of the drug.

Instructions for the use of Xamiol: method and dosage

Ksamiol gel is used externally. Shake the bottle before use.

The drug is applied in a thin layer to the affected areas of the scalp and / or to psoriatic plaques of other parts of the body once a day. The maximum daily dose should not exceed 15 g. The maximum weekly dose is up to 100 g.

The recommended duration of treatment for psoriasis of the scalp is 4 weeks, for psoriasis of other parts of the body - 8 weeks. The area of application of the gel should not exceed 30% of the body surface. For optimal effect, the drug should be left on the skin for 12 hours.

A repeated course of therapy with Xamiol is possible under the supervision of a doctor.

Side effects

Side effects from systems and organs and their frequency according to a special classification [very often (≥ 1/10), often (from ≥ 1/100 to <1/10), infrequently (from ≥ 1/1000 to <1 / 100), rarely (from ≥ 1/10 000 to <1/1000), very rarely (<1/10 000)]:

  • skin and skin appendages: often - itching; infrequently - burning sensation, rash, irritation, pain, folliculitis, exacerbation of psoriasis, dermatitis, pustular psoriasis, acne, dry skin. Long-term external use of betamethasone can cause skin atrophy, striae, telangiectasia, folliculitis, hypertrichosis, allergic contact dermatitis, perioral dermatitis, depigmentation, colloidal degeneration of the skin, as well as the development of generalized pustular psoriasis. The use of calcipotriol can lead to local irritation of the skin, cause itching, tingling / burning sensation, dry skin, dermatitis, rash, erythema, exacerbation of psoriasis, eczema, photo- and hypersensitivity reactions, including very rare cases of facial edema and angioedema;
  • eyes: infrequently - eye irritation;
  • systemic reactions: rarely (associated with the use of betamethasone, especially long-term, on large surfaces and with the use of occlusive dressings) - cataract, increased intraocular pressure, suppression of the function of the adrenal cortex, infections; very rarely (associated with the use of calcipotriol) - hypercalciuria or hypercalcemia. More often, systemic reactions develop when the gel is applied under occlusive dressings, on skin folds, thin skin, as well as with prolonged application to large areas of the skin.

Overdose

In case of an overdose of Xamiol, the following symptoms may appear: an increase in the level of calcium concentration in the blood (with rapid recovery after discontinuation of the drug), inhibition of the function of the pituitary-adrenal system with the occurrence of reversible secondary adrenal insufficiency.

Treatment consists in discontinuation of the drug and symptomatic therapy. In chronic toxicity, betamethasone should be withdrawn gradually.

special instructions

Care should be taken when applying Xamiol to the face and genitals. The drug should not be applied to the mucous membranes. In case of accidental contact of the gel with the eyes, such undesirable side effects as conjunctivitis, irritation of the eyes and skin of the face are possible. In such cases, rinse your eyes with running water.

Hands should be washed after using the gel.

Cessation of therapy for psoriasis with external GCS can cause withdrawal syndrome and the development of generalized pustular psoriasis. In this regard, after discontinuation of Xamiol, clinical observation of the patient should be continued.

In the event of a secondary infection, antimicrobial therapy is necessary.

During the entire period of treatment with the drug, it is recommended to avoid or limit excessive natural and artificial ultraviolet radiation.

Xamiol gel contains butylhydroxytoluene (E321), which can irritate mucous membranes, eyes and local undesirable skin reactions (for example, contact dermatitis).

Application during pregnancy and lactation

Xamiol can be used during pregnancy only in cases where the expected benefit to the woman is higher than the likely risk to the fetus.

If you need to use the drug during lactation, breastfeeding should be discontinued.

Pediatric use

According to the instructions, Xamiol is contraindicated in children and adolescents under 18 years of age.

With impaired renal function

Prescribing the drug to patients with severe renal failure is prohibited.

In severely impaired liver function, the use of Xamiol gel is contraindicated.
