Release form and composition
Lazolvan MAX dosage form - capsules of prolonged action: hard gelatinous, oblong, orange, with the symbol of the manufacturer's company (Boehringer Ingelheim); the capsule cap is red and the inscription "MUC 01" is white. The contents of the capsule are yellowish-white shiny granules, a small amount of powder.
Composition of 1 capsule:
- active substance: ambroxol hydrochloride - 75 mg;
- auxiliary components: crospovidone, carnauba wax, stearyl alcohol, magnesium stearate;
- capsule: gelatin, purified water, titanium dioxide, dyes - iron oxide red and yellow, ink (shellac, titanium dioxide).
Pharmacological properties
Pharmacodynamics
Ambroxol has the ability to increase the secretion of the respiratory tract, enhance the production of pulmonary surfactant and stimulate ciliary activity. This increases the flow and transport of mucus (mucociliary clearance), which promotes better sputum discharge and relieves coughing.
With prolonged (at least 60 days) use of the drug by patients with chronic obstructive pulmonary disease, a significant decrease in the number and duration of exacerbations and the number of days of antibiotic therapy was noted.
Pharmacokinetics
Ambroxol is rapidly and almost completely absorbed. The maximum plasma concentration after taking one capsule is achieved in 6.5 hours. The relative bioavailability of the drug is 95%. The connection with plasma proteins is 90%.
When taken orally, Ambroxol quickly passes from the blood to the tissues. Its volume of distribution is 552 liters. The highest concentration is observed in the lungs. 70% of the dose of ambroxol is metabolized in the liver, mainly by glucuronidation and by partial degradation. About 30% of the drug is exposed to the effect of the first pass through the liver. Ambroxol is metabolized to dibromantranilic acid mainly through the CYP3A4 isoenzyme.
The thermal half-life is approximately 10 hours. The total clearance is 660 ml / min. 83% of the single dose taken is excreted in the urine within 5 days. Age and gender have no effect on the pharmacokinetics of the drug.
Indications for use
According to the instructions, Lazolvan MAX is indicated for the treatment of the following respiratory diseases, accompanied by the separation of viscous sputum:
- pneumonia;
- acute and chronic bronchitis;
- chronic obstructive pulmonary disease;
- bronchiectasis;
- bronchial asthma, accompanied by obstructed sputum discharge.
Contraindications
Absolute contraindications:
- age up to 18 years;
- pregnancy (I trimester) and lactation;
- increased sensitivity to the components of the drug.
Relative contraindications:
- renal failure;
- liver failure;
- pregnancy (II – III trimesters).
Instructions for use of Lazolvan MAX: method and dosage
Lazolvan MAX capsules are taken orally, regardless of food intake, whole, without opening or chewing, drinking plenty of liquid.
It is recommended to take 1 capsule of prolonged action per day.
If the symptoms of the disease persist within 4-5 days of taking the drug, it is recommended to consult a doctor.
Side effects
- gastrointestinal tract: nausea, vomiting, dyspepsia, abdominal pain;
- skin, subcutaneous tissues and the immune system: rash, urticaria, itching, hypersensitivity reactions, angioedema, anaphylactic reactions (including anaphylactic shock).
Overdose
In case of an overdose of Lazolvan MAX, there may be side effects of the drug, such as nausea, vomiting, dyspepsia, diarrhea, abdominal pain.
Recommended treatment: gastric lavage within 1-2 hours after taking the drug, symptomatic therapy.
special instructions
There are isolated reports of cases of the development of Stevens-Johnson syndrome and toxic epidermal necrolysis during the period of treatment with expectorants. In this regard, if signs of these diseases appear and worsen (lesions of the skin and mucous membranes, fever, body pain, rhinitis, cough, sore throat), the drug should be discontinued and a doctor should be consulted.
Influence on the ability to drive vehicles and complex mechanisms
Studies of the effect of Lazolvan MAKS on the ability to drive vehicles and other complex mechanisms that require the speed of psychomotor reactions have not been conducted.
Application during pregnancy and lactation
Ambroxol is able to penetrate the placental barrier. The studies did not reveal an undesirable effect of the drug on pregnancy, embryonic, fetal, postnatal development and childbirth. Numerous data on its use after the 28th week of pregnancy did not reveal a negative effect on the fetus. However, in the first trimester, Lasolvan MAX is contraindicated. Its use in the II and III trimesters is possible only when the expected benefit to the mother outweighs the possible risk to the fetus.
Ambroxol may be excreted in breast milk, and therefore the drug is contraindicated for use during lactation.
Pediatric use
Lazolvan MAX is contraindicated for use in pediatrics.
With impaired renal function
In case of impaired renal function, taking the drug is permissible only as directed by a doctor. The drug is contraindicated in renal failure.
If liver function is impaired
Lasolvan MAX should not be taken by patients with hepatic impairment.
Use in the elderly
Elderly patients do not need dose adjustment.
Drug interactions
When ambroxol is taken together with amoxicillin, cefuroxime and erythromycin, the penetration of these drugs into the bronchial secretion increases.
It is contraindicated to use Lazolvan MAX with antitussive drugs that impede the excretion of sputum.
