Release form and composition

Dosage forms of Claridol:

  • tablets: round, flat, with beveled edges, almost white / white, with a break line on one side (7, 8, 10 and 16 pcs. in blisters, in a cardboard box 1, 2, 3, 4, 5 or 10 blisters; 10 pcs. In strips, in a cardboard box 1, 2, 3, 4, 5 or 10 strips);
  • syrup: clear orange liquid with a pleasant aroma and sweet taste (50, 60, 100 and 120 ml each in amber glass bottles, 1 bottle in a cardboard box).

Composition of 1 tablet:

  • active substance: loratadine - 10 mg;
  • auxiliary components: sodium methylparaben, sodium starch glycolate, magnesium stearate, anhydrous colloidal silicon dioxide, microcrystalline cellulose, purified talc, starch, purified water.

Composition of 1 ml of syrup:

  • active substance: loratadine - 1 mg;
  • auxiliary components: sodium propylparaben, disodium edetate, sodium benzoate, propylene glycol, sodium methylparaben, sodium saccharin, citric acid, sorbitol solution, sucrose, mixed fruit flavor American ice cream, Sun Sunset dye, purified water.

Pharmacological properties

Pharmacodynamics

Loratadine is a long-acting H 1 -histamine receptor blocker. It has antiallergic, antipruritic and antiexudative activity. Inhibits the release of leukotriene C4 and histamine from mast cells. Reduces capillary permeability, relieves spasms of smooth muscles, prevents the development of tissue edema. Prevents the development of allergic reactions and facilitates their course.

Antiallergic effect of Claridol develops within 30 minutes after taking the drug, reaches a maximum after 8-12 hours, lasts 24 hours.

Loratadine practically does not penetrate the blood-brain barrier, therefore it is not addictive and does not affect the central nervous system.

Pharmacokinetics

After oral administration of Claridol, loratadine is rapidly and completely absorbed in the gastrointestinal tract. The maximum concentration (Cmax) reaches within 1.3–2.5 hours. With simultaneous food intake, this time increases by 1 hour. In elderly people, Cmax is 50% higher, with alcoholic liver damage, Cmax increases depending on the severity of the disease.

Loratadine is 97% bound to plasma proteins. It is metabolized in the liver mainly with the participation of cytochrome P450 isoenzymes CYP3A4, to a lesser extent - CYP2D6. The active metabolite is descarboethoxyloratadine.

The equilibrium plasma concentration of loratadine and its metabolite is reached on the fifth day after administration.

The drug does not penetrate the blood-brain barrier.

The half-life of loratadine averages 8.4 hours (from 3 to 20 hours), for descarboethoxyloratadine, an average of 28 hours (from 8.8 to 92 hours), in elderly patients, respectively, an average of 18.2 hours (from 6.7 up to 37 hours) and 17.5 hours (from 11 to 38 hours). With alcoholic liver damage, this period increases depending on the severity of the disease.

The drug is excreted in the bile and kidneys.

In chronic renal failure and during hemodialysis, the pharmacokinetics of loratadine practically does not change.

Indications for use

  • allergic itchy dermatoses;
  • allergic rhinitis (seasonal or perennial);
  • allergic reactions to insect bites;
  • itching of various etiologies;
  • urticaria (including chronic idiopathic);
  • hay fever;
  • conjunctivitis;
  • Quincke's edema;
  • pseudo-allergic reactions.

Contraindications

  • children under 2 years old;
  • period of pregnancy and lactation;
  • the presence of hypersensitivity to any of the components of the drug.

According to the instructions, Claridol should be used with caution in the presence of liver failure.

Instructions for the use of Claridol: method and dosage

Claridol is taken orally.

Recommended dosage:

  • adults, children over 12 years old and children weighing> 30 kg: 10 mg [1 tablet or 10 ml (2 teaspoons) of syrup] 1 time per day;
  • children 2–12 years old: 5 mg [½ tablet or 5 ml (1 teaspoon) of syrup] 1 time per day.

For patients with impaired hepatic function, the drug is prescribed in a daily dose of 5 mg.

Side effects

  • adults: dry mouth, gastrointestinal disorders (nausea, vomiting, gastritis), fatigue, drowsiness, headache, allergic reactions (rash, anaphylaxis); in rare cases - palpitations, alopecia, tachycardia, liver dysfunction, anaphylaxis;
  • children: rarely - sedation, nervousness, headache.

The reported adverse events occurred with a frequency of ≥ 2%, which is approximately comparable to that with placebo.

Overdose

Loratadine overdose is manifested by headache, drowsiness, tachycardia. It is recommended to induce vomiting, gastric lavage, and take activated charcoal.

Drug interactions

Cimetidine, erythromycin and ketoconazole, with simultaneous use, increase the concentration of loratadine in the blood plasma, however, there are no clinical manifestations and changes in the electrocardiogram.

The effectiveness of loratadine is reduced by ethanol and inducers of microsomal oxidation (tricyclic antidepressants, rifampicin, barbiturates, phenylbutazone, phenytoin, zixorin).
